To provide social worker interventions which are recovery focused, safe, appropriate and effective. The care is based on comprehensive assessments, ensures continuity, is tangata whaiora and whanau centered and evidence based. The role works in collaboration with Taurawhiri (Cultural Advisor) and supports programme delivery

Purpose of the Service

:

Whitiki Maurea is a Maori clinical service within the Waitemata DHB District Mental Health Services Group. We provide clinical treatment in mental health and addictions with a Maori kaupapa (cultural values and concepts) approach.

KEY TASKS

To practice as a social worker, providing specialist social work interventions within a WDHB Adult Mental Health Community Team

EXPECTED OUTCOMES

Provide focused social work assessment and interventions with service users as per the Adult Community Mental Health Social Work Role. Social work issues for service users are highlighted to the clinical team. Social work interventions and outcomes will be recorded in clinical files, and communicated to the clinical team. Clinical practice is carried out to professional standards and legal requirements and in line with WDHB policies and procedures Triage, assessment and intervention is in accordance with written guidelines. Intake clinician role undertaken on rotational basis within the team Assessment and care planning including risk assessment and risk management plans are well documented ,safe and in accordance with service policies and protocols Work with Taurawhiri (Cultural Advisors) aspects that are recognized and incorporated into clinical practice. Contribute to team care coordination in clinical practice Provide specific recovery focused interventions to designated service users To demonstrate effective communication within the clinical team

Information is provided and referrals made to other services and agencies as relevant for addressing tangata whaiora social, occupational and other needs as appropriate

Work in partnership with tangata whaiora, whanau and significant others

Demonstrate understanding of the MoH ‘Involving Families’ Guidelines 2000. Supports involvement of and communication between the clinical team and whanau regarding client’s progress and care plans. Identify and assess the psychosocial needs of whanau / significant others. Establish liaison and advocacy role to support tangata whaiora in their relationships. Provide necessary information to assist others in meeting the tangata whaiora needs.

Establish and maintain community networks.

Identify and attend local community mental health networking groups as clinical practice allows Maintain regular contact / consultancy with NGO’s as part of integrated and coordinated practice Maintain updated information on community resources, accessible to the multidisciplinary team.
