Was sind „Me-Too-Präparate“ ? Bezeichnung für patentierte Arzneimittel-Wirkstoffe, die nur geringfügige Molekülvariationen bereits vorhandener Substanzen zumeist ohne eigene therapeutische Innovationen sind. Viele dieser Präparate sind zwar nicht besser in der Wirkung als herkömmliche Arzneimittel ohne Patentschutz, erzielen aber höhere Preise. Das Einsparvolumen bei den MeToo-Präparaten (Analogpräparate) lag für 2004 bei 1,2 Milliarden Euro. (lt. Arzneiverordnungs-Report 2005).
Was sind Analogpräparate ? Für ein neu entwickeltes Arzneimittel kann ein pharmazeutischer Hersteller in Deutschland 20 Jahre lang Patentschutz beantragen. Über den hohen Preis, den er für dieses Arzneimittel auf dem freien Markt und auch von den gesetzlichen Krankenkassen verlangen kann, deckt er seine Forschungs- und Entwicklungskosten und erzielt Gewinn. Ist der Patentschutz abgelaufen, darf dieser Wirkstoff auch von anderen Herstellern vermarktet werden - zu einem meist deutlich günstigeren Preis, da keine Forschungs- und Entwicklungskosten mehr anfallen. Der Umsatz des Originalherstellers mit dem Originalpräparat geht zurück. Der so gesetzte Anreiz für Innovationen auf dem Arzneimittelmarkt kann allerdings unterlaufen werden: Auch Arzneimittel, die lediglich Molekülvarianten bereits bekannter Wirkstoffe enthalten und pharmakologisch gleiche oder ähnliche Wirkungen besitzen wie das Originalpräparat, können patentiert werden. Die Änderungen des Wirkansatzes oder der Darreichungsform sind oft nur sehr gering, und diese Arzneimittel bedeuten oftmals keine Verbesserung der Arzneitherapie. Dennoch gelten sie als "neu" und kommen zu hohen Preisen auf den Markt. Solche Arzneimittel bezeichnet man als "Analogpräparate".

Fazit: „Me-Too“-Präparate sind alle Atypika mit Ausnahme von Amisulprid und Clozapin, d.h. hierfür gibt es gleichwertige Ersatzpräparate.

Was empfiehlt die S3-Leitline Schizophrenie der DGPPN 2006 ? Akuttherapie (12) Empfehlungsgrad B. Bei schizophrenen Ersterkrankungen sollten aufgrund der gegenüber den typischen Antipsychotika zumindest vergleichbaren Wirkung auf die Positivsymptomatik, Hinweisen auf eine überlegene Wirksamkeit bezüglich der Negativsymptomatik und geringerer dosisabhängiger extrapyramidalmotorischer Nebenwirkungen in erster Linie Atypika eingesetzt werden. Allerdings müssen hierbei die substanzspezifischen Nebenwirkungen berücksichtigt werden. (16) Empfehlungsgrad A. Bei der Behandlung der akuten schizophrenen Episode stellen atypische Antipsychotika aufgrund der geringeren Rate an extrapyramidal-motorischen Störungen bei vergleichbarer Wirksamkeit gegenüber konventionellen Antipsychotika Medikamente der ersten Wahl dar, falls nicht der Patient selbst konventionelle Antipsychotika präferiert oder er darauf bereits ohne relevante Nebenwirkungen remittierte.
Was empfiehlt die S3-Leitline Schizophrenie der DGPPN 2006 ? Beibehalten konventioneller Antipsychotika (20) Empfehlungsgrad C. Wenn unter einem konventionellen Antipsychotikum eine gute Kontrolle der Symptome erreicht wurde und eine gute Verträglichkeit und Akzeptanz seitens des Patienten besteht, sollte nicht ohne Veranlassung auf ein atypisches Antipsychotikum umgestellt werden. In jedem Fall sollte der Betroffene jedoch auf das erhöhte Risiko von Spätdyskinesien hingewiesen werden. Differentielle Wahl und Dosierung des Antipsychotikums (26) Empfehlungsgrad B. Bei vorherrschender Negativsymptomatik sollten als Medikamente der ersten Wahl atypische Antipsychotika mit erwiesener Wirkung auf Negativsymptome eingesetzt werden. (27) Empfehlungsgrad A. Zur Behandlung kognitiver Beeinträchtigungen sollten atypische Antipsychotika bevorzugt eingesetzt werden.

Empfehlungen der S3S3-Leitline Schizophrenie Langzeittherapie/Rezidivprophylaxe (33) Empfehlungsstärke C. Zur Langzeittherapie sollte dasjenige typische oder atypische Antipsychotikum beigehalten werden, unter dem eine Remission in der Akuttherapie bei guter Verträglichkeit erzielt werden konnte. (34) Empfehlungsstärke A. Bei der Auswahl des Antipsychotikums ist die überlegene rezidiv-prophylaktische Wirksamkeit der atypischen Antipsychotika als Gruppe gegenüber typischen Antipsychotika in der Langzeittherapie zu berücksichtigen. (35) Good Clinical Practice. Bei der Auswahl zwischen verschiedenen Antipsychotika in der Langzeit-therapie ist in jedem Fall das unterschiedliche Nebenwirkungsrisiko im Hinblick auf Spätdyskinesien, Sedierung, kardiale, metabolische und endokrine Effekte zu beachten. (39) Empfehlungsstärke C. Bei der Entscheidung für ein antipsychotisches Depotpräparat ist das erwartungsgemäß als günstiger einzustufende Nebenwirkungsprofil des verfügbaren atypischen Depot-Antipsychotikums Risperidon (insbesondere im Hinblick auf das geringere Risiko von Spätdyskinesien) zu berücksichtigen.

Cochrane: RIS vs. Typika In the short-term, risperidone was more likely to produce an improvement in the Positive and Negative Syndrome Scale (PANSS) when compared with haloperidol (n=2368, 9 RCTs, RR not 20% improved 0.72 CI 0.59 to 0.88 NNT 8). A similar, favourable outcome for risperidone was found in long-term studies (n=859, 2RCTs RR not 20% improved 0.51 CI 0.38 to 0.67 NNT 4;n=675 1RCT, RR not improved 40% 0.75 CI 0.66 to 0.84 NNT 5; n=675, 1 RCT, RR not 60% improved 0.90 CI 0.84 to 0.96, NNT 11). Risperidone was also more likely to reduce relapse at one year follow up, compared with haloperidol (n=367, 1 RCT, RR 0.64 CI 0.41 to 0.99, NNT 7). Less people allocated risperidone left studies before completion, both for short-term (n=3066, 16 RCTs, RR 0.76 CI 0.63 to 0.92, NNT 6) and long-term trials (n=1270, 4RCTs, RR 0.55 CI 0.42 to 0.73 NNT 4). People given risperidone had significantly fewer general movement disorders (including extrapyramidal side effects) than those receiving typical antipsychotics (n=2702, 10 RCTs, RR 0.63 CI 0.56 to 0.71, NNT 3). Significantly fewer people given risperidone used antiparkinsonian drugs (n=2524, 11 RCTs, RR 0.66 CI 0.58 to 0.74, NNT 4).

Cochrane: RIS vs. Typika Reviewers' conclusions: Risperidone may be more acceptable to those with schizophrenia than older antipsychotics and have marginal benefits in terms of limited clinical improvement. Its adverse effect profile may be better than haloperidol. With the addition of more studies to this review, the publication bias evident in previous versions is no longer a significant issue. Any marginal benefits this drug may have have to be balanced against its greater cost and increased tendency to cause side effects such as weight gain.
