Our Unwavering Commitment Message from the President & CEO Dear Unity Partners in Caring, In 2012, Unity Health Care reached a major milestone: This year, we provided vital health care and social service support for more than 100,000 men, women, and children in the District of Columbia. This is a new high in annual patients served, and it reflects Unity’s significant impact on the health and wellness of the District’s most medically and socially vulnerable residents. We pride ourselves on our ability to increase access to care, which helps build healthier communities throughout our nation’s capital. In the past year, we have: ■■ Established patient-centered medical homes that improve health outcomes. ■■ Built a new, state-of-the-art facility. ■■ Developed successful models of leadership throughout our ever-growing workforce. ■■ Maintained a solid fiscal operation, even in the midst of a fragile national economy. ■■ Maintained steadfast loyalty to our mission to provide comprehensive health and human services regardless of ability to pay. Unity provides a health care safety net for more than one of every five District residents. We are proud that more than 100,000 people walked through our doors this year and received quality health care, emotional support, and dignified service. We are among many Community Health Centers (CHCs) that have assumed an increasing role in our country’s health care landscape. CHCs now serve more than 20 million patients each year, and that number will expand to more than 30 million once the Affordable Care Act is fully implemented. We are honored to provide health care that is so critical for individuals and for our national economy. Since its founding in 1985, Unity has played an essential role as a health care provider in our nation’s capital, addressing ongoing health care needs and identifying new issues as they emerge. Today, we move forward confidently. Our dedication to providing compassionate, quality, and culturally sensitive primary health care is unwavering, as is our commitment to embracing innovation and new technologies to improve our efficiency and service. 100,000 Voices in One Year A Milestone in Patient Care
Creating a medical home for every patient Unity is committed to giving each patient a medical home — a single location where he or she can access primary care, specialty care, social services, and health and nutrition education. Patients, moreover, become partners in developing and implementing their own health care plans. This goal of creating a patient-provider collaboration is central to all of Unity’s work.
Making the most of technology Unity applies technology to providing better patient care. Both Anacostia and Parkside have wireless access throughout the buildings and laptops for their physicians. The Parkside facility also has medical equipment that electronically records a patient’s temperature, blood pressure, and other vital signs, and it automatically downloads them to the patient’s electronic medical record. This new addition makes electronic medical records, now used in every Unity facility, even more effective. This year, Unity reached the milestone of serving 100,000 patients, and it continues to expand and evolve. The organization’s work unites the best of tradition and cutting-edge advancements. The result: compassionate, quality health care delivered with outstanding medical skill and innovative medical technology. Unity’s health care providers continue to give their patients the individual attention that improves their health and changes their lives. Today, new technologies and infrastructure improvements help providers care for their patients more efficiently and effectively. These improvements are currently being used to best serve patients at Unity’s newly built facility in Anacostia and are under way at Parkside Health Center (which is under construction). The real value of these facilities, however, is that they help Unity better support its mission of promoting healthier communities. Unity embodies this mission with three commitments that are central to its work: creating a medical home for every patient, making the most of technology, and providing outstanding community service.

Providing outstanding community service The Anacostia Health Center brings a variety of services directly to the community. For example, the Women, Infants, and Children (WIC) program, now located in the Anacostia health center, gives families who live in this under-resourced Ward 8 community nutrition education and better access to fresh produce and other healthy food options. Unity’s Anacostia Health Center also provides re-entry services for recently incarcerated individuals; more than 750 patients used these services in 2012. One patient, Kevin*, was staying at a halfway house when his Unity physician, tipped off by Kevin’s high blood pressure, discovered a mass on Kevin’s adrenal gland. The doctor’s commitment to finding the source of this problem was lifechanging for Kevin, who says, “Without the dedication of the team at Unity’s Anacostia Heath Center, I would not be here right now.” The Anacostia Health Center is open from 8 a.m. to 8 p.m. most days. These extended hours decrease the use of emergency rooms and make it easier for working families to access care. *Names have been changed to protect patients’ identities.

Health Care Beyond the Clinic Unity has been serving homeless men, women, and children since it began as the Health Care for the Homeless Project in 1985. Not every patient, however, is willing or physically able to come to a health center. In those cases, Unity brings the health care to them. Dr. Catherine Crosland, Unity’s medical director for homeless outreach services, personifies this commitment. In addition to treating patients in clinics and handling administrative duties, she spends half a day each week seeing patients on sidewalks, under bridges, and anywhere else she can find them. She calls this type of outreach walking hours. Crosland begins her walking hours by providing basic health care at the Georgetown Ministry Center in the basement of Grace Church. In a tiny room there, she can check patients’ blood pressure, check sugar levels for diabetes, or look at a rash. When she finishes, she walks through several areas, including parks and underpasses, where homeless men and women tend to congregate. She seeks out patients she knows to follow up on their treatment and cares for anyone who needs help.
Bringing health care to the homeless Unity provides care to homeless individuals in a number of ways: 4 Site-based care, in which a practitioner provides service in one of Unity’s shelter-based sites or drop-in day programs. 4 Outreach care through Unity’s mobile outreach van. Practitioners will park the van near places where homeless individuals typically gather so individuals who are reluctant to go to a clinic might see the van and step in for treatment. Practitioners also leave the van so they can seek out patients and offer care. 4 Walking hours, in which a physician walks through the city, looking for homeless people who need health care. In the past four years, Unity providers have cared for more than 18,000 homeless patients, including the more than 6,200 who have been seen in an outreach setting. For some patients, outreach provides a bridge to health care provided at a center. Nearly 650 Unity patients, however, have never gone to a health center. These patients have only been seen in an outreach setting. If Unity didn’t bring health care to them, they likely would never be seen by a physician.
Crosland has become well-known among the people she serves. One day, Tom*, a man she knew well but who never had significant health needs, mentioned that he had been feeling more short of breath than usual. He also told Crosland that he had a heart murmur, but it hadn’t been checked in a while. “I encouraged him to go to our CCNV health center, but I knew he was never going to come in. It was too much for him,” Crosland recalls. “I managed to check his insurance, get a referral for him, and make an appointment with a cardiologist, all without his presence in a health care facility.” The cardiologist found that Tom had severe aortic stenosis. One of the valves of his heart was tight, so he wasn’t getting much blood pumped through his body. Tom needed to have his heart valve replaced — open-heart surgery. Moreover, the surgeon was worried that Tom, who was then drinking 15 to 20 shots a day, would go into alcohol withdrawal after the surgery. “He was nervous, so rather than waiting for him to come back to the health center, we would go back to see him every week and remind him that he needed this surgery,” Crosland explains. “Ultimately, he decided to do an outpatient taper off the alcohol on his own using the drug Librium, and he decided to recover at his sister’s house after the surgery.” Crosland arranged for the prescription and regularly followed up with Tom. She also coordinated care with his cardiologist and surgeon. Ultimately, Tom showed up for his surgery at 5:30 a.m., right on schedule, and came through the procedure well. Crosland visited him in the hospital and now sees him back on the street, fully recovered. This recovery happened because Tom knew and trusted Crosland. She was part of his community, so he told her about his shortness of breath. She took his complaint seriously and was diligent in her follow-up with Tom and his specialists. “In some ways, it was the most seamless continuity of care I’ve experienced at any health center,” Crosland says. “He lives on the street, but he had almost perfect continuity of care.” *Names have been changed to protect patients’ identities.

Unity Funding Sources As a Federally Qualified Health Center, Unity receives financial and other benefits that we pass along to our patients. These supports include cost-based reimbursements from Medicare, business-related cost savings, access to highly trained professionals, and discounted prescription drugs. Unity Health Care, Inc., is supported by the U.S. Department of Health and Human Services (HRSA) as well as by private and public funding sources, including the District of Columbia and federal grants, individuals, corporations, foundations, faith institutions, the Combined Federal Campaign, and in-kind support.
2012 support includes: ■■ A $1 million grant from the United Health Foundation to support Unity Health Care’s Center for Excellence at Congress Heights. Since 2003, Unity’s partnership with the United Health Foundation has enhanced the organization’s effort to improve access to care and reduce the risk of chronic disease and injury among the medically underserved in Ward 8.

■■ A $913,802 grant from CareFirst BlueCross BlueShield to enhance Unity’s patient-centered medical home initiative and provide Convenient Care — health care centers that are open seven days per week, including from 8 a.m. to 10 p.m. on weekdays. Two Unity health care centers, Minnesota Avenue Health Center and Upper Cardozo Health Center offer convenient care, which was created to reduce excess emergency room use. Support from CareFirst also includes a $375,000 grant to provide immunizations, digital dental exams, and other services to adults with chronic diseases. ■■ A $215,000 grant from Gilead Sciences’ HIV FOCUS to support Unity’s 5th and 6th Vital Signs initiative in Wards 7 and 8. Through the Vital Signs initiative, Unity integrates HIV and Hepatitis C testing into routine testing during primary care visits. Through this work, Unity strengthens HIV and Hepatitis C testing in the primary care setting so more patients learn their HIV and/or HCV status and can be connected to immediate care and long-term treatment. ■■ A $190,000 grant from the Alexander and Margaret Stewart Trust, which supports bringing Unity’s Youth and Families Initiative (Y-FI) to the Congress Heights Health Center as well as ongoing pediatric services at Columbia Road Health Services and programs for women and children at Stanton Road. ■■ A $175,000 donation from Teva Pharmaceuticals to support Unity Health Care’s commitment to providing care in the nation’s capital and to partially support the renovation of the Teva Walk-In Clinic at Unity’s Minnesota Avenue Health Center.

■■ $73,585 from the Summit Fund of Washington to support Unity’s Family Planning Health Education Initiative (FPHEI) at Ballou Senior High School. Support from the Summit Fund provides a family planning health educator for programs serving the District’s reproductive-age minority adolescent patients in underserved communities. ■■ A $60,000 grant from the Naomi and Nehemiah Cohen Foundation to provide long-acting reversible contraceptives to underserved women in the District of Columbia. This support enhances Unity’s efforts to reduce a large number of unwanted pregnancies and boost the self-esteem of patients as they gain control over their own fertility. ■■ A $50,000 Covidien matching grant from the National Association of Community Health Centers to support the renovation and rebuilding of the Walk-In Area at Unity’s Minnesota Avenue Health Center. ■■ A $30,000 grant from The Prince Charitable Trust to implement training software that helps medical assistants gain additional skills and assume more responsibility. This investment in the morale and continuing education of our medical assistants is directly connected to improvements in the quality, safety, and efficiency of care. ■■ A $25,000 grant from The William S. Abell Foundation to support adult immunizations and vaccine-storage refrigerators at Unity’s homeless sites. ■■ A $10,000 grant from the Verizon Foundation to provide digital dental equipment and software at Unity’s Southwest Health Center. ■■ A $10,000 donation from Pepco to support the Reach Out and Read Early Literacy Program at Unity @ DC General. ■■ A $7,500 grant from the Clark-Winchcole Foundation to support general operating services provided at Columbia Road Health Services regardless of patients’ ability to pay. This support helps ensure that some of the District’s most vulnerable residents — the uninsured, the homeless and formerly homeless, children from low-income families, and immigrants — have access to high-quality primary care.
■■ $5,000 from the George Preston Marshall Foundation to support Columbia Road Health Services’ Health Care for Children Project (HCCP). HCCP meets children’s needs from infancy through adolescence with services that focus on the prevention, early detection, and treatment of disease. ■■ $4,240 from the Association of American Medical Colleges to support Columbia Road Health Services’ Sisters and Brothers in Health wellness program as well as the We Can! (Ways to Enhance Children’s Activity & Nutrition) wellness program. The Sisters and Brothers in Health wellness program improves the overall well-being of adult patients, primarily those who face complications from obesity, hypertension, or depression. The We Can! program gives parents, caregivers, and entire communities strategies to fight obesity among children by promoting healthy weight and an active lifestyle for children and parents. ■■ $1,100 from the St. Joseph Parish Good Samaritan Fund to support vaccines for adult patients at Columbia Road Health Services.
