Performance Measures Carilion Clinic Hospice had a busy year preparing for upcoming quality initiatives. We continue to receive patient/caregiver experience data from the National Hospice and Palliative Care Organization (NHPCO), but will transition to the Hospice Consumer Assessment of Healthcare Providers and Systems (CAHPS) survey in 2015. The hospice CAHPS tool will allow a more accurate reporting of caregiver satisfaction with services provided and will provide valuable insight to our hospice staff on areas that we can improve upon. It also aligns hospice quality measurements with those used in the hospital and ambulatory settings. A new data collection tool will be implemented in 2014 to help collect patient data and prepare hospice agencies for public reporting. We have experienced significant improvement in our overall NHPCO scores in the areas listed below. In areas where our scores did not meet our target, action plans were put into place at each agency to address improvement.

These indicators show that our hospice program helps to address the physical, spiritual, and emotional needs of our patients and their families.

Goals and Objectives Our scorecard provides staff and leaders a forum where they can access and monitor the key performance indicators, goals, targets, and benchmarks defined by the organization for the year.

Our Community

Music Therapy

Music therapy has proved to be an effective and often requested way for patients and families to be comforted as the end of life approaches. It is currently available in all the communities we serve.

Camp treehouse

Because hospice care is a concern for families as well as patients, we make bereavement education and support available for at least a year. One way we accomplish this is through Camp Treehouse, a program dedicated to children who have lost a loved one. This year, volunteers helped the program run smoothly for the 35 children who attended.

Sharing the heart The holiday season is a poignant time for

hospice patients and their families, and we help make it special through our Sharing the Heart of Christmas at Home program. Our caring physicians and staff adopted more than 270 patients and family members, purchasing gifts to make the holidays a time of joy and gratitude.

We honor veterans

We Honor Veterans is a program created by the National Hospice and Palliative Care Organization to serve our veterans. We proudly participate by collecting supplies to distribute to patients and their families.

card party The Hospice Card Party in Franklin County, now in its 14th

year, drew approximately 170 people in April and raised about $10,000 in funds for hospice services in Franklin County.

Bridge to Caring Hands To help caregivers in our community, we began offering the Bridge to Caring Hands program in the New River Valley. In a series of classes taught by our hospice team, caregivers learn about physical care and safety for both caregiver and patient, how to give medications properly, and receive training on medical equipment (oxygen tank, wheelchair, hospital bed, etc.), among other helpful tips.

Family Treasures Family Treasures is a program that allows our patients to tell their life story in their own words, and in their own way, as a gift for their loved ones. Our staff can guide patients in finding the best way to tell a complete life story, or just a memorable chapter, through a variety of options, such as video memoirs, oral history, audio memoirs, legacy books, photo collages, legacy letters, recipe collections, and more.

Journey of Hope

Losing a loved one can be a lonely and painful experience. To help ease the pain we offer Journey of Hope, a facilitated education and support group led by an experienced bereavement staff member that meets weekly for six to 12 weeks.

MIFI Devices Both Carilion Clinic Hospice and Home Care clinicians depend on access to McKesson’s electronic medical record to review important patient information such as discharge summaries or clinical information related to a recent hospitalization or office visit. To enable our clinicians to utilize this information while on home visits, we acquired MiFi mobile devices, a wireless router that acts as a mobile Wi-Fi hotspot. MiFi mobile network devices allow clinicians to connect to the McKesson server, Epic (another highly secure electronic medical record), or the Internet for searches. Safety surrounding patient care is tightened and it allows field clinicians to view a more complete clinical picture. As we partner with inpatient settings to prevent avoidable hospital admissions, access to current and more complete information is invaluable.
