1. Introduction to Cholesterol Understanding the Risk of High Cholesterol High cholesterol can cause atherosclerosis, a dangerous accumulation of cholesterol and other deposits on the walls of your arteries. The build up of plaque in the arteries can cause complications, such as chest pain, heart attacks, and strokes. Some risk factors that can cause high cholesterol include:

Symptoms of high cholesterol may not be obvious. High cholesterol complications are usually not immediate. The damage grows each year.

• Smoking

• Diabetes

• Obseity

• A Family History of High Cholesterol

• Lack of exercise • High blood pressure

In adults, Total Cholesterol levels of 240 mg/dL (6.2 mmol/L) or higher are considered high risk. Levels from 200 to 239 mg/dL (5.2 – 6.2 mmol/L) are considered borderline-high risk. An estimated 101 million adults in the US have Total Cholesterol values of 200 mg/dL (5.2 mmol/L) or higher, and of these about 35.7 million American adults have levels of 240 mg/dL (6.2 mmol/L) or above. 1. Introduction to Cholesterol • POCT Market Size The Point Of Care Diagnostics market was worth $13.7 billion in 2010 and rose by 3% since 2009.

• Global Market for Cholesterol

Market Opportunity

Based on the market research by Global Industry Analysts, the global market for cholesterol and other cardiovascular testing is forecasted to surpass 2.3 billion units by the year 2015.

• Key Driver The key factors driving the market growth include an aging population, increasing incidence of cardiovascular diseases, and a growing awareness among people about the significance of reducing their unhealthy cholesterol levels.

• Demand for Cardiovascular Testing Excluding the glucose market, the POC market rose by 7% in 2010, driven by a demand for cardiovascular testing products, including cholesterol monitoring systems. CONFIDENTIAL For Distributor Reference Only

1. Introduction to Cholesterol Overview of Cholesterol Tests There are multiple types of cholesterol circulating the blood. Cholesterol levels are measured and diagnosed with a simple blood test, often called a Lipid Panel. The Lipid Panel test provides results for four different types of lipids: Total Cholesterol (CHOL) High-Density Lipoprotein (HDL) Low-Density Lipoprotein (LDL) Triglycerides (TRIG)

1. Introduction to Cholesterol Total Cholesterol CHOL is the Total Cholesterol measurement of HDL cholesterol, LDL cholesterol, and other lipid components. The American Heart Association states that the desirable level of CHOL is less than 5.2 mmol/L (200 mg/dL), which puts the patient at a lower risk for coronary heart disease.

Total Cholesterol Level

Category

< 200 mg/dL (5.2 mmol/L)

Desirable level that puts you at a lower risk for coronary heart disease. A cholesterol level of 5.2 mmol/L (200 mg/dL) or higher raises the patients risk.

200 – 240 mg/dL (5.2 – 6.2 mmol/L)

Borderline high

> 240 mg/dL (6.2 mmol/L)

High blood cholesterol. A person with this level has more than twice the risk of coronary heart disease as someone whose cholesterol is below 5.2 mmol/L (200 mg/dL).

A CHOL level of 5.2 mmol/L (200 mg/dL) or higher raises the patient’s risk. CONFIDENTIAL For Distributor Reference Only

10

1. Introduction to Cholesterol HDL Cholesterol HDL Cholesterol is sometimes referred to as the “good” cholesterol because it helps carry the LDL cholesterol away from tissues and to the liver for disposal. Low HDL level puts you at risk for heart disease. For the average man, the normal HDL level range is from 40 – 50 mg/dL (1.0 – 1.3 mmol/L). For the average woman, the normal range is 50 – 60 mg/dL (1.3 – 1.6 mmol/L). Activities, such as smoking, being overweight, and being sedentary can lower a person’s HDL level. To raise your HDL level, avoid tobacco, maintain a healthy weight, and get at least 3060 minutes of physical activity each day. CONFIDENTIAL For Distributor Reference Only

11

HDL Cholesterol Level

Category

> 60 mg/dL (1.5 mmol/L)

High HDL cholesterol. An HDL of > 60 mg/dL (1.5 mmol/L) is considered protective against heart disease.

Men: 60 – 40 mg/dL (1.5 – 1.0 mmol/L); Women: 60-50 mg/dL (1.5-1.3 mmol/L) Men: < 40 mg/dL (1.0 mmol/L ); Women:< 50 mg/dL (1.3 mmol/L)

Borderline high

Low HDL cholesterol. A major risk factor for heart disease.

1. Introduction to Cholesterol LDL Cholesterol LDL Cholesterol is also known as the “bad” cholesterol. Too much LDL in the blood will cause a buildup of plaque in your arteries, which can reduce blood flow. An even greater danger than slow blood flow from a blockage is a sudden rupture of the surface of the plaque. A blood clot can form on the ruptured area, causing a heart attack. The lower your LDL level, the lower your risk of heart attack and stroke. For people at high risk of heart disease or with known heart disease, LDL of less than 5.6 mmol/L (100 mg/dL) is advised. 1. Introduction to Cholesterol Triglycerides Triglycerides are a type of fat in the blood that are used for energy. Triglyceride levels are strongly influenced by diet. It is recommended that triglyceride levels be tested after fasting for 8-12 hours. When a person eats more calories than what they can burn, they may have high triglycerides. High levels are also seen in overweight people, in those who eat too many sweets or drink too much alcohol, and in people with diabetes who have elevated blood sugar levels. Many people have high triglyceride levels due to being overweight, little or no physical activity, smoking, consuming excess alcohol and/or eating a diet very high in carbohydrates . Coronary Heart Disease (CHD) Analysis

 Two Evaluation Methods Are Recommended for CHD Analysis: PRO

(Procam method), popular in Europe, for men between the ages of 35-65. FRA (Framingham heart study), popular in the US, for both men and women between the ages of 30-79. 

The Mission® Cholesterol Monitoring System offers both the PRO and FRA evaluation methods.

Choose either PRO or FRA before beginning the CHD risk assessment.

15

Innovation

Coronary Heart Disease (CHD) Analysis

The factors that increase a person’s risk for CHD included in the PROCAM calculation are age, LDL cholesterol, HDL cholesterol, Triglycerides, Systolic Blood Pressure, Diabetes Mellitus, and if Myocardial Infarction runs in the person’s family history.

The factors that increase a person’s risk for CHD included in the Framingham calculation are age, Total Cholesterol, HDL cholesterol, Systolic Blood Pressure, whether the person has had treatment for hypertension, and if they smoke cigarettes.
