President’s Message
Chris Skinner, MSN, RN, CAPA Summertime can be hectic in most perianesthesia units. The maximum number of employees is granted vacation, which can leave units scrambling for staff to safely care for the patients. Add trauma and accidents into the mix, and it could spell disaster for PACU, Prep, and Phase II recovery areas. Whether it is a “staycation” or vacation, we look forward to some time away from the unit! While most of you are not thinking about it, school is right around the corner for our children, and for us! Fall is the time to renew and invigorate our practice. I encourage you to go back to school to further your education, or continue your learning by attending one of the district meetings in which you are able to obtain a continuing education unit.

This will help you cultivate your professional development, which supports your practice during performance reviews and your professional portfolios. Fall is the time for our component seminar. This year it will be in beautiful Frederick, Maryland, hosted by the Western Maryland District of CBSPAN. It is a full day conference, offering continuing education units, breakfast and lunch, rafGles for very nice prizes, and another chance to network with colleagues from all over Maryland and Delaware. We will hold a board meeting the Friday evening before the seminar. If you are interested in attending the board meeting, please contact me, or one of the board members. District of Columbia District Update Margaret Young, BSN, MDiv, CPAN, CAPA District President The DC District of CBSPAN held a half-day seminar at Washington Hospital Center on Saturday, June 27, 2015. Our speakers were Dina A. Krenzischek PhD, RN, CPAN, FAAN and Chris Price MSN, RN, CPAN, CAPA, both a part of our CBSPAN family and both past Presidents of ASPAN. Dr. Krenzischek became President of the American Society of Perianesthesia Nurses in April 2004. Her topic focus for the seminar was on JBI, Joanna Briggs Institute database and tools. She was very skilled in her ability to simplify the process on nursing research using JBI as the primary search engine. Chris Price became President of the American Society of Perianesthesia Nurses in April 2010. Her focus topic for the seminar was on the 2015-2017 Perianesthesia Nursing Standards and Interpretive Recommendations. Our discussions were lively with poignant questions on patient ratios, staffing, pain management and pediatric patients. I want to personally thank Dina and Chris for sharing their time and talents with the attendees at the seminar. The DC District Board will be meeting in August to plan for this calendar year. We hope to provide our members opportunities to grow in their nursing profession and expand their career goals on the state and national level with the ASPAN organization. The annual CBSPAN Fall Conference is just around corner. The Western Maryland District of CBSPAN will host the conference. The date is October 10, 2015 at Hood College in Frederick, Maryland at the Hodson Auditorium in Rosenstock Hall. The Fall Conference theme is: “Reignite Your Passion”. I encourage our members and non-members to attend this seminar. Please visit our website at www.cbspan.org for more information on registration. Healthcare is constantly evolving. Unprecedented changes in the healthcare system, for instance, financial constraints, healthcare reforms, mandates from regulatory agencies to improve care quality and paOent safety, and work force shortages, are affecOng how we deliver care and allocate available resources in all pracOce selngs.1 Nurses need to be cognizant of these changes, and to advocate for the wellbeing of their paOents and the future of the nursing profession. The nursing work force comprises the largest professional group within health care and has been recognized by the public as the most trusted profession.2 Despite nursing’s strength inherent in its size, diversity, and unique relaOonship with the public, the full potenOal for nursing advocacy and influence has yet to be realized.3 Advocacy does not end at the bedside. It conOnues beyond the hospital doors and all the way to Capitol Hill. Nursing organizaOons need to explore strategies to promote paOent and nursing profession advocacy, hence the birth of the Nurse in Washington Internship (NIWI). NIWI is an annual program sponsored by the Nursing OrganizaOons Alliance (NOA). NOA, of which ASPAN is a member, is a coaliOon of nursing organizaOons united to create a strong voice for nurses. The objecOves of the NIWI are for the parOcipants to: •
Describe how nurses can be involved and influence policy at the local and naOonal level.
•
Discuss how to work effecOvely with legislaOve staff to advance policy agendas.
•
Describe key steps to effect change in the legislaOve process
•
IdenOfy techniques to advance legislaOve issues at the grassroots level.
•
IdenOfy legislaOve, poliOcal, and economic forces driving health care policy and delivery changes today.
•
Learn to schedule Capitol Hill meeOngs.
The program provides nurses with an opportunity to learn advocacy strategies to further professional nursing and healthcare concerns through the legislaOve and regulatory processes. NIWI parOcipants are given the opportunity to learn from health policy experts and government officials, network with nurses from many geographical locaOons and disciplines, and gain hands-‐on experience through self-‐scheduling meeOngs and visiOng members of Congress and the Senate.4 LegislaOve “asks” are provided for aKendees to pracOce with and present in their meeOngs with their members of congress. The NIWI is held for 3 days in February, March, or April of each year and offers contact hours. The program typically starts on Sunday at noon and concludes on Tuesday with a closing recepOon at 5pm. Nurses and nursing students need to lead the efforts and share the message, whether it is in the workplace selng, classroom selng, or the state level. You can start simply by: •

EducaOng yourself on nursing legislaOon and sending out an e-‐mail to nursing friends and colleagues whom you know are interested on the informaOon
•
Joining the nurses lobby day in Annapolis
•
MeeOng or sending a leKer to your legislators and talking to them about your health care concerns
•
Leading or joining your hospital’s LegislaOve council

The Eastern Shore District has had a change of board members for the upcoming year. Susan Volk is now our past president. Susan has been a valuable asset to our component and will conOnue to mentor our members of our board. Judy Pekasciewicz will conOnue with her role as recording secretary and the newest member of the board in the posiOon of treasury is Michele Meeks. Our first board meeOng will be held on Saturday, September 12, 2015. We are presently seeking a candidate for our vice president posiOon. If anyone is interested, please contact me. The Eastern Shore Chapter of the Chesapeake Bay SPAN donated $500.00 to the HaiO Nursing FoundaOon. This foundaOon provides support for nursing educaOon in HaiO. Diz Boomer, our outgoing Treasurer, has volunteered in HaiO and was able to share her experiences. November 21, 2015 CBSPAN along with CORN sponsors a bus trip to New York City. The cost of the day trip is $40.00 and the buses leave from the RCA Surgicenter at 8:00am and returns 8:30pm. This is a fundraiser for us and anyone interested in aKending please email one of our board members. There will be a CAPA/CPAN review class for anyone interested in seeking their cerOficaOon at ChrisOana Care in Newark, DE. Review classes will begin on September 8, 2015 with 2 sessions. The first secOon will begin at 2:30pm with the second secOon starOng at 4:00pm. Chris Skinner (

As with any organizational group, the goal of the Chesapeake Bay Society of Perianesthesia Nurses (CBSPAN) is to assure that all education provided to our members is timely and up-‐to-‐ date. To this end, the Baltimore District of CBSPAN is conducting a literature review to look at the evidence surrounding the review statement: “The effect of multimodal anesthesia and/ or analgesia on the post operative/ post procedural Obstructive Sleep Apnea (OSA)patient.” Beginning in April, 2015, the Baltimore District board members and hospital liaisons' research group (Dustin Te, Mary Joy Lazo, Kristin Rouse, Liza Anicoche, Bea Hazzard, and Trish Ryan) began reviewing 126 articles referencing the use of multimodal or alternative anesthesia or analgesia. From the 126, the group chose 33 articles most speciGic to our review statement. Of those 33 articles, 18 were chosen to level and grade the evidence. The CBSPAN Baltimore District Research group is currently leveling the evidence using the Johns Hopkins Evidence Based Practice (EBP) research and non research literature leveling tools. The evidence is to be leveled and summarized by the Baltimore District’s half day educational event on September 26th 2015 (at Franklin Square Hospital). After the class, the group will meet to review the summary and begin to write a poster presentation submission to be sent to ASPAN for consideration for the Spring 2016 National Conference in Philadelphia, PA. Eventually, the goal is to use the poster presentation to write an article for submission to the Journal of Perianesthesia Nurses (JOPAN) for publication.
The Evidence Based Practice (EBP) process has been proven to be an accurate way to improve patient care. EBP does this by reviewing and testing the evidence to prove its accuracy. Once translation of the evidence is complete, nurses are left with reasonable ways to change their practice to improve patient outcomes. Unfortunately many nurses do not understand the steps of the EBP process. To this end, the Baltimore District, CBSPAN, has worked in two ways to help nurses understand the steps of EBP. First, the Baltimore District was able to work with the Institute of Johns Hopkins Nursing (IJHN) to provide a concise, easy to understand, 5 module EBP learning system at a reduced price ($25). This 5 module set also provides contact hours. Second, the Baltimore District is making available online journal club segments relating to the steps of EBP. In June, the journal club reviewed the article “How to create and use a PICO question in research” This journal club was presented real time, online, through a “Go to Meeting” link offered free to the members. The next Journal club will be September 21st (Monday) from 7:30p-‐8:30P. A link to join the journal club from home (with step by step instructions for using Go to Meeting) will be posted on the Baltimore District (and Component) website. The article we will be reviewing on September 21st is “Understanding the levels of evidence for scientiGic communication” All are welcome to sign in online and attend.

Background Information: A significant number of pre-‐operaOve paOent and system issues were idenOfied by the pre-‐operaOve staff the day of surgery – paOents coming in early for a later day surgery and vice versa; paOents not showing up or cancelled; paOents with cardiac device not idenOfied, device not interrogated, or Arrhythmia Device Service recommendaOon note cannot be located in EMR; and special needs not idenOfied such as language interpreters or isolaOon precauOons, to name a few. These issues did not only affect paOent saOsfacOon and hospital experience, but also posed a huge risk in the safety and well-‐being of paOents. Addressing pre-‐operaOve paOent and system issues through effecOve pre-‐operaOve phone call process is crucial to a successful surgery and overall paOent outcomes. Objective: To develop a pre-‐operaOve phone call process and idenOfy strategies that effecOvely enable the nurse to contact at least 80% of pre-‐operaOve paOents for surgery/procedure day/s before; to allow the staff to address the pre-‐ operaOve paOent and system issues idenOfied during the pre-‐operaOve phone call. Implementation Process: In the previous pre-‐operaOve phone call process, calls were divided among nurses who were also assigned to admit pre-‐operaOve paOents. Calls were done once or twice per day according to staff availability and pre-‐ operaOve flow. This process reached only 35-‐38% of our pre-‐operaOve paOents per month. This prompted the unit leadership team to meet with the pre-‐operaOve staff and recommended consistently assigning a nurse devoted solely to calling pre-‐operaOve paOents on a daily basis. A new process was developed, effecOve strategies were idenOfied, staff were educated, results were evaluated, and shared with the staff. Successful Practice: Consistently assigning a nurse who is devoted solely to calling pre-‐operaOve paOents a day/s before surgery has shown a dramaOc increase in the number of paOents reached pre-‐operaOvely for the last three consecuOve months. Positive Outcome: The new process has allowed the staff to address pre-‐operaOve paOent and system issues in a Omely manner, thus promoOng paOent safety and saOsfacOon, staff saOsfacOon, mulOdisciplinary collaboraOon, and an efficient OperaOng Room schedule ulOmately improving paOent outcomes and insOtuOon viability.
