1. Main Purpose of Job To work as an End of Life Care Facilitator across Airedale, Wharfedale, Craven, Bingley, Eldwick, Cullingworth and Wilsden to facilitate the identification of people in the last year of life. The post holder will work with the Specialist palliative Care teams, will provide expert support, advice and education on end of life care. The post holder will facilitate and lead the change management process required to support the continued implementation across all settings of the End of life care tools Gold Standards framework (GSF) Advance Care Plans(ACP), use of the EPaCCS and promoting the 5 Priorities for Care of the Dying Person , to enable seamless care for patients, families and their carers. The post holder will audit and evaluate the use of these tools and present findings both locally and potentially nationally. The post holder will increase staff knowledge/communication and confidence in caring for people towards the end of life .

3. Scope of Job Clinical / Technical Responsibility: . To identify end of life care learning/training needs for all levels of staff working in North Bradford & Airedale Locality. Participate and lead the planning and provision of education for dissemination of end of life tools to Hospital, GP practice & Community staff Work closely with key partners to develop and deliver education and training programmes for all staff groups to support the palliative care pathway and ensure their sustainability. Develop identified champions in each area to take forward palliative and end of life care initiatives so they are embedded and sustained in practice.

Clinical Governance / Quality: To support health care teams in the Hospital and the community . To roll out the end of life care tools to improve end of life care standards and quality of care for patients and their carers , regardless of culture or language. To work alongside all clinical staff to deliver care within a clinical governance framework ie following policies, procedures and agreed reporting mechanisms. To ensure key service evaluation data is collected, analysed and reported Managerial Responsibilities: To provide support and leadership to the Band 6 NHS EoL facilitator post holder in the hospital . Training and Development: To participate and lead on the planning and provision of education for dissemination of end of life tools to GP practices, hospital and Community staff. To Identify end of life care learning/training needs for all levels of staff working in AWC, Bingley, Eldwick, Cullingworth and Wilsden Locality To take responsibility for ensuring own training is fully up to date.

5. Qualifications Registered Nurse First Level (adult). ENB 931 (Care of the dying patient and their family) or equivalent. ENB 285 (enhanced Palliative Care) or equivalent. Degree in relevant study or working towards. Teaching qualification. Relevant 1st level degree Evidence of relevant post graduate or post

Relevant experience or knowledge of caring for patients with palliative care needs. Experience and understanding of how the gold standards register works within the hospital and community setting. To work within a multidisciplinary team, supporting staff to manage patients and carers with end of life care Take a lead role with Clinical Nurse specialists in supporting the generalists in rolling out The End of Life Care Tools to patients with palliative care needs their families and carers. Acts as a clinical resource of expertise to other areas Organising and undertaking palliative and end of life care awareness-raising workshops with all community teams To build relationships with GPs in order to engage them in improving palliative end of life care for residents Ability to draw on knowledge on a range of theories to inform practice. Develops strategies for the management and delivery of education across the locality. To build relationships/network with other key individuals and teams across all areas, including mental health, social and voluntary sectors to facilitate the improvement of end of life care ensuring seamless care , and patients choice is paramount . To attend GSF meetings and palliative care meetings across Bradford and Airedale To link closely with other palliative and end of life care staff Lead in research, audit and quality initiatives, e.g., End of life care tools, GSF, PPC,5 Priorities etc Participates in the development of evidence based practice and ensures that clinical practice is based on the latest available clinical evidence to provide optimum patient care. To collate, analyse and submit statistics relating to palliative end of life care To promote open honest communication regarding end of life care 4

As an experienced practitioner evaluates the quality of patient care and ensures that the results of evaluation are used to sustain or improve quality of patient care. Provides informal/formal teaching sessions to all staff groups within ANHSFT and Sue Ryder Manorlands catchment area To work with all members of the palliative care service to ensure best practice is taken forward and any training gaps are identified Ensures patients, families and carers views are taken into account in the decision making process utilizing patient public involvement To work with other end of life Care facilitators to ensure that training and education follows a Network wide approach. Evaluates the quality of own and others work and raises quality issues and related risks with the relevant people. Supports the introduction and maintenance of quality systems and processes in own work area. Ensures documentation is accurate and up to date and monitors and promotes high standards of nursing documentation within the areas of working. To gain an in depth understanding of current levels of documented care of palliative care patients their families and carers from existing baseline audits already undertaken locally. To maintain accurate and contemporaneous records, maintaining high levels of patient and staff confidentiality. Participate in the co-ordination of audit data relating to the End of life care tools Interpret audit data in conjunction with BDCT Palliative Care Services To disseminate audit and research findings To prepare reports on the development of the End of Life care for all stakeholders i.e.Sue Ryder, ANHSFT, Managed Clinical Network To positively publicise and promote the work and achievements of the end of life care tools liaising effectively with colleagues within and outside the organisation To promote an organisational culture where staff are empowered to challenge inappropriate behaviour and unsafe practice using appropriate organisational processes via line manager or by using our internal whistle blowing policy



To continuously improve on the reporting and management incidents, reflecting on own processes and giving feedback to others appropriate to the role



To work within the wider team to ensure that care is delivered within a clinical governance framework

Evaluates effectiveness of learning activities and agrees how learning and development may be improved in the future To maintain one’s own personal and professional development enabling the post holder to keep up to date with current issues relating to the End of life care tools and more generally, e.g. mandatory updates, which are agreed within post holder’s personal development plan. To attend mandatory training as requested To participate fully in the Sue Ryder performance management process To be aware of own training needs and take appropriate action
