Working Pattern:

The post holder will be expected to work flexibly over seven days each week as required. Whilst overnight working is not generally required the post holder will be expected to, at times, work in the early mornings and late evenings. The role will require some Bank Holiday or Weekend working.

2. BACKGROUND Framework is acting as Lead Agency for the partnership which is delivering the Big Lottery’s Fulfilling Lives programme in Nottingham. This service will be known as Opportunity Nottingham. The programme is aimed at a group of people who have been significantly socially excluded due to their multiple and complex support needs. The principal point of contact between the service and its beneficiaries will be through Personal Development Coordinators. The work of these posts will be informed by principles adopted by the partnership as part of service development: 

The service should reach everyone who needs it without exception



The service will operate on the basis of informed beneficiary consent at all times



The service will not take ‘No’ for an answer –staff will offer access to the programme and if an individual declines it, will return, encourage and support them until they feel ready for the programme – however long this takes



The service will not give up on anyone – the programme will work with all beneficiaries through difficulties, obstacles and backward steps. It will stay with them to work at their pace and enable them to achieve their goals.

3. MAIN PURPOSE OF THE JOB 

Working closely with colleagues in the Opportunity Nottingham Delivery Team to

coordinate and drive key aspects of Programme Delivery 

Directly supporting a beneficiary caseload



Leading Personal Development Teams for each caseload beneficiary



Liaison with referring and partner provider agencies



Negotiating with service providers and commissioning services on behalf of beneficiaries



To provide opportunities for beneficiaries to benefit from existing mainstream facilities and services.



To build the confidence of beneficiaries to enable them to engage with services &activities.



To empower beneficiaries to speak up for themselves and make their views known, to speak on their behalf when needed.



to enable beneficiaries to make progress on their journey towards social and financial inclusion, throughdeveloping supportive relationships in one to one sessions, and providing the information, advice and guidance needed to enable them to access appropriate servicesand networks.



Promoting the service, its ethos and benefits to potential beneficiaries, other professionals and agencies, and the wider public.

4. KEY TASKS 4.1 Engagement 

To receive referrals from agreed sources and, in consultation with colleagues, make assessments of eligibility;



To engage with beneficiaries in their homes or at other suitable locations ;



To work positively with beneficiaries to help identify their needs



To motivate and work creatively with beneficiaries in identifying appropriate service options;



To maximise the impact of contact with beneficiaries by working productively to agreed short term goals;



Where appropriate accompany beneficiaries to appointments

4.2 Personal Development Planning 

To lead the formation and workings of a Personal Development Team (PDT) around

each beneficiary 

To assist beneficiaries to create their Personal Development Plan (PDP), ensuring that it remains person-centred and reflects how the individual wants to lead their life and what support they need to do this.



To be prepared to seek creative and alternative solutions in meeting beneficiary needs.



To develop and maintain action plans informed by the PDP



To assist beneficiaries in making their plans happen by researching and providing information about services and opportunities, and liaising with service providers on people’s behalf .



To refer to and where necessary accompany beneficiaries to other agencies



To establish joint working protocols with outside agencies engaging with beneficiaries

4.3

Procurement



Where required, and properly authorised , assist or act on behalf of beneficiaries in purchasing specific services which form part of the expectations of their PDP.



Work with colleagues in order to maintain a schedule of approved providers of such “spot purchased” services.



Review and record monitoring and other information submitted by such providers in order to regularly evaluate services.

4.4

Risk Management



Carry out the practical tasks required of the job being continuously aware of any risks presented by the working environment and consistently acting in a way to minimise these risks



Carry out a risk assessment with every new beneficiary identifying with them the risks presented by the beneficiary and the environmental risk to the beneficiary



Continue to monitor beneficiary risk and to re-evaluate their risk management strategy



Ensure that information sharing protocols are developed and that risk assessments are shared with others appropriately



Report concerns appropriately, such as adult and child safeguarding issues and substantial risk issues.

4.5 Team Working



To work cooperatively with other members of the team, constructively contributing to team meetings, covering office duty and assisting with the smooth running of the service;



To ensure consistency in approach;



To ensure that the service is delivered to a high standard;



To cover for colleagues when they are absent;



Through development days and team meetings suggest ways of improving the service;



Be willing to attend wider Framework functions such as working groups;



Mentor and support students and/or volunteers in the service.

4.6 Personal Qualities 

5. BREADTH OF THE ROLE The individual performing this role will be expected to plan and organise their workload in consultation with colleagues and Line Management. The complex nature of the beneficiaries will require the coordination of tasks and support to maximise outcomes. The post holder will be aware of how their work influences and impacts on the work of others and therefore they will need to have a comprehensive knowledge of external services available. The development and maintenance of effective partnerships is crucial to the role. The post holder will refer into other specialist agencies and will understand that they are

representing the organisation and partnership when carrying out this work. The post holder will have a flexible attitude to the work and be solution focused in their approach. The post holder will take active responsibility for their own career development by coming forward for additional tasks and engaging in relevant training to enhance their own competence.

6. ROLE OF THE SERVICE The defined outcomes of the Opportunity Nottingham programme are: 1. To empower people with multiple and complex needs and support and enable them to take control of their lives. 2. To change front line services and make them more effective by listening to what beneficiaries want and need – making them better coordinated and integrated, more person-centred, responsive and realistic in relation to targets and timescales. 3. To deliver change at strategic and commissioning level by working with strategic leaders and using the learning, outcomes and impacts of the programme to change the system’s ‘DNA’.

7. KEY PERFORMANCE INDICATORS The post holder will be expected to demonstrate competence in the role and this will be measured by:   

Their knowledge; Productivity and performance; Conduct and commitment.

The post holder will have an enhanced understanding of risk management and how it applies to the role. The post holder will be expected to have read, applied and complied with Framework’s Policies and Procedures. The post holder will possess good time management.
