Tutor Self-declaration

The very nature of this workshop means the tutor will be imparting and dealing with sensitive information with a group of 13–16 year olds and therefore in a ‘position of trust’. In order to be considered to deliver this workshop, we ask that all potential tutors complete this self-declaration section regarding any criminal convictions.

Any information that is disclosed will be confidential and used to inform the recruitment to the tutor orientation for this workshop. Once the recruitment decision has been made, this information will be destroyed.
