Synthetic Turf Rebate Program Receive a Rebate When You Purchase and Install Synthetic Turf Save Water and Help the Environment Did you know that 50-60% of Orange County’s water supply is used outdoors? You can decrease your water usage dramatically by installing synthetic turf. Synthetic turf is both kid friendly and pet friendly. It’s low maintenance — you’ll never have to mow or edge it — and it looks great year-round. Southern California’s arid climate is not suited for the high-water-usage grass varieties found at most homes. So do yourself and the environment a favor and replace your traditional turf with synthetic turf. What is Synthetic Turf? Synthetic turf consists of man-made materials that are grass-like in appearance and function. Most synthetic turf systems include a drainage layer, a composite layer, and synthetic grass blades that can be infilled with a granular filler to resemble natural turf. Rebate Program Information Orange County residents are eligible to receive a rebate for synthetic turf. To qualify, the synthetic turf must be purchased and installed on or after July 10, 2007. For a list of rebate-eligible synthetic turf makes, a rebate application, program guidelines, and relative contact information, please visit www.mwdoc.com/wue. Please confirm with your homeowner association and your local City Planning Department prior to the purchase and installation of any synthetic turf that it meets all of their specific requirements.

MUNICIPAL WATER DISTRICT OF ORANGE COUNTY SYNTHETIC TURF REBATE PROGRAM INFORMATION SYNTHETIC TURF Please Note: Applications are processed in the order received on a first-come, first-served basis, while funding lasts. To be eligible for the rebate, the synthetic turf must have been purchased on or after July 10, 2007, must be installed, and must be identified on the list of approved products. Please remember to enclose a copy of your most recent water bill and the original sales receipt for the purchase of synthetic turf with this application. To qualify for the rebate, the synthetic turf installed must replace a minimum of 500 square feet of irrigated landscaping. INSTRUCTIONS Synthetic Turf must be installed prior to submitting application. • Read and agree to the program eligibility rules. • Complete all sections on the other side of this application. • Confirm that the synthetic turf purchased and installed is identified on the list of approved products. Visit www.mwdoc.com/wue or call (800) 954-4344 to confirm eligibility. • Tenants must submit owner authorization letter. • If you choose to give your rebate to your synthetic turf installer, the Payee Information Section must include the installer’s Tax I.D. Number. • Attach the original sales receipt for the purchase of the eligible synthetic turf. The original sales receipt must show all costs incurred in the purchase of the synthetic turf. If the purchase price of the synthetic turf includes installation, the charges for the purchase and installation must be listed separately on the original receipt. (Photocopies, journal receipts, rebate receipts or fax copies are NOT acceptable). • Retain a copy of this application and the sales receipt for your records. This rebate program is not responsible for materials lost or delayed in the mail, including misdirected mail. • If you would like your original sales receipt returned, please provide a stamped, self addressed envelope, the original receipt, and a photocopy of the original sales receipt when you submit your application. • Attach a copy of your most recent water bill for the address where the synthetic turf is installed to this completed application. Rebates CANNOT be processed without a copy of your most recent water bill. • Agree to the Terms and Conditions of the rebate program by signing and dating the application. • Send the completed application to the address above. By signing the application, I acknowledge and agree to the following Terms and Conditions: • Only single family residences located within Orange County, California are eligible for synthetic turf rebates through this program. Rebates for synthetic turf installed in multi-family sites such as condominiums, mobile home parks, townhomes, apartments, or installed in apartment/ condominium common areas, may be available through www.mwdsaveabuck.com. • There is a limit of one application per single family residence. • Synthetic turf purchased and installed must replace a minimum of 500 square feet of irrigated landscaping, and must be purchased on or after July 10, 2007. Confirm that the synthetic turf purchased and installed is identified

on the list of approved products. Visit www.mwdoc.com/wue or call (800) 954-4344 for a list of approved products. • Installation costs are NOT eligible for rebate reimbursement. • Rebate checks will only be issued to established rate payers of the water agency listed in item #1 on the reverse side of this form. Exceptions may be made for spouses, tenants with written permission from their landlords, and the synthetic turf installers. • If I receive a rebate, I acknowledge that I will be required to participate in a post-installation verification audit if so requested by MWDOC. • I authorize MWDOC and my Retail Water Agency to release my water consumption data. This data will not be released to the public, but if needed, used for water savings analyses. • Rebates will be processed in the order they are received on a first-come, first-served basis, while funding lasts. I understand that this offer is limited by available funding. I further understand that processing of my rebate may take up to 16 weeks. • Synthetic turf must be installed at the address identified in Item #2 on the reverse side prior to submittal of this application. • Any synthetic turf for which a rebate is issued is subject to an on-site installation verification inspection. If synthetic turf installation cannot be verified by the rebate program, I will reimburse the rebate program for all rebate funds issued, and any other costs associated with processing the rebate, and will promptly pay all invoices from the rebate program, MWDOC, and/or my own Retail Water Agency. • I understand that I must provide my Social Security Number in order to receive a rebate. Synthetic turf installers must provide their Tax I.D. Number if they are to receive the rebate. The Internal Revenue Service requires rebate program participants receiving $600 or more in rebates in a calendar year to be issued an IRS Form 1099. • The rebate program is not responsible for materials lost or delayed in the mail, including misdirected mail. By participating in the rebate program, you waive and release the Municipal Water District of Orange County, the Metropolitan Water District of Southern California, your City Water Utility or Retail Water Purveyor, and their contractors or agents, from any and all claims and causes of action arising out of the installation or use of the rotating nozzles purchased in connection with the Rotating Nozzles Rebate Program. Any claim you may have based upon any defect or failure of performance of a rotating nozzle purchased by you should be pursued with the manufacturer/ distributor. PRIVACY NOTICE The California Information Practices Act of 1977 and the Federal Privacy Act require the Municipal Water District of Orange County (“MWDOC”) to provide the following to individuals who are asked to provide information in order to participate in MWDOC’s rebate programs (“Programs”). The principal purpose for requesting information is to administer the Programs. The Programs have been established by MWDOC pursuant to powers granted it under the Municipal Water District Law of 1911, California Water Code, Sections 71000 et seq. Submission of the requested information is voluntary. However, failure to submit the requested information will preclude participation in the Programs. As authorized by law, information furnished pursuant to participation in the Programs may be provided to the United States Internal Revenue Service and/or the appropriate official of any state imposing an income tax or tax measured by income (see Section 6041 of the Internal Revenue Code, and Section 1.6041-1 of the Income Tax Regulations). Information may also be provided to other governmental entities when required by law.
