REPORTING STAFF: ROLE PURPOSE: PURPOSE:

Nil The role of the Care Team Leader is to provide and coordinate resident care, and to supervise staff to ensure an effective, efficient functioning team. The Team Leader will support the Care Manager and will perform in accordance with legislation and practice within a professional and ethical framework. The Care Team Leader will be responsible for assessing and evaluating individual resident care, to act as a resource for staff, perform competency assessments and have input into training of staff. The Care Team Leader will liaise with the Care Manager to ensure best practice resident care, resident well-being and respect for resident rights.

QUALIFICATIONS AND SKILLS:

1. Current Certificate as a Nurse with AHPRA (Enrolled or Registered Nurse); 2. Current First Aid Certificate; 3. Understanding of Dementia; 4. Experience in working with the elderly; 5. Strong Leadership skills;

ESSENTIAL: ESSENTIAL:

1. Ability to provide quality client care in accordance with current Client Care Plans. 2. Criminal History Check to work in Aged Care. 3. Ability to lead, direct and supervise other carers as required. 4. Ability to follow instructions and accept direction as required. 5. Ability to report any changes in a client’s condition to the Care Manager on duty for review.

6. Good communications skills – written and verbal. 7. Ability to work independently, without supervision, as required. 8. Ability to communicate effectively with staff and clients. 9. GENERAL RESPONSIBILITIES: RESPONSIBILITIES:

The range of duties to be carried out includes: 1. To carry out your duties in a timely manner and in accordance with the established policies and procedures for the related Key Business Area of: a. Governance & Leadership; b. Resident Services; c. Facilities & Equipment; d. Safety & Security; e. Business Services; f. Marketing and Sales; g. Resident Committees. 2. To deliver care to residents with a focus on Best Practice Care principles, building Community Vitality and continuous improvement; 3. Provide required reports in a timely and accurate manner; 4. To manage and administer medication management as required; 5. To lead a small team of carers to provide discreet care services to residents; 6. To assist with the supervision of other staff during shifts where you assume the responsibility of Manager (weekends, relief etc.) 7. To relief the Care Manager from time to time;

8. Assist the Care Manager to implement and improve systems relating to the care services at the Village. 9. To receive requests from residents and attend to them following established procedures. 10. To provide high level administrative support to Village Management as required; 11. To provide hands on support to all other personnel as required; 12. To provide feedback for continuous improvement on service delivery to the Village and its residents; 13. To provide emotional support to residents as required; 14. To carry out light cleaning functions as required; 15. To assist with the delivery of meals and clearing as required; 16. To establish and maintain effective working relationships with all businesses, practitioners and individuals related to the carrying out of your function; 17. To ensure that all work is performed in accordance with relevant laws and regulations; 18. To ensure that all relevant systems and procedures are protected and that confidentiality of all client records are maintained; 19. To actively promote the village within the local community;

20. To attend to front desk/reception; 21. To answer incoming calls from time to time; 22. To gain an appreciation of all residents personal interests to provide suggested activities that cater to all residents; 23. To maintain open communications with senior operational staff both informally and through formal meeting schedules; 24. To assist in maintaining and promoting a positive organisational culture that ensures the Key Business Areas are focused on and achieved; 25. To ensure that all tasks on the relevant work cycles are carried out in a timely basis and that they are accurate; 26. To ensure all work is properly documented; 27. To provide leadership and support to all other personnel; 28. To generally support all activities of the village as directed. 29. Other duties reasonably directed
