Your 2015–2016 Free Application for Federal Student Aid was selected for review in a process called verification. In this process, the Financial Aid Office will be comparing information from your FAFSA (Free Application for Federal Student Aid) with copies of your (and your spouse’s or parent(s)’) tax information as provided from the IRS. The law states that we have the right to ask you for this information before awarding federal aid (CFR 668.51). If there are differences between your FAFSA data and other requested documents, you may need to make corrections to your FAFSA and you also authorize the College to make corrections on your behalf. Complete, sign, and return this worksheet, along with any other documentation requested by the Financial Aid Office. On your FAFSA application, if you did not enter your tax information through the IRS Data Retrieval process or you used the IRS Data Retrieval process and made subsequent corrections to your return, The College requires you to submit to us a “Tax Return Transcript.”. To obtain a Tax Return Transcript from the IRS, please call 1.800.908.9946, or order one by mail using IRS Form 4506T (Request for Transcript of Tax Return) and have the document sent to you. Please submit the transcript via the OCC Verification Documents Portal link found on the last page of this document, with the student’s full name and ID number clearly marked on each page. Do NOT submit your 1040, 1040A, or 1040EZ forms unless you are requested to do so. If we require any further documentation to complete the verification process, you will be notified.

C. Tax Forms and Income Information Instructions: Complete this section if the parent(s) and student filed, will file, or will not file a 2014 income tax return. The best way to verify income is by using the IRS Data Retrieval Tool that is part of FAFSA on the Web. If the student’s parent(s) have not already used the tool, the parent(s) and the student should go to FAFSA.gov, log in to the student’s FAFSA record, select “Make FAFSA Corrections,” and navigate to the Financial Information section of the form. From there, follow the instructions to determine if the parent(s) and student are eligible to use the IRS Data Retrieval Tool to transfer 2014 IRS income tax information into the student’s FAFSA.
