2016 – 17 Back to School Information Pursue Truth, Take Risks, Grow Stronger As the first day of school approaches, our faculty and staff are anticipating to once again have our hallways and playgrounds filled with the energy and spirit of our students. Reflecting on the theme for this year, we want our students to develop resilience in growing stronger mentally and physically while committing to pursue the truth. For students to accomplish this, they will have to take risks. By giving our students the encouragement to take risks, while being a support to help them learn from their failures or successes, we will be building skills for challenges and obstacles throughout their life. Brookfield Academy continues to demonstrate our strength in academics, leadership, decision-making, character, and culture. We must continue to understand what skills our students will need to be successful for the next stages in their life. By doing so, we become stronger as a school and community while keeping our Five Stars, Academy Triangle, and classical approach cemented firmly underneath our feet.

Parent-Student Handbook Please read the enclosed handbook with your student(s). Before you go shopping, review the dress code printed in the handbook. Following this dress code is important in establishing an effective learning environment. We expect parents to purchase clothing that conforms to the dress code.   Sign the form attached to the Parent-Student Handbook and return it to the school office by the first day of school. Upper School parents, please return before fall sports practices begin.

Volunteer Information & Activities Sheet

Directory The School Directory is being updated and will be distributed to all Academy families approximately two weeks after school begins.

 Please visit the BA website to view and sign up for upcoming volunteer opportunities. Information is enclosed on the golf outing, Founders Fest, Mothers Night Out, Homecoming, Five Star Run, and Grandparents Day.

Weekly Newsletter

BA NEWS: A newsletter is published every Wednesday throughout the school year. It is available on the BA website brookfieldacademy.org every Tuesday by 2 pm. The newsletter contains information on upcoming events, student successes, and volunteer activity notices. It is the primary form of communication with school families. A Sports Report comes out weekly as well. The newsletters are sent home via the youngest student in each family. It is carried home in the homework folders of Primary School and Lower School students and is mailed to Middle School and Upper School student families (if they don't have a student in the Primary School or Lower School). 2

Information for All BA Families

Transportation BUS TRANSPORTATION Last year’s riders will automatically be put on the bus rider list unless you call to tell us otherwise. Schedules for specific stops and pick-up times will be mailed shortly before the first day of school. REMEMBER: For bus drivers to become familiar with the routes, it is crucial that students ride the bus beginning with the first day of school.  Please call the school office if your student is a new bus rider this year.

Forms Checklist 4   Emergency/Insurance: The enclosed BA Emergency Contact Form (one for each family) and Insurance Waiver Form (one for each family) must be completed and returned by Friday, July 29. In May, the Athletic Department mailed the Emergency Form to all families with Upper School students. Please submit only one 2016 – 17 Student Emergency Form per family.

PARKING AND TRAFFIC FLOW GUIDELINES

 OTE: Please indicate on the Emergency Form if you N want your child to receive Acetaminophen (Tylenol) and/or Ibuprofen (Advil/Motrin) from school stock.



  Health/Medical: The Administering Medication to Students Policy is enclosed, along with the Confidential Student Health Information Form (one for each child) and Medication Administration Authorization Form (one for each child). All medical forms can be found on the BA website. These forms must be returned by Friday, July 29.

Patriots Hall: • Please do not park in the driveway at the entrance to Patriots Hall. • If your student is not available, please pull through and park in the lot.

Middle School Parents:

  Immunization Record: If you are new to BA or have an incoming Level A, Level B, or Level 6 student, you were sent in June the Student Immunization Requirements and the Student Immunization Record Form (one for each child). The Student Immunization Record Form (one for each child) must be returned by Friday, July 29. All immunization requirements are to be met by the first day of school.



You may view your child's immunization record from your computer on the Wisconsin Immunization Registry (WIR). The internet address is www.dhfsWIR.org. To obtain the dates of your child's immunizations, type in your child's name and social security number.

Freedom Hall: • For Level 6 pickup, use the upper (north) drive of Freedom Hall. Please make a RIGHT turn when exiting the north lot between 3:15 pm and 3:45 pm. • For Levels 7 and 8 pickup, use the lower (south) lot of Freedom Hall. Please make a RIGHT turn when exiting the south lot between 3:15 pm and 3:45 pm. • DO NOT stop or park in the THRU lane. • Parents with students in both Constitution and Freedom Halls should pick up Middle School students first (use north drive) and then proceed to Constitution Hall. • Drivers waiting in line for extended periods of time will be asked to park until the student is ready. • Arriving after 3:30 pm to pick up students will reduce waiting time.

Lower School & Primary School Parents:

  Tuition: Tuition is due by September 1 or alternate payment arrangements must be in place. Whether paying in full or by an alternate payment plan, arrangements must be made with the Business Office by Monday, August 1.





  Photo Release Form: An Authorization/Photo Release Form (one per family) is enclosed. Complete authorization form, sign, and return by the first day of school.

Liberty Hall: • Use drive off W. Beverly Hills Drive if your child’s last name begins with A – K. • Use drive off Parkview Drive if your child’s last name begins with L – Z. Constitution Hall: • Pick up Middle School children first, if applicable. • Remain in the center lane until you have passed Freedom Hall and then proceed into pick-up lanes 1 – 4. • DO NOT stop or park in the THRU lane. • Use angle parking if you must leave your vehicle. • Pedestrians should use the crosswalk at all times.

Everyone: • Please communicate well with your children so that they are ready when you arrive. • Please do not park in spaces that are RESERVED by name. • We strongly encourage all right turns after school. Plan ahead. • Use caution and drive slowly. Keep our children safe.

Upper School Information

CHECKLIST for Class of 2017 (Seniors)  Review the application instructions for the colleges listed on your Final College List. You do not need to complete the applications during the summer.  Remember to register for the fall SAT and ACT.  Have a senior picture taken for the yearbook by the photographer of your choice this summer. Photos are due by Monday, August 29, to receive a $10 yearbook discount.  Gather pictures for the Senior Dinner DVD (three pictures ages 2 – 8) and the yearbook (one senior and one baby picture). This information was previously mailed. Submit all photos by Wednesday, September 7.

Freshman & New Student Orientation Thursday, August 25 • 9 am – 2 pm, Patriots Hall Freshmen and new students will spend a day on campus touring the Upper School, meeting teachers, and making new friends. All freshmen and new students are expected to attend this orientation.

I f the enclosed schedule does not reflect your student’s course requests, please contact Dr. Dan Davis, Head of Upper School. In general, it is not possible to make changes in teacher assignment or sections at this time, as class sizes must be balanced.  Check enclosed schedule for errors.

Upper School Fall Sports Practices  Boys football practices begin Monday, August 1. Girls field hockey practices begin Monday, August 8. Girls swimming and girls tennis practices begin Tuesday, August 9. Coed cross country, boys soccer, and girls volleyball practices begin Monday, August 15. Coed fencing practices begin Tuesday, September 13.   Return WIAA physical card, or alternate year card, Athletic Agreement, AND Emergency Form by Monday, July 18, in order to participate in fall sports. Forms are available on the BA website.

Saturday, November 5 – Save the Date!

Student Parking All Upper School students driving a vehicle to school must register their vehicle(s) before the first day of school.  Return vehicle registration form.

Book Purchases A complete, up-to-date list of required BA text books is available at brookfieldacademy.org/academics/textbooks. Books may be purchased through our official online bookstore, mbsDirect.net, through a retail or online vendor of your choice (be sure to check the ISBN number), or from a BA student. Used books for sale by students are listed on a bulletin board in the Patriots Hall lunchroom.  Purchase books in time for the first day of school.

On the first day of school, students should report to the Freedom Hall North Gym for assembly by 8:15 am. Parents are welcome to attend this assembly and any assembly throughout the school year.

Off-Campus Trips

Levels 6, 7, & 8 fall trip information and permission forms are included with this packet. Please call Doug Ricci or the MS Office (262-252-2241) with any questions. Forms and fees (check made payable to Brookfield Academy) should be returned before the first day of school.  Return respective trip form and fee before the start of school.

After-School Programs

Level A: Morning Session: 8:15 – 11:45 am, 11:45 am – 12:15 pm – optional lunchtime in Peter Rabbit Room. Afternoon Session: 11:45 am - 3:15 pm, 11:15 – 11:45 am – optional lunchtime in Peter Rabbit Room.

Parents interested in registering students for MAP (or other after-school programs) should review the MAP/After-School Program booklet. Please contact Doug Ricci, doug.ricci@ brookfieldacademy.org, with any questions.

Level B: Children will be dismissed at 12:15 pm for the first week (August 29 – September 1) of school. Our experience has shown that students do best if they are eased into our full-day kindergarten program. Beginning Tuesday, September 6, dismissal time for all Level B students will be 3:15 pm.

School Supplies Supplies List can be viewed at brookfieldacademy.org/parents/ forms/middle school.  Gather supplies.

Level 6 & New Student Orientation Tuesday, August 23, 3:00 – 4:45 pm

Levels 1 – 5: 8:15 am – 3:15 pm. Peter Rabbit Program (for PS-A): Half days, starting as early as 7:30 am, ending as late as 3:15 pm (see enclosed booklet).

All Level 6 (since you are new to the Middle School) and all new students in Levels 7 and 8 will spend the afternoon touring the Middle School, meeting teachers, setting up lockers, and getting technology login information. All Level 6 and new students, as well as their parents, are expected to attend this orientation.

After-School Enrichment Courses: Generally 3:15 – 4:15 pm, in three 9-week sessions (see enclosed booklet). Narnia Program: 3:15 – 6 pm (see enclosed booklet)  Return Peter Rabbit registration.  Return Narnia and After-School Enrichment Course registrations.

Lockers  All returning students in Levels 7 and 8 can set up their lockers on Tuesday, August 23, anytime between 12:30 – 5:00 pm. Parents should accompany their students. Teachers will be available in the locker areas during this time to assist with combinations and questions.

School Supplies Besides a big smile and lots of enthusiasm, Primary School and Lower School students need only bring:  An empty book bag (all necessary school supplies will be provided).  Gym shoes.

For those with a bag lunch, microwave ovens are available. Milk is provided for all. Please order online by Sunday, August 14. Lower School & Primary School Information Class Assignments Approximately one week before school begins, a letter will be sent to each student with his or her classroom teacher assignment. We go to great lengths to make certain each class contains a range of ability, a balance of boys and girls, and the best environment for the success of each child. On the first day of school, students should go to their classrooms. Staff members will help them find their way.

Daily Schedules The school day begins with the morning assembly at 8:15 am at both Constitution Hall and Liberty Hall. Parents are most welcome to attend.
