Case narrative including clinical course, therapeutic measures, outcome and additional relevant information: Bericht der Meldenden Ärztin: bei einem 14-jährigen Patienten, der ca. 1 Jahr mit 30-40mg Fluoxetin und zusätzlich mit 10mg Chlorprothixen behandelt worden ist, wurde vom Kardiologen eine QTc-Zeit-Verlängerung unter Fluoxetin auch nach Absetzen des Chlorprothixens diagnostiziert.

Anamnese vom 23.02.2017: ANOMALY / BIRTH Bei dem Jungen ist eine Autismus-Spektrum-Störung bekannt, es erfolgt seit ca. einem Jahr eine DEFECT antidepressive Medikation mit Fluoxetin. Die Mutter berichtet, dass er zusätzlich abends wegen starker Anspannung 10mg Truxal erhalte. In einer Klinik sei ein EKG abgeleitet worden, welches OTHER MEDICALLY angeblich unauffällig gewesen sei. Es fiel dann bei Ableitung eines EKGs im Januar diesen Jahres IMPORTANT unter 40mg Fluoxetin eine verlängerte QT-Zeit auf. Daraufhin wurde empfohlen, das Fluoxetin auf CONDITION 30mg zu reduzieren und das Truxal abzusetzen. Da auch wiederholt hypertone RR-Werte II. SUSPECT DRUG(S) INFORMATION 14. aufgefallen waren, erfolgt jetzt die Vorstellung zur erneuten EKG-Untersuchung und Ableitung einer 24h-RR-Messung. Zurzeit erhält der Junge Fluoxetin 1x 20 mg sowie eine Behandlung seines Asthma mit 2x 1 Hub Viani. In der Familie ist kein Long-QT-Syndrom bekannt, keine unklaren Todesfälle im Kindes- oder jungen Erwachsenenalter. Beurteilung und Empfehlung: Auch nach Reduzierung des Fluoxetin auf die o.g. Dosierung besteht bei dem Jungen eindeutig eine verlängerte frequenzkorrigierte QT-Zeit fort. Somit muss das Fluoxetin abgesetzt werden und eine erneute EKG-Kontrolle nach Absetzen durchgeführt werden, unter der Frage, ob möglicherweise ein Long-QT-Syndrom unabhängig von einer möglichen Fluoxetin-Nebenwirkung besteht. (Halbwertszeit des Medikamentes bzw. des Abbauproduktes bis 2 Wochen) Bei der 24h-RR-Messung konnte ein signifikanter arterieller Hypertonus nachgewiesen werden. Diesbezüglich ist weitere Diagnostik und auch Therapie erforderlich. Therapeutisch empfehle ich eine Behandlung mit Propranolol 3x 20 mg (3x 1/2 Tablette 40 mg). Da dieses Medikament auf der einen Seite möglicherweise die Aufregung des Jungen positiv beeinflusst und auf der anderen Seite bei möglichem Long-QT-Syndrom ebenfalls therapeutisch eingesetzt werden würde. Mit der Mutter wurde eine Kontrolle zum EKG wie oben empfohlen am 09.03.2017 vereinbart. Sollte dann die QTc weiter verlängert sein, dann erneute Kontrolle des EKG Anfang April mit einer Kontrolle der 24- Std-Blutdruckmessung. Therapieempfehlung: 3x 20 mg Propranolol
Beurteilung und Empfehlung: Nach Absetzen des Fluoxetin jetzt erstmals normale Frequenz korrigierte QT-Zeit. Somit scheint kein genetisches Long-QT-Syndrom vorzuliegen, sondern die QT-Verlängerungen sind durch das Fluoxetin bedingt. Somit muss dieses Medikament weiterhin abgesetzt bleiben. Die Einzelblutdruckmessung unter 3 x 20 mg Propranolol ist im Normbereich.
