SERVICE PROFILE PHYSIOTHERAPY SERVICES

Introduction The aim of this service profile is to describe as clearly as possible a highly complex service. It should be a reference document for anyone wishing to know more about different aspects of the Community Physiotherapy Service – or a starting point for accessing more detailed information. Any information is only as accurate as it is current and all our services are evolving and growing almost on a daily basis. Students will be placed with in these services and are given the phone number of the therapist coordinating students in that area. They may also contact student coordinator John Firth at our main office on 2716363 in case of difficulty or general enquiries. We would ask that students contact us well in advance of their placement as travelling community staff are often harder to contact than hospital staff. We would ask that you contact the coordinator for your placement 3 weeks prior to your placement. We look forward to seeing you.

Background There have been physiotherapists working within the community i.e. outside traditional hospital environments since 1982 in Sheffield. Originally only 6 physiotherapists were involved in what was then a ground breaking approach – establishing a domiciliary service for vulnerable adults. The original community physiotherapy service focused on supporting older people in their own home, developing therapy programmes which adapted and used the physiotherapists techniques, making treatment relevant and appropriate to real life situations. Since those early beginnings the service has grown beyond all recognition. With the push to deliver services closer to patients and, through GP fundholding initiatives, GP practices chose to commission their own musculoskeletal physiotherapy services delivered in the primary care clinics of GP practices. This reduced the need for patients to travel to outpatient clinics at acute hospital sites and allowed much closer relationships between GPs and physiotherapists to develop. Another great boost to community physiotherapy services arose in the mid 1990’s with the push to find alternatives to lengthy inpatient hospital rehabilitation programmes – thus reducing pressures on hospital beds. New multi-disciplinary community rehabilitation teams were created which established new and SERVICE PROFILE PHYSIOTHERAPY SERVICES

alternative models of intensive and effective therapy outside of the hospital setting. This group of services now known collectively as “intermediate care” teams rely on the leadership and professional input of physiotherapists working closely with other therapy and nursing colleagues. There are many other areas where physiotherapists have extended the scope of their practice or have developed specialist roles, for example, in orthopaedic screening, women’s health and physical disabilities. In these areas new services have grown up and made vital contributions to the patients and the health care community. In the 21st century it is no longer unusual to receive physiotherapy or rehabilitation programmes within a community setting – it is more and more becoming the norm. This reflects the growing confidence held by and in physiotherapists and reinforced by a body of evidence that we can continue to deliver quality and effectiveness to patients in this manner. There remain many areas where physiotherapists continue to develop new roles and services, e.g. coronary heart disease and occupational health. We develop new partnerships with voluntary, statutory and private organisations and continue to review and modify methods by which we deliver these services. Organisational Structure Physiotherapy Services is organised as a city wide professional agency delivering locally based services through service agreements with our many partners. Though hosted for contractual purposes by Sheffield South West Primary Care Trust we deliver to the other 3 PCTs, to Community Health Sheffield and to other external contractors. Physiotherapy Services professionally manages its qualified physiotherapists, therapy assistants and clerical staff with reference to a clear clinical governance framework and with guidance on professional standards from our professional body, The Chartered Society of Physiotherapy. Physiotherapy Services was transferred to Sheffield South West PCT in April 2002. Before April 2002 Occupational Therapy and Physiotherapy had been jointly managed as Therapy Services, hosted by Community Health Sheffield.

Scope

South of the city if require transport 3.16 WTE qualified staff and 2 Therapy Assistants (shared with OT) to cover up to 35 patients per day (not all require physiotherapy)

Patients

Where treatment in a day hospital is identified as the most appropriate setting for rehabilitation. Mainly older adults with longer term multiple physical problems requiring ongoing rehabilitation. Also some younger patients with neurological problems such as stroke.

Main objective

To maximise functional independence, assist hospital discharge and prevent hospital admission

Description of service A day unit for interdisciplinary assessment and treatment of older adults requiring rehabilitation. This may be in groups or on an individual basis. Educational and falls programmes are held on a regular basis through a strong team approach. Support is given for both the stroke and falls pathways. Discharge is determined by patient’s progress. Main objective

To help patients achieve the best possible qualify of life and functional level and to reduce mortality and morbidity through secondary prevention.

Description of service Each patient has an initial assessment and then joins the programme. Based on the findings of the assessment, patients will have an individualised programme of physical activities designed. Each programme runs twice a week for 6 weeks. The patients will do their exercise circuit followed by an educational session. Scope

Main objective

To reduce the number of falls which result in serious injury.

Description of service Can be individual patient treatment in any community setting or can be a group setting in a Falls Programme of 6 or 12 weeks incorporating individualised evidence based exercises, alongside multidisciplinary education and information sessions. A home environment assessment is usually performed to reduce the risk of hazards. Following a screen or full assessment patients may be referred on to other services including the Falls Clinic. Description of service Patient phoned within 24 hours of referral, triaged by qualified physiotherapist. If it is decided that they require urgent treatment they are then seen within 48 hours. They will be treated with appropriate physiotherapy techniques with emphasis on self management and educ ation. Service operates 8.30 – 4.30 Monday to Friday.

Profoundly disabled adults and occasionally children who usually are severely restricted in movement (particularly upper limb) often have conditions such as motor neurone disease, multiple sclerosis or spinal injury.

Main objective

To provide assistive technology to profoundly disabled people to enable safer independent living.

Description of service Environmental Controls System provided technology which enables disabled people to control many appliances in the home to support increased independence. The units use infra red and radio frequency technology accessed by a single switch to control lighting, heating, door entry, telephone, TV and other electrical appliances. The co-ordinator oversees the provision of the most suitable equipment to disabled people meeting the medical criteria within a defined budget.

Main objective

To provide an effective rehabilitation service to enable patients to be discharged earlier from hospital, prevent inappropriate admission to hospital or premature admission to long term residential care.

Description of service On admission comprehensive assessments are carried out by the team and patient centred goals are agreed. The patients undertake treatment programmes with the appropriate team members in the fully equipped treatment areas – gym and rehab kitchen. Most patients have a home visit prior to discharge ensuring that all services and equipment are in place for discharge. Description of service 1. The Rapid Response team is one part of the Intermediate Care Services offering a time limited support service for older people suffering from specific medical conditions which put them at risk of hospitalisation. The aim is to prevent hospitalisation and support the patient at home with short term rehabilitation, or if necessary to admit to specific short term beds for therapy. 2. The discharge pathways are community based teams treating patients who have had an acute medical episode usually a stroke, fracture or joint replacement but not exclusively. The patient has a rehab program set up, based around patient focused goals. The intervention is initially very intensive up to five days a week reducing as appropriate up to discharge when their future care is discussed and organised.

Via GP only. Referral on to secondary care orthopaedic and soft tissue consultants only if necessary. Approximately 20% of cases.

Main objective

To provide a primary care based service which is efficient and effective in the diagnosis and management of patients with orthopaedic conditions.

Description of service This city wide service was established in 2000 in response to the long waiting times for patients seeking an orthopaedic appointment in secondary care, and the published evidence that extended scope physiotherapists were successful in managing approximately 75% of these patients within primary care. The OSS continues to develop its links with secondary care to ensure the most effective management of patients with complex orthopaedic conditions. Extended scope physiotherapists have been trained to undertake injection therapy where necessary for the treatment of specific conditions.

To adopt an integrated and client-centred approach in order to improve quality of life.

Description of service Established in 1989, PDTT provides a community based specialist assessment, therapy and advisory service. Working with people aged between 16 and 65 years of age who have an enduring physical disability, communication and/or swallowing difficulty. Physiotherapy, speech and language therapy and occupational therapy are provided in an MDT setting. The team can work with people on a long term basis. The service can be provided in a location most appropriate to their lifestyle. This is usually the client’s home but may also be in a day centre, workplace or college.

Main objective

To assist people with respiratory disease to live their life to their full potential using individual exercise training with health and lifestyle education, usually within the context of a group.

Description of service The patient attends for a comprehensive assessment and then attends the group for between 6-8 weeks. The rehabilitation consists of a functional exercise programme and educational sessions to promote a healthy lifestyle. At the end of the programme the patient is re-assessed and the patient is followed up again 1 year later. Description of service The service is rapidly developing to provide specialist physiotherapy as part of Sheffield’s integrated team, aimed at women with or at risk of compromised continence. Clients range form young pregnant or newly delivered women with low back or pelvic pain to elderly women with incontinence.
