Living Maths Sleepover Notice and Application Form What is the Living Maths Sleepover all about? Living Maths is an academic programme with a difference. We teach school children about LIVING, using MATHS. We do this through a practical and hands-on based curriculum. Living Maths emphasises that children should experience the application of mathematics in order to fully grasp and appreciate its usefulness. A Sleepover is merely a natural extension of the Living Maths classes with the added dynamism of interaction, socialising, developing ideas, exploring and discovering together with peers. The Sleepover will be taking place at Olive Schreiner Hall, First Floor, 1820 Settler’s National Monument. The Sleepover has a limit of 40 places and it will work on a first-come-first-served-basis. Exciting activities have been put together to ensure that the children maximise their experience. There will be ample adult supervision. Tell me more about the programme The Sleepover has been designed in such a way that there will be activities to cater for everyone. Our experience has taught us that the variety on the programme will have even the quietest or the shyest child feeling comfortable when taking part. We have prepared many activities that involve the Living Maths approach to understanding Maths. Without divulging too much about the programme, here is what your child can look forward to while having loads of fun and making new friends:   

There is a strong likelihood that your child will have limited sleep at the Sleepover, partly due to the excitement, and partly due to nocturnal children, and subsequently they will probably be exhausted. If you feel that your child needs their sleep and they cannot exercise self-control, then we do not recommend that you send your child. When is this Sleepover anyway and how much are we talking? The Sleepover begins at 20h00 on Saturday, 21 March 2015. Please ensure that your child is dropped off at the Olive Schreiner Hall, First Floor, 1820 Settler’s National Monument by 20h00. The total cost of the Sleepover will be R50-00 and it includes accommodation, dinner, breakfast, security, plenty adult supervision and the brilliant programme. The Sleepover will end at 08h00 on Sunday, 22 March 2015. Kindly collect your child before 08h30 as the venue opens shortly afterwards to the general public.

So where do we sign up? If you are interested in registering your child, please understand that we can only take a maximum of 40 children and therefore we highly recommend that you contact us at your earliest convenience. We are operating on a first-comefirst-served basis. The deadline for the registration (if there are any places left) will be Monday, 9 March 2015. Due to catering and accommodation arrangements, we need to have final figures before we embark. So what must my child bring? The following items are recommended to ensure a comfortable Sleepover:   

Sleeping bag, mattress, pillow Change of clothing, toiletries Torch, snacks and sweets. (Optional)

Parents: Please ensure that all items that belong to your child are clearly marked. We kindly ask parents not to send your child with expensive clothing or expensive items since we will not be held responsible if these valuables go missing. We will have a cellphone (083 308 3883) at the venue in case you would like to contact us in an emergency. We have decided that as much as we enjoy parental support and participation in everything that we do, this Sleepover is the only exception. Children will be taking a break from their parents and vice versa. As parents, you deserve a rest too. If you have any further questions, please do not hesitate to contact me. Yours sincerely. This is not only unpleasant for you, but also for us, as we may not be allowed to make use of the Olive Schreiner Hall as a venue for future Sleepovers.

Please note the following: The safety of your child is our primary concern. If your child, in anyway, knowingly compromises the safety of any of the other participants, under the discretion of the Director, your child will be expelled from the Sleepover. You will be required to fetch your child from the Monument building and you will forfeit the Sleepover fees. No child will be permitted to leave the Sleepover without the supervision of an Adult. No child will be permitted to bring harmful weapons (pocket-knives included), cigarettes, alcohol or drugs. Any child who is caught with these items in their possession will be expelled from the Sleepover – with no further discussion. Finally, every child is required to behave appropriately and any destruction of property, invasion of people’s privacy, intimidation or violence, etc. is not permitted. Please explain this to your children before they take part. We thank you for your understanding.

Living Maths Sleepover - Consent, Indemnity and Waiver.

I, the undersigned, (print your full name)________________________________in my capacity as the legal guardian of (name of child)_________________________________(herein referred to as the applicant), request the permission for the applicant to participate in the Living Maths Sleepover , (herein referred to as ‘the Sleepover’), which is to be held at the Olive Schreiner Hall, First Floor, 1820 Settlers National Monument on Saturday, 21 March 2015 or as such other time and/or place as the organisers, as defined below, in their sole direction decide, do hereby : 1. Acknowledge that the organisers reserve the right to refuse at any stage to grant the applicant permission participate in and/or attend the Sleepover; 2. Certify that all the information contained in these forms is true and correct, that no material information whatsoever has been withheld, and acknowledge that any incorrect information may result in this application being refused and/or withdrawn; 3. Undertake to abide by the rules, regulations and procedures of the Sleepover. 4. Undertake to pay on demand all amounts payable in connection with the applicant’s attendance at the Sleepover or reasonably ancillary thereto, including transportation expenses thereto and therefrom; 4.1 Irrevocably indemnify the staff at the Sleepover, Living Maths, any of its affiliated and/or constituent bodies and Directorates and/or any representative, employee, servant or agent of any of the aforegoing bodies or persons (hereinafter referred to collectively or individually, as the case my be, as, ‘the organisers’) against any claim which may be incurred though and accident or act of God, on the part of the applicant; 4.2 Should the organisers undertake to institute claims upon the applicant, due to gross negligence or damage caused, by the applicant, such claims and/or liability will be paid on demand by me and/or the applicant. 5. Waive, all the claims that I, or the applicant may, have against the organisers or any of them in respect of any loss, damage (arising directly, indirectly or consequentially) of property of the applicant or injury caused to the person, accidentally or through an act of God, during the Sleepover and/or during the conveyance of the applicant by whatever mode of transport to, from and during the Sleepover, 6. Designate the Director of the Sleepover (hereinafter referred to as ‘the Director’) or anyone appointed by him/her to act in loco parentis on my behalf during the course of the Sleepover. In particular, but without derogating from the generality of the aforegoing, the Director or his/her appointee shall in his/her sole discretion have the authority to give permission for any emergency operation or medical treatment required for the applicant and to incur on my behalf all the expenses flowing therefrom, which I undertake to pay on demand; 7. (Please note that every step will be taken to contact parents should such an incident occur) 8. Agree that – 8.1 The Director or anyone appointed by him/her shall have the right at any time during the course of the Sleepover to terminate the applicant’s participation therein and to send the applicant home if, in his/her sole discretion, it is considered advisable for any reason whatsoever; 8.2 The Director or his/her appointee may send the applicant home at my and/or the applicant’s own cost

by any reasonable means of transport which s/he in his/her sole discretion select; 8.3 I and/or the applicant shall pay, on demand, to the organisers all the expenses so incurred, acknowledging the fact that there shall be no refund for the unexpired period of the Sleepover; 8.4 Should the applicant be sent home in terms of clause 8.1 above, the applicant shall not be permitted to participate in and/or attend any other Sleepover, unless the Director, in his/her sole discretion, decides otherwise; 9. Accept that in the event of the applicant being unable for any reason whatsoever to attend or participate in the Sleepover for the full period thereof or at all, neither the applicant nor I shall have any claim for a refund of any Sleepover fees paid, unless the organisers in their sole discretion agree to make such payment; 10. Acknowledge that each of the aforegoing provisions are severable from the other and may be enforced against me and/or the applicant separately from the other provisions.
