Continuing Education The Nationwide Children’s Hospital is accredited by the Accreditation Council for Continuing Medical Education to provide continuing medical education for physicians. The Nationwide Children’s Hospital designates this educational activity for a maximum of 12 AMA PRA Category 1 Credit(s)™. Physicians should only claim credit commensurate with the extent of their participation in the activity.

Registration Attendees may register online at NationwideChildrens.org/ Conferences (Search: Powell) or by using the attached registration form. Please follow the instructions carefully. Upon receipt of the registration payment, each participant registering by September 2, 2016 will receive a confirmation letter via email. The registration fee includes educational materials, exhibits, all meals provided, break snacks, complimentary Starbucks coffee on site and 2 drink tickets to the Wine and Cheese Reception on Friday. Deadline for registration is Friday, September 2, 2016 Note: Individuals utilizing a purchase order must include an original copy of the order with a signature, number and billing address in order to be registered. This issuance of a purchase order to secure a registration slot is an obligation for payment whether the participant attends, cancels or is absent from the conference. Payment for purchase orders is required within 30 days of receipt by Nationwide Children’s Hospital.

Confirmation A confirmation letter, receipt, parking information and directions to the conference will be sent to those whose registrations are received and processed by the September 2, 2016 deadline.

Location and Parking The conference will be held at Nationwide Hotel and Conference Center, 100 Green Meadows Drive in Lewis Center, Ohio 43085. Ample free parking is available.

Cancellation Policy A refund, minus 30 percent for administrative costs, will be given for cancellations received in writing before September 2, 2016. After this date, no refund will be available.
