Terms Of Payment & Invoice Billing • Please pay your invoice and make checks payable to Amsino International, Inc. • Invoices not paid according to terms of sale are subject to a 1.5% late payment penalty per month.
Returns • • • •
Authorization to return products must be obtained from Amsino’s Customer Service Department prior to the return of any goods. Failure to obtain authorization will result with denial of the return. To obtain authorization the following must be provided: product number, lot number, quantity, and reason for the return. Amsino’s Customer Service Department will provide a Return Authorization Number (RGA). This number must be on the packing slip of the returned products in order to receive credit. Product returns are subject to the following fees: – 20% restocking fee – Return freight fee – $100 handling fee per restocking shipment

Freight prepaid within the continental U.S.A. for orders of 100 cases or more. Orders for less than 100 cases are FOB Amsino. Amsino I.V. sets and Enteral Pump Sets are freight prepaid on orders of 50 cases shipped within the continental U.S.A. All pump orders have separate PO and are FOB Amsino. Freight subject to a fuel surcharge.
Drop Shipment • Fees will apply to drop shipment orders. Freight & MOQ policies will apply. • Drop shipment fee $50 for orders that are under $5000.
