Dear Friends, For many of North Coast Health Ministry’s patients, some days it feels like the sun will never shine. Days when the rent is overdue, there’s not enough money to pay the electric bill, and the prescription medication they need to control their blood sugar has run out. Days spent figuring out how to get to and from our clinic for a scheduled appointment when it’s snowing and the car won’t start. Days dominated by fear that recurring chest pain may mean another hospitalization that they can’t afford. For the past quarter century, North Coast Health Ministry has striven to bring a ray of sunshine to those in our community struggling to maintain hope in the face of unrelenting challenges. With the help of our volunteers, donors, partner hospitals, supporting foundations and other community partners, we’ve been a “way” to good health for families experiencing some extremely difficult trials. It hasn’t been easy – especially in this time of growing need and scarce financial resources. What has made our work possible is faith. As Saint Augustine wrote, “Faith is to believe what you do not see; the reward of this faith is to see what you believe.” We have always believed that we can make a difference in the health and well-being of the low-income uninsured, and, through our belief, together we have made that possible. On our most recent survey, 100 percent of our patients reported that their health has improved or stabilized since they’ve become a patient at North Coast Health Ministry. Many have been able to return to work as a result of our care. We hold dear the idea that every person is innately worthy of being treated with dignity and respect and that the sun is always shining even when we cannot see it. Sincerely,

SERVICE OVERVIEW We offer primary medical care, prescription assistance, specialty referrals and health education to the low-income uninsured in western Cuyahoga and eastern Lorain counties. Our goals are accessible, continuous, coordinated and comprehensive patient-centered care. The services we offer, therefore, are integrated and coordinated to this end and consist of the following: Primary Care Clinic: Offered five days per week Chronic Illness Management Program: Geared toward patients with commonly diagnosed chronic conditions and managed by a coordinated team of NCHM caregivers, with the goals of improving the quality of care provided to those with chronic illnesses and preventing and minimizing complications related to chronic illness Women’s Health Program: Includes preventive breast health and gynecological screenings and care Prescription Assistance Program: Enables our patients with chronic illnesses to receive free or low-cost medications to manage their diseases and prevent complications; also enables the provision of age-appropriate immunizations and home health supplies to patients Specialty Referral Program: Allows our patients to access specialty care donated by our volunteer referral physician network Patient Critical Needs Program: Provides immediate resources to meet patient critical needs and links patients with social service needs to resources and outside agencies that can help

Service Statistics In 2011, North Coast Health Ministry: • Served 2,740 individuals, for a 9 percent increase compared to 2010 • Registered 1,123 new patients. • Provided 26,355 services, a 5 percent increase over 2010, which included: • 6,180 primary care visits onsite • 819 primary care visits offsite • 964 mental health counseling and social work visits • 1,425 specialist referrals • An additional 16,987 services, which includes help with prescription assistance forms, specialty referrals, medication pick-up, nurse visits and lab testing • Dispensed $3.9 million in prescription drugs, a 25 percent increase over 2010; included: • $51,317 in drug repository medications • $2.6 million in medications acquired through pharmaceutical manufacturer patient assistance programs • $1.1 million in medications through the Astra-Zeneca institutional patient assistance program (IPAP) • $132,759 in medications provided by medication samples

Dean Stamatis Recently laid off as a public school music teacher, Dean Stamatis was at a loss on what to do or where to go when his 12-year old daughter began experiencing severe stomach pains and unexplained vomiting. A family member referred him to North Coast Health Ministry, where his daughter was examined and referred for testing. “When we came here,” says Dean, “we were assisted in other ways where my daughter could get medical attention from St. John Medical Center, including free blood work to find out what was going on inside of her. This also prompted the Cleveland Clinic to have a scope put down her stomach to find the abnormalities inside. Through North Coast, we were able to get these other services–which was awesome–at little or no cost to us.” Dean says that because he was unemployed, there was no way he would have been able to afford the care and testing his daughter needed. “It would have been detrimental to us,” he says, “but because of North Coast, we were able to get the assistance in these other areas to find out what was happening with her.” Dean’s daughter recovered, thanks to the care provided by NCHM and its partner hospitals, and is now back at school and thriving. Although Dean has recently found two part-time jobs teaching preschool children, his new jobs do not offer health benefits, and he does not earn enough to purchase coverage on his own. “Even though I have two jobs, I’m still in need of your services,” Dean says. “It’s still hard to get medical coverage. We live paycheck to paycheck still.” North Coast Health Ministry will continue to be the “medical home” for Dean and his family until their circumstances improve. “North Coast Health Ministry has provided a wonderful safe haven for my family,” he says. “I’m very grateful that we have this.”

NCHM’s 25th Anniversary Commemorated at Silver Celebration Gala A sell-out crowd of 300 guests feted North Coast Health Ministry’s 25th anniversary at our Silver Celebration gala at Westwood Country Club on Thursday, September 1, 2011. The event, co-chaired by Sandy Haas, Wendy Kieding, and Carol Rini, raised record proceeds of over $150,000, which represents more than 10 percent of our agency operating budget. Dr. Russell Elmer, NCHM founder and honorary chair, flew in from California to be present and kicked off the evening with opening remarks and reflections. “It’s humbling to see the community pull together to provide this level of support,” says Executive Director Lee Elmore. “It’s gratifying to know there are so many others that share our commitment to providing health care to our low-income neighbors in need.” In addition to providing valuable operating support, event proceeds allowed us to fulfill patient critical needs, such as prescriptions and home health supplies.

Our Treasured Volunteers: Bringing a Ray of Hope into the Lives of our Patients

In 2011, 140 volunteer physicians, nurses, social workers and other laypeople donated their time to provide care and assistance to our patients. Some physicians and nurse practitioners come to our clinic on a volunteer basis to provide care. Others see our patients in their offices and waive the fee. Non-clinical volunteers help by screening and enrolling new patients and assisting in clinic with prescription medication application processing, making patient reminder calls, linking patients with specialty referrals, and updating patient financial information. Other volunteers help with fundraisers and special projects on an as-needed basis. Without our valuable network of volunteers, we would not be able to provide care to as many low-income uninsured individuals and families as we do today. Through their contributions, they bring light into darkness and provide comfort in the trials of our patients’ lives.

North Coast Health Ministry spends less than four percent of its total revenue on management, general and fundraising expenses. Ninety-six percent of revenue is spent on providing patient care.

North Coast Health Ministry (NCHM) is a faith-based free clinic serving the low-income uninsured on Greater Cleveland’s West Side. With the support of a network of volunteer physicians, nurses and other caregivers, NCHM provides primary health care, specialty referrals, prescription assistance, health education and social service benefits access to the medically underserved. We strive to be our patients “medical home,” providing a patient-centered source for preventive care and chronic disease management. We believe health care is as basic a human need as food, clothing or shelter—especially for those with chronic illnesses. Over 70 percent of North Coast Health Ministry’s patients have chronic conditions, such as diabetes, hypertension and heart disease. The majority of our patients report that they would have nowhere to go for care other than the local hospital emergency room if North Coast Health Ministry were not here. We are grateful for the generosity of our supporters that enables us to provide essential health services to our medically underserved neighbors in need.
