Yolo County Homeless Services Inventory HOUSING SERVICES

Type of Service

Program Information Housing support services for former foster care or probation youth (ages 18 - 24) to transition from out-of-home placements to independent living. THP-Plus provodes services like job training, educational support, & counseling.

Serves 8 homeless families for a maximum of 18 months

Transitional Housing

Serves up to 16 individuals; 12 men and 4 women. Individuals may stay in the program for up to 18 months. Provides intensive case management, supportive services, life skills, counseling, employment assistance, and assistance finding housing. Serves 10 families; Provide case management, life skills classes, parenting classes, job training, child care, medical and transportation services

8 units of transitional housing for ADMH clients: 4 units in Woodland / 4 units in West Sacramento

Operates several multi-family affordable housing communities throughout Yolo County

Affordable Housing

Operates sustainable housing for low income families

Provides low cost housing: Section 8 Vouchers, Project-Based Vouchers, and housing for migrant workers during growing seasons

Emergency shelter

31 bed emergency shelter for women and children escaping domestic violence, sexual abuse, or human trafficking. Offers therapeutic services and case management

Transitional Housing Emergency Shelter

55 housing units-- Provides intensive case management

Cold Weather Shelter

Serves 10 individuals-- 8 men and 2 women-- Open from mid-November through March 31

Permanent Supportive housing (mental illness)

Housing and life skill support for individuals with major mental illness at 5 independent houses and two apartment complexes in Davis and Woodland.

Supportive Housing (substance abuse) Transitional Housing (substance abuse)

45-bed residential substance abuse program that uses the 12-step model. Provides individual and group counseling, offers courses on various life skills / 22-bed transitional program for clients in various stages of recovery.

Supportive Housing (substance abuse)

Transitional Residential Treatment Program (mental illness)

Residential substance abuse program that provides employment assistance, stable housing, and cognitive behavioral therapy

Adults live and work at the Farmhouse for long-term residential treatment; learn life skills to transition to independent living.
