Trileana™ ® Instructions and Guidelines It is important that you follow these instructions carefully. Administration of Trileana™ ® Drops Dosage is ¼ ml - 2x a day (12 hours apart, if possible) Place under the tongue and hold for 3 minutes After you swallow the remaining content, be sure not to eat or drink anything for 10 minutes. Not eating or drinking for 10 minutes is important for full absorption of the hCG. Trileana™ ® must be kept refrigerated Trileana™ ® is effective for 60 days after being reconstituted Administration of Trileana™ ® Topical Cream To get the cream to start pumping, strike the bottle on the palm of your hand several times. Then pump the cream until the cream starts to flow. You will only have to do this once to start it. Apply 1 pump to forearm and rub until absorbed morning and evening, preferably 12 hours apart. You must administer Trileana™ ® twice daily. It doesn't matter if it is taken on an empty stomach or not. Patients, who have routinely forgotten the second dose, will have markedly increased hunger and their weight loss will be affected. Day 1 & 2 - These days are critical for success Take Trileana™ ® twice a day for 2 days and eat anything you want, particularly foods high in fat. The foods eaten during these two days do not need to be healthy. Eat as much as you can of the high fat foods that you like during this time without overeating. This fat will not stick and will be used for energy. You will still lose the first week. Many patients are resistant to this part of the diet, but it is necessary. The reason for these two days is to create enough fat reserves that can be used in the first 3 days of the diet. It seems to take 3 days before abnormally deposited fat begins to circulate and thus become available. If you do not eat high fat foods for the first two days of the diet, you will be markedly hungrier than those who do. Day 3 – 20 and beyond if weight goal has not been reached You will follow a low calorie diet plan made up of healthy, nutritious food. See Trileana™ ® Food Guide below. So even though you are restricted to consuming only 500 calories, those calories are from lean meats, fresh vegetables and fruits that you will shop for at your local market and feed to your family. Follow the diet strictly without interruption for the first 20 days. This is the time when the body metabolism is improving. If you have more weight to lose you will continue on the diet past 20 days until you reach your goal. You cannot make any exceptions to the diet plan. Only eat the quantity suggested and the food suggested on the diet plan. Your success will depend entirely on the how well you follow the first two days eating high fat foods and the subsequent days on the diet plan. The first 20 days reprograms the hypothalamus. It takes this long for the effects on the hypothalamus to occur so it is critical to be completely dedicated to the diet for this time. The most positive results will be obtained if you stay on the diet without adding anything for the first 20 days. If you go off the diet for a day or even a meal, there will be a set-back in weight loss by about 3 days and you will experience increased hunger, but it will not undo the progress that has been made to that point. During the first 20 days the reprogramming of the body is taking place. A disruption beyond a day during this time will affect long-term weight loss. After 20 days or later if you have not yet met you weight loss goal After 20 days, the occasional lapse in diet will not affect long-term weight loss. You can continue the diet for as long as needed until your weight loss goal is achieved. At the end of the 20 days or the day you have reached your goal, you will stop taking the Trileana™ ® and continue the 500 calorie diet for three more days. At the end of the three days, you will go into a transition phase (also known as maintenance) to prepare your brain/body to eat normally. See After Trileana™ ® below. Diet Tips  Purchase a food scale and weigh all the meat and vegetables raw; 

If possible, eat only organic meat, chicken, vegetables, and fruit. In order to be organic, beef needs to be free-range hormone free and chicken should be free range, no antibiotics. Beef should be very lean cuts, no marbling and all fat removed before weighing and cooking. Ground Beef was not originally allowed, however people have been successful eating occasional meals of 93% or leaner ground beef.



No fat whatsoever is allowed on this diet. Nothing can be added to the diet without increasing hunger. Even fat-free salad dressings have sugar, which will increase appetite. Do not take Omega-3 capsules.



No non-dairy creamer or other substitute is allowed.



You can add spices and hot sauce, but read the labels and buy those without sugar. Making food spicy can be very satisfying, help to decrease cravings and increase water intake.



There is no limit of coffee or tea. 2 diet sodas are allowed daily. Those liquids should not be consumed in place of water.



There is absolutely no alcohol of any kind allowed.

Weigh-Ins You may come into our office once a week to weigh in. You can come in anytime we are open, please call ahead of time to set up an appointment. We encourage you to weigh at home every morning after emptying your bladder and without clothes before eating or drinking. A digital scale is recommended. Weight loss occurs in stair-step fashion. First you will drop then plateau, etc. We have found that even during the plateau phase, patients will still be losing inches. Usually the plateau does not last more than a week and may coincide with the onset of menstruation in women. Supplemental Medications Vitamin B12 During weekly weigh-ins, patients are encouraged to have a B12 injection for energy and overall health. Or you may take a sublingual B12 tablet or liquid drops in the morning each day. Let it slowly dissolve in your mouth. Do not take for 10 full minutes after the hCG. Potassium 99mg We ask you to take a potassium supplement as the hCG can cause some loss of potassium over time. Magnesium OptiMag - take 2 tablets daily or more if needed for constipation. Appetite Suppressants Because the Trileana™ ™ diet is very low calorie, some patients might need an appetite suppressant. We encourage you to attempt the diet alone first. If you continue to suffer from intense hunger we suggest Chromium picolinate, a natural supplement to decrease hunger and food cravings. Chromium can be used safely up to 3 times a day. Chromium is available in our office for $25.10 for 180 tablets. Phentermine, a prescription strength appetite suppressant can be used second line in some cases. It is a stimulant and is regulated by the federal government. Patients are instructed to take 1/2 a tablet in the morning 1 hour before they eat. Many times only a half a tablet is needed because this protocol will reduce and correct the appetite. You can take the second half later in the afternoon if needed. In some cases the patient can take 1 whole tablet in the morning. Stimulants can raise blood pressure and cause your heart to race. We are not able to prescribe this medication for anyone with a heart condition or elevated blood pressure.

Trileana™ ® Food Guide Breakfast: Any Non-Calorie Fluids in any amount. (See fluid options below) Protein Foods: 3 ½ oz 's twice daily (lunch and dinner) Example: About the size of a makeup compact Choose only lean meats particularly  Veal or Steak (free-range only)  Fresh White Fish (Halibut, Cod, Flounder, Monkfish, Perch, Pike, Pollock, Snapper, Sole, Whiting, Crab meat, Lobster, Shrimp, Scallops)  Chicken-no skin (Chicken Breast Only) All visible fat must be removed before cooking and the portion should be weighed raw. It must be cooked without additional fat. Helpful Hints Hunger If you feel too much hunger, you most likely did not eat enough calories or high fat foods for your two days of loading. This hunger usually will subside after a few days. Make sure you are drinking enough water. Drinking teas may help. Recommended teas are black tea, green tea, wu long tea (oolong tea) and eleotin tea. Also effective is drinking 2 tablespoons of apple cider vinegar in a glass of water. The herb chickweed has been shown to curb hunger without side effects and can be bought in capsules. You may just be experiencing cravings, which will subside. Another reason for hunger is your body telling you that you have lost all your excess fat. If you are close to your ideal weight, and you feel a lot of hunger, you should consider that this might be what is happening. At this point it might be wise to move into the maintenance phase. If you still feel that you need to lose more fat, you can do another round of the diet after a brief maintenance period. Remember, that you have to remain on the diet for at least 18 days (through day 20), no matter if you want to lose 5 pounds or 100 pounds. Constipation Constipation is common on a low calorie diet like this. You may use herbal laxative supplements or teas that have senna leaf or cascara sagrada. Also magnesium supplements or epson salts are effective. But with such a small amount of food being consumed, you shouldn’t be concerned about going everyday. Itchy Skin This is a sign of toxins that were stored in fat cells being released. This is a good thing. You are detoxing. After a certain amount of weight loss, this will go away. Dry Skin With so little fat being consumed in the diet, dry skin is a very common occurrence. Try to look for lotions that are made with alpha hydroxy acids like glycolic acid. Just make sure you look at the other ingredients to make sure there isn’t any animal or vegetable oils. It also helps to exfoliate before putting on any of these moisturizers. Low Energy Some people experience tiredness or feel weak on this diet. Not everyone feels this, and for those that do, it comes and goes. Be sure you are taking a potassium supplement. Also, don’t go too low on your sodium intake. Your body needs some salt to be healthy. Use a sea salt, which has many minerals besides sodium chloride. If experience extreme fatigue you may want to up your protein portions to 6 or 8 oz for 2 days. Leg Cramps Be sure you are taking potassium supplements if you have leg cramps. Also, make sure you don’t go too low on your sodium intake. Not Losing Enough Weight Everyday Everyone loses differently on this diet, so you shouldn’t worry if you have a low weight loss for several days in a row. It’s better to look at your weekly weight loss. If you are averaging at least three pounds a week you should not worry. Losing Too Much Weight You must stay on the diet for at least 18 days (days 3-20). If you have reached your weight loss goal before 18 days, you must raise your caloric intake to 800 calories per day until you finish the 18 days. Stopping the diet early does not allow your hypothalamus to be reset, and you will gain back your weight loss very rapidly.

Men and Using Trileana™ ® Men can safely use Trileana™ ® without side effects. In fact, it is used to make underdeveloped adolescent boys reach normal development. Bodybuilders use hCG to normalize testosterone production after using steroids. And the dosages used for weight loss are very low compared to those used for these medical treatments. At these low dosages used for weight loss, there have been no side effects. Over the last 40 years there have been thousands of men using hCG without any problems. Reshaping Even if you are not losing as much weight as fast as you would like, your body may be going through a period of reshaping. If you had an uneven distribution of fat, like big hips with a small waist, for example, you will notice that you will lose inches in your hips first. The loss in inches can be dramatic compared to the number of pounds lost. Plateaus Plateaus are normal, but if you are on a plateau for more than four days, you can do an “Apple Day”. The apple day starts at noon and continues 24 hours until noon the next day. During this time you are to eat six apples, and drink only water, and only enough water to barely satisfy your thirst. The apples act as a diuretic, causing your body to expel the excess water. Your scale will reflect a dramatic weight loss and have you motivated again. If the “Apple Day” doesn’t work, you should examine your diet and make sure you are following the protocol exactly. Also check all your beauty products and toiletries for animal or vegetable oils. A sticking point can happen when your weight gets to level that you maintained for a period of time in the past. This is your body trying to stay on a weight that it thinks is right. Hang in there, because you will pass this weight before you know it. This diet has worked 100% of the time, so keep that in mind. Diet Interruptions If you must interrupt the diet to eat normally, try to stop Trileana™ ® 72 hours before you plan on eating more calories and different foods. This will allow your body to be clear of hCG. If you still have hCG in your body and you eat more calories, you will gain a lot of weight very rapidly, but if there is no hCG in your body, you may not gain at all. You can go up to 2 weeks off the diet before you will need to restart from the beginning, including the 2 days of loading with Trileana™ ®. Trileana™ ® Interruptions You can go up to 3 days without hCG, and then you will start to feel very hungry and it will be very difficult to stay on the diet. We recommend that you begin to eat normally after 3 days without hCG, consuming at least 800 calories, adding meat, eggs, cheese and milk to your meals. If it has been less than 2 weeks without hCG, you can resume the 500 calorie diet along with the hCG, but if it has been more than 2 weeks, you will need to restart from the beginning, including the 2 days of loading with hCG. Salt Salt is allowed and is necessary for normal body function. It is also important for making the food palatable. If you don’t use salt, it may be more difficult to stay on the diet if the food is too bland. Don’t go overboard with the salt, but use a normal amount. With salt use, however, there can be a build up of fluid retention, which can appear as a stall in weight loss, or even a gain in weight. This is nothing to be concerned about as it will balance out over time. But if you want to, you can do an “Apple Day”. Apples are a natural diuretic, and effective at reducing excess fluids safely from your body. Artificial Sweeteners Artificial sweeteners may fool your body into thinking they are sugar and your body releases insulin, which causes your body to store fat. Stevia is not artificial and does not have this effect. It might take a little getting used to for some, but eventually you will appreciate it when you need something sweet. Use the liquid stevia, as the powdered varieties sometimes have additives that are not allowed Carolina Integrative Medicine

Sleep It has been found that sleep is very important for weight loss, and particularly on this diet as far as how much weight you lose per day. Make sure you get as much as you need. Water Water has also been shown to be very important to how much weight is lost per day on this diet. However, it is different for everyone. Two liters (68 ounces or .5 gallons) are recommended, and some who have consumed a lot more than that have lost less weight than if they kept it around two liters (68 ounces or .5 gallons). But everyone’s different so you may want to experiment a little. Diabetes On this diet, many diabetics are able to stop all anti-diabetic medication after a few days of treatment. The blood sugar continues to drop each day until it reaches normal values in 2-3 weeks. This is not the case with all diabetics, so make sure you do the diet under a doctor’s care and continue to monitor your blood sugar levels. Medications/Supplements You can continue to use prescription medication if necessary, but try to stay off other medication as much as possible. Do not take fish oil or omega fatty acids. Skin Sag One of the great aspects of this diet is, despite the great and rapid amount of weight loss, there is no skin sag. This is due to the hCG burning the stored fat cells instead of your structural fat cells. If you are getting towards the end where you are close to your desired weight loss, and you start to notice some skin sag, that means you have lost some structural fat cells. If this is the case, you should have felt extreme hunger by now, but there are some that don’t feel this. If you have done at least 18 days on the diet, then you should move on to the maintenance phase. If you still haven’t completed 18 days on the diet, raise your calories to 800 per day until you finish 18 days. Vegetarians Vegetarians may have a hard time with this diet. They may try substituting an equivalent amount of vegetable protein for the meats. However, the weight loss will not be as dramatic. Cooking Tips Since you have to weigh your meat before cooking, and it can be difficult to cut off the right amount of meat to equal 3.5 ounces, you can end up with a lot of little pieces of meat. It can be a hassle to keep all the pieces organized so you have the correct amount in the end. One way to solve this is to put the cut pieces equaling 3.5 ounces in ziplock bags, after weighing and then cook them as needed. You can also put a little marinade in the ziplock bag. A barbeque grill or a George Forman grill works well and can allow you to cook several portions at the same time, by putting the portions in different sections of the grill. Once cooked you can put them in new ziplock bags, and reheat when needed. Another option is to put the pieces equaling 3.5 ounces on individual skewers. You can then put them in a large container to marinade, cook them all at once on a barbeque grill or in the oven, store enough for a week in one container, and reheat as needed. This keeps the correct portion intact from weighing to cooking to storing. Food Substitutions While the foods listed have proven to work for most people, you may find that some stall or lessen your weight loss. For example, some do not lose when they eat beef. This could be because the amount of fat in beef is hard to gauge. If you change your food choice and you don’t lose for a few days try going back to the foods that have worked for you and see if weight loss improves. Carolina Integrative Medicine

After Trileana™ ® The Maintenance Phase At the end of 20 days or later if you have more weight to lose, you will stop taking the Trileana™ ® and continue the 500 calorie diet for three more days. At the end of the three days, you will go into Maintenance – the transition phase. It takes about 3 weeks for the weight reached at the end of the Trileana™ ® diet to stabilize at your new weight, i.e. weight does not show violent fluctuations after occasional excess. In order to do this, you must follow this plan exactly. During this 3 week transition phase you are allowed to eat as much as you want and what ever you want of all meats, eggs, dairy, and most fruits and vegetables but you must be aware that the sugar based and starchy carbohydrates: sugar, rice, bread, potatoes, pastries, etc, are not allowed. You will NOT be able to eat any carbohydrates like grains, breads, cereals, foods made with flour, root vegetables like potatoes, corn, pumpkin, winter squash, yams, sweet potato, beans, lentils, rice, pasta, tortillas, cookies, crackers, popcorn, cakes, bananas, mango, or pineapple. Fats can be indulged somewhat more liberally and even small quantities of alcohol, such as a glass of wine with meals, will do no harm. But if fats and starchy carbohydrates are combined, weight gain can get out of hand. See Maintenance Food Guidelines below. It is very important to weigh yourself every morning after emptying your bladder and without clothes before eating. If the scale goes up by two pounds, on that same day do not eat anything until dinner except drink plenty of liquids. At dinner eat a huge steak (you can substitute chicken or fish) plus 1 apple or 1 raw tomato and nothing else. See Skipping a Meal below. Some people don’t believe that they can eat normally without gaining weight. They are so afraid to gain back the weight they have lost, that they continue on a relatively low calorie diet. A low calorie diet without Trileana™ ® is NOT a good idea. Without the Trileana™ ®, you will feel extremely hungry, and your body will go into famine mode. You will actually gain weight as your body shuts down your metabolism and conserves fat cells. It’s very important to get adequate amounts of protein in this phase. When you are taking the Trileana™ ®, it is assisting in the break down of fat, which then feeds protein back into your body, allowing you to get adequate protein levels. Without Trileana™ ® you need to eat an adequate amount of protein to prevent protein deficiency. You may be familiar with the many low-carb diets, like Atkins and The South Beach Diet. You can use many of the recipes from these diets, but try to use whole food ingredients and stay away from artificial ingredients and fake foods. Try to eat as close to the healthy diet you will ultimately want to follow for life. Another diet that has recipes that are mostly allowed in transition is called Somersize by Susan Somers. If you eat too little, you will gain weight, and if you eat too much, you will gain weight. You may have to adjust your calories until you find the right amount for you. find the right amount to maintain your weight. In this transition phase you can eat a lot more food, but don’t go wild. Increase your calories gradually over a few days. Skipping a Meal (the steak and apple day) As long as your weight stays within 2 pounds of the weight reached on the day of the last Trileana™ ® dose, you should take no notice of any increase, but the moment the scale goes beyond two pounds, even if this is only a few ounces, you must on that same day entirely skip breakfast and lunch but take plenty to drink. In the evening eat a huge steak and 1 apple OR 1 raw tomato – nothing else. It is of utmost importance that this is done on the day the scale goes above 2 pounds and not postponed until the following day. If a meal is skipped on the day in which a gain is registered in the morning, this brings about an immediate drop of often over a pound. But if the skipping of the mealand skipping means literally skipping, not just having a light meal- is postponed, the phenomenon does not occur and several days of strict dieting may be necessary to correct the situation. Most patients hardly ever need to skip a meal. If they have eaten a heavy lunch they feel no desire to eat their dinner, and in this case no increase takes place. If they keep their weight at the point reached at the end of the treatment, even a heavy dinner does not bring about an increase of two pounds on the next morning, and does not therefore call for any special measures. Most patients are surprised how small their appetite has become and yet how much they can eat without gaining weight. They no longer suffer from an abnormal appetite and feel satisfied with much less food than before. In fact, they are usually disappointed that they cannot manage their first normal meal, which they have been planning for weeks. Losing more weight You should never gain more than two pounds and you should not lose more than 2 pounds either because the loss will be at the expense of normal fat. Any normal fat that is lost is invariably regained as soon as more food is taken, and it usually results in a greater than 2 pound gain. If you are stopping Trileana™ ® but have not yet reached your goal, we recommend you maintain your present weight and restart Trileana™ ® when you are ready.

Trileana™

After Three Weeks of Maintenance You may eventually begin to add in some starches and sugars, but this must be done with caution as some carbohydrates will not be beneficial to the body and may have been a major cause of your original weight gain and unwanted fat storage. Refer to the handout: MORE TIPS ON MAINTAINING FOR THE REST OF YOUR LIFE for detailed recommendations on maintaining your healthy new weight. Continue to weigh yourself every morning after emptying your bladder and without clothes before eating. If the scale goes up by two pounds follow the Skipping a Meal protocol. Relapses Dr. Simeons had 60 to 70% of his cases experience little or no difficulty in holding their weight permanently. When relapses did occur it was usually due to negligence in the basic rule of daily weighing. Many patients think that this is unnecessary and that they can judge an increase from the fit of their clothes. This is a disastrous mistake, because after a course of Trileana™ ® as much as 10 lbs. can be regained without any noticeable change in the fit of their clothes. The reason for this is that after treatment newly acquired fat is first evenly distributed and does not show the former preference for certain body parts. Pregnancy or menopause may annul the effect of a previous treatment. Women who take treatment during one year after the last menstruation- that is at the onset of menopause- do just as well as others, but among them the relapse rate is higher until menopause is fully established. The period of one year after the last menstruation applies only to women who are not being treated with ovarian hormones. If these are taken, the pre-menopausal period may be indefinitely prolonged. If you experience weight gain, coming back for a second course of Trileana™ ® is completely acceptable. Repeat courses are often even more satisfactory than the first treatment and have the advantage that you already know that you will feel comfortable throughout treatment.
