URGENT EQUIPMENT NEEDS VITAL SIGNS MONITORS Every day critical patient care decisions are made based on an essential medical device called a vital signs monitor, which tracks a patient’s vital signs. The QEH needs vital signs monitors for all the wards. Number required: 24 / Estimated Cost $3 000 USD each

FOETAL MONITORS With an average of 3 100 births at the QEH annually, these monitors are a valuable tool for monitoring the wellbeing of the foetus and the progress of labour. Due to sensitivity of the monitor, it may indicate when a contraction is diminishing even before notice of the pain subsiding occurs. Estimated cost: $12 000 USD each

CARDIAC MONITORS The nature of heart disease in Barbados has changed and added to that, the number of cases being diagnosed has risen and continues to rise. With the move to expand cardiac services with the establishment of a cardiac unit at the QEH, there is a need for more cardiac monitors Number required: 20/ Estimated cost $17 000 USD each

ANAESTHETIC MACHINES An anaesthetic machine is a complex collection of equipment used by our anaesthologists to support the administration of anaesthesia. These machines are designed to provide an accurate and continuous supply of medical gases mixed with an accurate concentration of anaesthetic vapour and deliver this to the [patient at a safe pressure and flow. The QEH needs eight (8) of these machines for its surgical department. Number required: 8 / Estimated cost: $95 000 USD each

4

ECG machines An electro cardiogram or (EKG) machine is one of the simplest and fastest procedures used to evaluate the heart. It is also used to determine the cause of unexplained chest pain which could be caused by a heart attack, inflammation of the sac surrounding the heart or angina, the cause of symptoms of heart disease such as dizziness, fainting or rapid, irregular heartbeats (palpitations). Number required: 7 / Estimated cost: $73 000 USD each

SURGICAL LAMPS Surgical lamps are designed to work for extended periods of time without radiating excessive heat.

They provide lighting in surgical suites and are

designed to illuminate the surgical site for optimal visualization of small, lowcontrast objects at varying depths in incisions and body cavities. Number required: 9 / Estimated cost: $35 000 USD each

AUTOCLAVES An autoclave is a pressure chamber used to sterilise equipment and supplies at high pressure saturated steam at121° for about 15-20 minutes depending on the size of the load of the contents. The QEH is in need of nine autoclaves and their acquisition, would go a long way to reducing bacteria, viruses and spores. Number needed: 4 / Estimated cost: $235 000 USD each VENTILATORS A ventilator is a machine that supports breathing. These machines: • Gets oxygen into the lungs • Remove carbon dioxide from the body (Carbon dioxide is a waste gas that can be toxic) • Help people breathe easier • Breathe for people who have lost all ability to breathe on their own. A ventilator is often also used for short periods, such as during surgery when you are under general anaesthesia. Number required: 20 / Estimated Cost: $35 000 USD each

5

EMERGENCY AMBULANCE SERVICE STRETCHERS A stretcher is an apparatus for moving patients who require medical care. A wheeled stretcher is usually equipped with variable height frames containing wheels, tracks or skids. Stretchers are primarily used in acute out-of-hospital care situations by EMS, military or in search and rescue operations. Number required:6/ Estimated cost: $12 000 USD each Two Ambulances

OTHER EQUIPMENT NEEDS TRANSPORT MONITORS When patients are moved to a new location, it is vital that their monitoring information goes with them. The transport monitor provides uninterrupted monitoring so you can stay aware of the patient’s condition even during transport. The QEH is in need of three additional Transport Monitors to facilitate the quick and safe movement of patients throughout the hospital without losing a moment of critical monitoring information. Number required: 4/ Estimated Cost 18 000 USD each BLOOD GAS MACHINE The QEH’s Paediatrics and Accident and Emergency Departments, is in need of two Blood Gas Machines also called the Blood Gas Analysis or the Arterial Blood Gas Analysis. This machine is used to diagnose and evaluate respiratory diseases and conditions that influence how effectively the lungs deliver oxygen to an eliminate carbon dioxide from the blood. Number required 2/Estimated cost US 158 000 USD each

6

INFUSION/ SYRINGE PUMPS The QEH requires additional infusion/syringe pumps for distributed use across all wards, Intensive Care Units ICUs and Recovery Rooms. These pumps administer fluids, medication or nutrients into a patient’s circulatory system in ways that would be impractically expensive or unreliable if performed manually by nursing staff. For example, they can administer as little as 0.1 ml per hour (too small for a drip), injections with repeated boluses requested by the patient up to a maximum number per hour (e.g. in patientcontrolled analgesia) or fluids whose volumes vary by the end of the day. Number required: 30 /Estimated cost $9 000 USD each

LITHOTRIPSY MACHINE A lithotripsy machine is used to treat kidney stones but may be used to treat stones in other organs such as the gall bladder and liver. The process of Lithotripsy uses sound waves to break the stones apart. The QEH is in need of one of these machines for the Urology department to assist medical personnel in the effective management of this condition. Number required: /Estimated cost $ 185 000 BDS

ULTRASOUND MACHINE An ultrasound, also called a sonography, uses sound waves to develop ultrasound images of what’s going on inside the body. Ultrasound imaging has many uses in medicine, from confirming and dating a pregnancy to diagnosing certain conditions and guiding doctors through precise medical procedures. Doctors employ ultrasound imaging in diagnosing a wide variety of conditions affecting the organs and soft tissues of the body, including the heart and blood vessels, liver, gallbladder, spleen, pancreas, kidneys, bladder, uterus, ovaries, eyes, thyroid, and testicles. The QEH is in need of three of these machines to service, Paediatrics, Accident and Emergency and Radiology Departments. Number required: 4 / Estimated cost: US$80 000

OTHER EQUIPMENT NEEDS

INFANT WARMERS Infant (Radiant) Warmer device helps to maintain the body temperature and limit the metabolism in babies. The heat loss in some babies is rapid and the infant warmers hence provide an artificial support to keep the body temperature constant. In certain areas with very cold climate, babies are kept on a Radiant Warmer for couple of hours immediately after birth to ensure the baby is stabilised after birth. Number required: 2/ Estimated cost: $15 000 USD each DEFIBRILLATORS With more Barbadians contracting heart disease, it is important that the Queen Elizabeth Hospital is equipped to manage these cases. One device that is critical in this process is the defibrillator. The process of defibrillation involves the delivery of an electric shock to the heart which causes depolarisation of the heart muscles and re-establishes normal conduction of the heart’s electrical impulse. The QEH needs 10 of them to be used on various wards. Number required: 10/ Estimated cost: $10,000 - $15 000 USD each

SURGICAL DRILLS The cutting-edge surgical procedures in the Department of Surgery have been improved over the years with the donation of modern equipment. The Department is in need of two surgical drills to complement the ones currently in use. The surgical drill is used to drill holes in bone for the attachment of surgical pins, plates or screws. Number required: 2 /Estimated cost: $8 000 USD each

8

COLONSCOPES Colonoscopy is generally recommended when the patient complains of rectal bleeding or has a change in bowel habits or is experiencing other unexplained abdominal symptoms. The test is frequently used to investigate colorectal cancer especially when polyps or tumour-like growths have been detected using a barium enema and other diagnostic tests. A colonscope is a necessary tool in this process and the QEH needs two of them. However, this piece of equipment requires regular replacement. Number required: 2 / Estimated cost: $20 000 USD each PCA PUMPS Patient Controlled Analgesia or (PCA) pumps, is used to assist medical staff in pain management. The PCA pump administers pain killing drugs whenever you need them and without having to wait for the nurse to bring an injection. When you feel pain you press the white button on the handset, which will be in your hand (this is connected to the PCA pump). You will hear a beep to confirm that the pump has acknowledged this request. The pump delivers the drug down a fine length of tubing to the drip in your arm. The QEH is in need of six of these pumps for the Main Operating Theatre. Number required: 6 /Estimated cost: $15 000 USD each

LARYNGOSCOPES A laryngoscope (larynx + scope) is a medical device that is used to obtain a view of the vocal folds and the glottis, which is the space between the cords. This investigation is conducted in two ways. • Indirect laryngoscopy in which ENT specialists use mirrors to examine the larynx and hypopharynx, which is a portion of the passageway to the lungs and stomach • Direct laryngoscopy in which a special instrument, most often a flexible scope is used. The QEH is in need of 10 of these for its operating theatres. Number required: 10 /Estimated Cost: $ 10 000 USD each

9

OTHER EQUIPMENT NEEDS

FLUOROSCOPY MACHINES Fluoroscopy is an imaging technique that allows physicians to utilise x-ray images in real time to see the internal structure and function of a patient. For example, the pumping action of the heart or the motion of swallowing can be watched by doctors. This is useful for both diagnosis and therapy and occurs in general radiology, interventional radiology and image-guided surgery. Estimated cost: $300 000 USD MICROSCOPES Microscopes are used by scientists and medical personnel to see objects which are not visible by the naked eye. There are many types of microscopes but the most common one used is the optical microscope which uses light to image the sample. The QEH’s lab is in need of three of these. Number required: 3 / Estimated cost: $12 000 USD each DOPPLER ULTRASOUND A Doppler ultrasound test uses reflected sound waves to see how blood flows through a blood vessel. It helps doctors evaluate blood flow through major arteries and veins, such as those of the arms, legs, and neck. It can show blocked or reduced flow of blood through narrow areas in the major arteries of the neck that could cause a stroke. It can also reveal blood clots in leg veins (deep vein thrombosis, or DVT) that could break loose and block blood flow to the lungs (pulmonary embolism). During pregnancy, Doppler ultrasound may be used to look at blood flow and to check on the health of the unborn baby. Number required: 8/Estimated cost: $1 000 USD each

10

SONIC WASHER Sonic washer or ultrasonic cleaning is a process that uses ultrasound usually from (20-400 KHz) and cleaning solvent (sometimes ordinary tap water) to clean items. The ultrasound can be used with just water, but use of a solvent appropriate for the item to be cleaned along with the soiling enhances the effect. Cleaning normally lasts between three and six minutes, but can also exceed 20 minutes, depending on the object to be cleaned. Ultrasonic cleaners are used to clean many different types of objects, including surgical items and lenses. The QEH’s CSSD Department is in need of a sonic washer. Number required: 1 /Estimated cost $18 000 USD TOURNIQUET When physicians are treating patients with severe bleeding from trauma, they apply tourniquets (tight bands) to control the bleeding by completely stopping the blood flow. They are also used to prevent the patient from developing shock. Number required: 2/Estimated cost $8 000 USD RESUSCITAIRE The Resuscitaire is the ideal device to have during labour and delivery procedures. It combines an effective warming therapy platform along with the components you need for clinical emergency and resuscitation. The QEH is in need of three of these. Further information on the cost and the number required may be obtained by contacting the Queen Elizabeth Hospital.
