The ACCHH received $1,450,932 in support and revenue in our Fiscal Year 15 – 16. This includes grants secured by the Continuum of Care that are administered by ACCHH. It does not include the value of In-Kind donations.

We do pay utilities and a CAM fee (Common Area Maintenance) totaling 7.4% of our budget.

*Providing services to 300 clients is very labor intensive. Case Management loads must be kept a manageable size to ensure that clients receive the services that they need. Staff is also crucial for a shelter that is open 24 hours a day, 7 days a week, 365 days a year. GRACE also pays 85% of the health insurance costs for individuals and 75% of the health insurance cost for employees and dependent’s coverage

m. What are the Organization’s programs and activities? The ACCHH is responsible for 4 major programs: 1. GRACE Marketplace is the one stop homeless center in Gainesville. GRACE Marketplace opened two years ago and this grant is seeking support for our new Community Services Center. More information on this program is in the body of this grant. 2. The Continuum of Care is a coalition of all the homeless service providers in Alachua County. This group works together to coordinate services within the community, secures Federal and State grants, and reduces duplication of efforts. ACCHH is the lead agency for the Continuum. 3. The Homeless Management Information System (HMIS) is used by all homeless service providers to help keep track of the services provided by each agency for an individual. The data management system helps to provide an overview of the services received by each homeless person. The system insures that all agencies are working together to provide the best services possible without duplication of efforts. 4. Coordinated Entry is the first step in the Rapid Rehousing process. Each person who comes into any agency is evaluated using the super tool called the VI- SPDAT (Vulnerability Index- Service Prioritization Assessment Tool). The tool then provides each person with a score that is used to determine which is the best housing track for them.

o. Is the Organization financially accountable to any other organization? If yes, please explain. ACCHH is financially accountable to our grant funders. We must provide accounting and program data as requested by each funder.

p. What assets does the Organization have that are used in the performance of its functions? We have office equipment and furniture, computers, shelter furniture, and a 15-person van. We also have a lease with the City of Gainesville for the Empowerment Center.

THE PROJECT a. What is the Organization’s Project? Please describe it in detail.

GRACE Marketplace opened its doors 2 years ago as the one stop center for homeless services recommended in the City and County 10-Year Plan to End Homelessness. After an arduous search for a location the City was able to secure a 25-acre former prison located just off 39th Avenue near the Airport and contracted with ACCHH to open and manage GRACE Marketplace on a portion of the campus. The one stop concept is simple and powerful. If you are homeless, or at risk of becoming homeless rather than have to run all over town to get the services that you need, you can come to GRACE and you will be able to connect with everything while your most basic human needs for safety, food, and shelter are met. This initial contact helps to get our clients on the right housing path quickly. We are currently serving between 250 – 300 people a day. There are currently 115 people staying at GRACE and anywhere from 150 to 200 people living in Dignity Village. Dignity Village is the tent city that surrounds GRACE. GRACE is not responsible for the management of Dignity Village, but we do provide services to the people who choose to live there. GRACE offers a wide range of services including • • • • • • • • • • • • • • • •

Four dorms: Men’s (40 beds), Women’s (10 beds), Veteran’s (10 beds), and a low barrier Pavilion (55 beds) serving 115 clients a night A full service commercial kitchen offering 3 meals a day Showers and restroom facilities Free laundry facility Storage for important paper and personal items Mail service Computer Lab Assistance with obtaining an ID – Holy Trinity Episcopal Church Legal assistance – 3 Rivers Legal Service Assistance getting Food Stamps – DCF Mental Health Services – Meridian Medical Services – Wound Care, UF Mobile Medical Bus, HIV Testing Job training – CareerSource and in-house Culinary Training Program Support group meetings NA, AA, Women’s Issues Church sponsored services and Bible Studies Stress management programs- Yoga, self-care,

The problem is that we have run out of space to effectively provide the services our clients require. Service providers are doubled up in offices, sharing space and taking their files back to their main office after each shift at GRACE. Every available space in the buildings we occupy is booked to capacity. The solution to this problem is to the rehab one of the unused buildings on campus into a Community Services Center. This U shaped building has offices, meeting space, 4

Thanks to the Women’s Giving Circle, the building will also house a women’s safe space. Activating the Community Services Center would give us the last piece we need to complete the one stop concept. The 10-Year Plan to End Homelessness called for the campus to be used to provide community services, not just services for the homeless. With the addition of the new Community Service Center we will be able to fulfill this mandate. With the additional space we will be able to increase the number of hours that service providers are on campus and in many cases we will be able to provide real time services rather than just setting up appointments. The buildings at GRACE are sturdy and meant to last. However, since it was a prison there are some challenges. In each concrete wall there is a thick steel sheet that prevents escape and makes it very difficult to run wires. The State did not need to meet building codes when they built these buildings so major rehabs require significant upgrades. Luckily the new Community Services Building does not need a gut and rebuild, but rather an upgrade to HVAC, new handicapped rest rooms, some electrical work and other upgrades. Working with Dr. Bernie Machen we have been able to raise $150,000 to cover the rehab cost of this project but still need funding for a number of items that will make the center more efficient and effective. A detailed list if provided below. 1. Communications system – This includes Wi-Fi, wiring the building and installing two network jacks in each room (4 in some of the bigger ones), 3 wireless conference phones, and expansion of the phone control unit to accommodate 8 lines and 30 more phones (more than 50 total), instead of the 4 lines and 20+ phones we have now. The anticipated cost is $27,527.97 An estimate for the work is attached. 2. Security Cameras and key cards – Unfortunately in this day and age we need to imagine the worst and then plan for it. Because the Community Services Center will house so much personal information we need to be sure that it is secure. Exterior security will be provided by strategically located security cameras that can be accessed at any time by laptop or mobile device. Interior security will be provided by a key card entry system. When a traditional key is lost the only response is to rekey the lock and provide new keys to everyone. This is partially difficult at an old prison facility since the locks are not standard. By moving to a key card system we can deactivate a key card without having to change the entire system. We anticipate this will cost $30,000. An estimate for the work is attached. 3. Campus Information System- Effective communication with 300 clients on a 25-acre campus is challenging. When you add the space covered by Dignity Village it is almost impossible. We have tried to communicate service opportunities by using bulletin boards but that does not allow for real time changes. We want to place four TV screens on campus that are constantly scrolling service information and other messages. This will be extremely helpful in emergency situations (i.e. hurricanes, to notify people about cold night shelter, etc.). These TVs will be placed in the Welcome Center, Café 131 (the dining hall), the Community Services Building, and the Intake office. By using the computer program associated with the system we can add services in real time and

We anticipate that this will cost $ 1,676. An estimate for the work is attached. 4. Energy Efficient window blinds – A prison is built with unobscured line of sight. You can see almost the entire campus for any vantage point. We have been planting trees to soften the look, but it will be years before they provide any shade. That means that GRACE is a hot place on a sunny day. Having a new energy efficient HVAC system will help keep energy costs lower in the building but being able to secure energy efficient window blinds throughout the building would help even more. Currently our GRU bill is roughly $8,000 a month for the entire campus so any efforts at energy efficiency really help. This building has total of 27 windows (20 single windows and 7 double windows). We have priced 2-inch faux wood blinds for a total cost $1,696.73. Information about the blinds is included. 5. ADA Compliant Wayfinding Signs — Because the Community Service Center has 29 rooms signage will be extremely important as clients search for the right office for their appointment. Cost for the sigs is estimated at $2,371.95. Information about the signage is attached,

b. If the Project is funded, how would Rotary be recognized? The Rotary will be acknowledged as a major donor on the donor board to be located in the lobby of the Community Services Center as the second largest donor for this project. A plaque will be placed on each TV used in the Campus Information System. We will feature the Rotary at the ribbon cutting and will acknowledge the Rotary grant in all of our press releases regarding the project.

c. What is the geographic area the Project will serve? (e.g., the City of Gainesville, or Alachua County, or the City of High Springs, etc.) The Community Service Center will be available to anyone in Alachua County who needs services.

d. How does the Project benefit, improve or serve the Alachua County community? The new Community Services Center at GRACE will provide a place where anyone who is homeless, or at risk of becoming homeless, can come to the one stop center to receive the services they need to deal with their situation. By using homelessness diversion tactics we will be able to prevent homelessness in many cases and keep citizens out of a crisis. By centralizing homeless services we are also preventing concentration of homeless people in the woods and on the downtown plaza. When people can access service and are provided a stable place to stay we also decrease the number of trips to the Emergency Room and save on police time dealing with camps spread thought out the community.

e. Does the completion of the Project depend on receipt of this Grant? We will be able to complete the rehab of the Community Services Center, however the Rotary Grant will make the building more efficient and effective, and therefore more productive for the community.

*GRACE shelters only serves single homeless men and women. We do not house families or children. Shelter is provided for them with other organizations. When the new building opens we anticipate that some of the people using our services will be families but it is impossible to determine that number at present time.

USE OF GRANT FUNDS a. How much Grant money is the Organization requesting for its Project from the Foundation? We are requesting $62,500.

b. Is this Grant the major funding source of the Project? No, we have currently raised $150,000 for this project. These funds will be spent on the rehab of the building. The City and the County have also budgeted $18,000 for Year 1 building operations.

With the help of Dr. Bernie Machen and the Community Foundations Amazing Give program we have been able to raise $150,000 for the rehab of the building. We have this cash in hand and are hoping to begin work on the project before the end of the year.

We anticipate that it will take 4 months to complete the work. The pieces that we are seeking funding for can be added when the funds become available.

SUPPORT OF THE WILD GAME FEAST

Participation in the planning and operation of the Wild Game Feast by the recipient is critical to ultimate success of the event. We ask the recipient to provide help for our cleanup days prior to the Feast, at the event itself and for our cleanup day on the Saturday following the Feast. The pre-feast cleanup days are normally the Saturdays immediately preceding the event. In question 14 of the summary please let us know how many people you will provide to help. GRACE has strong volunteer support and we are confident that we can provide the number of volunteers that you need before, after, and during this event.

FOUNDATION POLICY ON LOBBYING

Any contact by either a Grant applicant or someone representing a Grant applicant with a member of the Board of the Foundation for the purpose of promoting the applicant’s request during the selection, presentation or balloting process is a violation of the selection procedure; and can lead to the disqualification of the applicant’s Grant application. Lobbying efforts by a Foundation Board member on behalf of an applicant will have a negative effect on the applicant’s request. Lobbying activity may detract from the merit of an application. The Foundation intends to select what it views as the most worthy project based on the merit of the application and the criteria set forth herein; and not on any lobbying (no matter how well intended). The Foundation intends that the process is fair to all concerned.

CONFIDENTIALITY AGREEMENT

The successful applicant(s) will be required to provide the information requested in 1 g, i and q above within 7 days of notification that they were selected. The applicant(s) will be notified within 5 business days after receipt of the required information whether or not they have met all conditions of the grant. In the event that the Foundation determines that they do not meet the conditions the award will be granted to another applicant. During this period the applicant(s) must not discuss the award.

CONDITIONS OF AWARD

As a condition to funding the project, the successful applicant shall maintain its IRS publication 78 listing on the IRS Select Check web site and other qualifying requirement(s) through the payment of the award. The sums paid must be used for the approved purpose within a reasonable time frame. Funds will be distributed as the project progresses. In the event the total project cost exceeds the grant amount, the amount distributed will be on a pro rata basis. Grant funds must be used for the purposes in the request and as communicated to the Foundation in the application. Distribution of funds will be contingent upon progress in completing the project. The successful applicant will be required to provide the Foundation with quarterly updates on the status of their project beginning on April 1, 2017. The net proceeds generated by the WILD GAME FEAST vary; and therefore a specific amount cannot be guaranteed. The Foundation’s actual contribution will be determined by the Board of Directors of the Rotary Clubs of Gainesville Foundation after the event when the net amount realized can be determined and allocated.

TV Schedule Broadcasting Proposal

Our tech volunteers have looked into possibilities and they suggest 4 “smart” TVs linked to our network. Schedule can be controlled by OnSign software by multiple admins. The four recommended locations are: 1. Welcome Center 2. Café 131 Dining Hall 3. Current Admin/ Case Managers building 4. Community Service Center These locations will provide the most coverage because they are located in places where the clients congregate. OnSign software will allow us to make real time changes to the schedule as needed.
