Happy Camper, Happy Cabana, Kiddie Camper Play Yards by Evenflo Date of recall: June 25, 1997, revised March 8th, 2006 Evenflo has recalled the Happy Camper, Happy Cabana and Kiddie Camper models. The Happy Camper and Cabana playpens are rectangular and the Kiddie Camper is a square. “Evenflo” and the model name (Happy Cabana) are printed on the top side rail. They come in a variety of colors and fabrics. Each folds compactly and comes with a carrying bag and mattress. These play yards were sold in major department, discount, and baby stores and were sold since January 1990. Reason for the recall: There have been three deaths related to these yards. In two cases, the hinges collapsed and entrapped the child. The final case involved an infant being placed in a broken play yard. 107 cases have been reported of children receiving cuts and bruises from broken hinges. If the hinges on the product are not fully rotated, the product can collapse and possibly trap a child in the “V” formed by the folded rails. If leaned or sat on, the rotating hinges in the middle of the top rails can crack or break, presenting a sharp edge or allowing the child to escape. What should you do? Stop using the play yard. Contact Evenflo (800-447-9178) for free hinge covers. o These hinge covers only work if the railing is properly rotated. o The hinge covers also provide additional reinforcement to the sides of the play yard. o They snap on easily but firmly and can be removed (by an adult) for folding. Make sure you fully rotate the top rail hinges when setting up the play yard. Broken play yards should be properly disposed of. Inspect the hinges regularly for signs of cracking or breaking and avoid leaning or sitting on them.

Mesh-Sided Playpens by Graco, Evenflo, Kolcraft, Playskool, etc. Date of recall: November 24th, 1998, revised November 6, 2002 More than 9.6 million play yards/playpens are being recalled from brands like Bilt-Rite, Graco, Evenflo, Gerry, Kolcraft, Playskool, Pride-Trimble and Strolee.

Bilt-Rite, Gerry, Graco, Kolcraft, Playskool, Pride-Trimble and Strolee playpens are drop-sided, mesh playpens that fold in half for storage. Evenflo’s “Houdini” is a portable, mesh playpen that folds up for storage and travel. All Houdinis made between 1994 and 1996 are subject to this recall even if they have plastic covers over the rivets. Those covers need to be replaced because they can crack, break off, or fall off and expose the rivet. Reason for the recall: There have been 8 deaths related to these playpens. In two cases, the hinges collapsed and entrapped the child. The final case involved an infant being placed in a broken play yard. 107 cases have been reported of children receiving cuts and bruises from broken hinges. Standing infants and toddlers can strangle if pacifier strings or loose (or loosely woven) clothing catches on the playpens protruding rivets. What should you do? Stop using the play yard if you find a protruding rivet. More Mesh-Sided Playpen Recalls by Graco, Baby Trend, etc. Date of recall: August 21, 1998, revised May 10, 2004 After two more infants died as a result of being in a previously recalled playpen, the CPSC is again warning parents and caregivers to make sure they do not have older recalled playpens. The following chart includes the deadliest of the older recalled cribs that are still claiming victims. These playpens are all common in that they require the top rails to be twisted around and locked to secure. 13 children have died in recent years from suffocation in a collapsed play yard and portable cribs. Modern playpens have top rails that automatically lock into place when the yards are fully set up.

These cribs have caused the death of several children. Please check your daycare, church, and homes of friends and family to see if any of these play yards have been recalled and should be thrown away.

Playpens by Cosco Date of recall: October 10, 2001 Dorel Juvenile Group is recalling about 102,000 Cosco playpens made between May 1995 and December 1997. The recalled playpens were manufactured by Cosco and sold under the “Zip n Go,” “Okie Dokie” and “Carters” brand names. Only model numbers 05-361, 05-362, 05363, and 05-364 are included in this recall. The model numbers can be found on a label on the bottom of the playpen, on the metal tubular frame near one of the corners. The phrase “Manufactured in China for Cosco, Inc” are written on the label. The 28” x 40” playpens have four mesh sides on a folding metal frame. The side rails have the brand name imprinted on them. The padded floorboard forms a carrying case for the playpen.

The “Zip n Go” and “Carters” playpens were sold at department, toy, and juvenile specialty shops nationwide. The brand name “Okie Dokie” was sold exclusively at JC Penny stores. They sold from May 1995 until January 2000 for between $40 and $70 each. Reason for the recall: The plastic tabs on the playpen that lock the rails into the corners can break or loosen over time, allowing the rails to turn inward, collapse, and entrap an infant. There has been 1 death due to these playpens. An 11-month-old infant died when his chest was caught in the V-shape created by the collapsed sides of his playpen. There have been 421 reports of the rails not locking or collapsing. What should you do? Stop using the playpen immediately. Call Dorel Juvenile Group (800-3149327) to receive a refund or replacement product.

Playpen with Raised Changing Table by Graco Date of recall: September 24, 2003 Graco has issued new instructions for 538,000 “Pack n Play” portable play yards with raised changing tables. This play yard is also sold with an infant bassinet. “GRACO” and “Pack n Play” are written on the sides. They have model numbers 2016, 35235, or model numbers that begin with 9531or 9533. The last three digits of the model number will vary. Those numbers can be found on a label on the white plastic center cone underneath. “Made in China” is also written on the bottom label. Models 2016, 35235, 9531, 0533 that are currently being sold and have green stickers on the boxes, indicating the unit has been modified, are not being recalled. The “Pack n Play” was sold at discount, department, and juvenile product stores nationwide from October 2001 until October 2003 for between $99 and $119 each. Reason for the recall: If children are placed in the playpen when the changing table is still attached, they can crawl under and lift the table up. If this occurs, his head and neck can become trapped between the changing table and play yard rail, causing strangulation.

This recall is designed to inform parents of the hazard and provide warning labels to attach to the changing table encouraging parents to take the table off before placing a child inside. There has been 1 death due to this hazard. A 13-month-old girl died after having her neck trapped between the “Pack n Play” rail and raised changing table. What should you do? Never place your child inside while the changing table is still in place. Contact Graco (800-233-1546) to receive a free warning label to be affixed to the changing table.

Travel Play Yards by Kolcraft Date of recall: September 27, 2007 Kolcraft Enterprises is recalling about 425,000 infant play yards after a 10-month-old child was found strangled by the changing table’s restraint strap. They were sold from January 2001 through September 2007 for between $50 and $130 each. The play yards (or playpens) were sold in a variety of colors and patterns. The model numbers are printed on a white sticker located on of the play yard’s feet. They were made in China. Reason for the recall: All the recalled playpens have raised changing tables with a restraint strap that forms a loop beneath the changing table, posing a strangulation hazard. The Contours 3-in-1 Play Yard has a raised cradle that rocks back and forth. A child can roll and get trapped against the side of the cradle, possibly suffocating. There have been 45 reports of children rolling to the side of the rocking cradle attachment. What should you do? Stop using the play yard immediately. Immediately cut off and remove the existing changing table restraint strap. Remove the raised changing table and cradle or bassinet before placing a baby inside. Contact Kolcraft at 888-655-8484 for a free non-looped replacement strap for the changing table and free repair kit to secure the base of the cradle.

Fisher-Price Rainforest Playpens Date of recall: January 15, 2009 These play yards were portable and were sold with a bassinet, changing table, and mobile. The products have Rainforest patterned fabric, Rainforest themed artwork and bear the “Fisher Price” logo. The Simplicity (manufacturer of this Fisher-Price brand) model numbers are 5310 RNF, 5310RNFC, 5310RNFT and 5310RNFW. You can find this model number on a sticker on one of the legs underneath the play yard. The recalled playpens were sold nationwide from January 2007 through January 2009 for about $100. Simplicity has chosen NOT to recall these products, so the Consumer Product Safety Commission is recalling the products. Reason for the recall: One or more of the rails can unexpectedly collapse, posing a fall hazard or entrapment hazard for babies. There have been at least 1,350 reports of one or more rails collapsing. There are numerous reports of children receiving bumps and bruises when the rail collapsed. 5 injuries included a broken nose, broken wrist, concussion, a cut to the hand requiring stitches, and a chipped tooth. What should you do? Stop using the play yards immediately. Although Fisher-Price did not manufacture or sell these playpens, Fisher-Price has voluntarily agreed to assist consumers who purchased the product (since manufacturer Simplicity is refusing to cooperate).

Travel Tender Play Yards by Simplicity Date of recall: April 7th, 2009 These recalled play yards are portable and were sold with a bassinet, changing table, and mobile features. They exhibit the “Simplicity” logo. The model numbers are 5500DRM, 5500WDS, 5500FEL, 5501FEL, 5502MON, 5520PRO, 5550HAN, 5700MAN, and 5750MIR. The model number is located on a sticker on one of the legs underneath the play yard. The play yards were sold at Burlington Coat Factory stores nationwide and online at Babiesrus.com, Target.com, and Kohls.com from March 2005 through January 2009 for about $100. Reason for the recall: One or more rails can collapse unexpectedly, creating a fall or entrapment hazard. There have been at least 5 reports of the rails collapsing. What should you do? Stop using the play yards immediately. Simplicity, Inc. is no longer conducting day to day operations. Eddie Bauer Soothe & Sway Play Yards Date of recall: May 13, 2009 These portable play yards has a bassinet and a changing station feature. A mobile with three teddy bears is also included. The recalled models are 05046 (all models) and 05044 units manufactured before December 1st, 2008. Different models were sold in Canada. You can find the model numbers and manufacture dates printed on a sticker on one of the support legs underneath the play yard. Manufacture dates are printed in the YY/MM/DD format. These particular play yards were sold at Target, Sears, and Burlington Coat Factory stores nationwide and online from January 2008 through May 2009 for around $150. Reason for the recall: The play yard’s rocking bassinet can tilt when secured by straps (in the non-rocking mode), which can cause it to stay tilted without returning to a level sleeping surface. This tilted sleeping surface can cause an infant to roll to the corner or side of the bassinet and become wedged in the corner or pressed to the side or bottom of the bassinet. This can cause suffocation or positional asphyxiation. There have been 10 reports of infants rolling to the side, including 6 that had their faces pressed against the side or bottom of the bassinet. One baby was reportedly purple and out of breath when discovered. No further injuries have been reported. What should you do? Immediately stop using the bassinet attachment.

Contact Dorel Juvenile Group (888-233-4903) for a $40 voucher toward the purchase of a new Dorel product. You can continue to use the play yard without the bassinet.

Play Yards by Kolcraft, Carter’s, Sesame Street, Jeep, Contours, Care Bear, and Eric Carle Date of recall: July 8th, 2009 1 million play yards are being recalled by distributor Kolcraft. The “Travelin’ Tot” series by Kolcraft is involved, as well as other playpens manufactured for Carter’s, Sesame Street, Jeep, Contours, Care Bear, and Eric Carle. (Model numbers provided below.) Some of these play yards include a bassinet and/or changing table. Some models have a mobile, vibrating unit, or parent organizer. You can find the model numbers printed on a white sticker located on one of the feet. Some of these models were also recalled in September, 2007 after the death of a 10-month old.

Safety 1st Disney Care Center & Eddie Bauer Complete Care Playpens Date of recall: December 30th, 2009 These playpens are portable and were sold with a bassinet attachment and built-in changing station. Models being recalled are 05025, 05026, 05037, 05088 and 05350. You can find the model numbers printed on a sticker located on one of the support legs. “Safety 1st” or “Eddie Bauer” are printed near the bottom on the fabric sides of the play yards. The playpens were sold at Babies “R” Us, Kmart, Sears, Target, and Walmart from January 2007 through October 2009 for between $100 and $130 each. Reason for the recall: The one piece metal bars supporting the floorboard of the bassinet attachment can come out of the fabric sleeves and create an uneven sleeping surface, posing a risk of entrapment and suffocation. What should you do? Immediately stop using the bassinet attachment.
