1. Introduction: 1.1. Quality Management Trainings for Safe Blood Transfusion Blood safety has been identified as one of the seven priority areas of WHO and quality management has been identified as one of the most significant initiative to improve blood safety. WHO conducts trainings on Quality Management globally through its Quality Management Project (QMP). The National Blood Transfusion Programme is conducting a series of 3-day National Training Workshops on Quality Management in 5 different cities, supported by the WHO/OFID Joint Project “Prevention of Transfusion- Transmitted HIV/AIDS and Hepatitis Infections in Priority Countries (Bangladesh, Bhutan, Nepal and Pakistan). The trainings utilize the WHO Quality Management Training Material and deal with theoretical and practical aspects of quality management in blood banking. The 15 Day QMT Material has been condensed by the National Blood Transfusion Programme to a 3-day programme for the target group of blood bank staff. The training objective is to increase the participant level of understanding of the quality systems in a blood bank. The participants include Blood Bank Managers, Medical Technologists or Senior Laboratory Technicians, from public and private sector, who are involved in quality management aspects of a blood bank. A team of facilitators have been identified by the NBTP and the WHO for conducting the 5 training workshops. These experts include experienced Blood Transfusion and Management Experts serving in the public and private sector all over the country.
1.2. Objectives of Workshops The objectives of the workshops are: •
Development of human capital for future needs of the Safe Blood Transfusion Project in Regional Blood Transfusion Centres and the attached Hospital Based Blood Banks.

To improve the skills and knowledge of the Technical Staff of the target Blood Banks.
•
Standardization in the Blood Transfusion Process.
•
Introduction of Quality Systems in Blood Banks & Hospitals of remote areas.
•
Advocacy of importance of documentation in a Blood Bank.
•
Emphasize importance of Proper Screening of Blood.
2. Highlights of Individual Workshops 2.1. Islamabad Workshop The first Quality Management Training workshop was held in Islamabad Hotel, Islamabad on April 28-30, 2011. The workshop started with the recitation of the Holy Quran. Mr. Hasan Abbas Zaheer, National Programme Manager, National Blood Transfusion Programme welcomed all the participants on behalf of the National Programme especially the ones from far flung areas like AJK, FATA and FANA.. In his opening remarks he briefed the participants about the Safe Blood Project and its objectives. The participants were then sensitized about the objectives of the 3- Day training. Mr. Quaid Saeed, National Programme Officer, WHO gave his opening remarks and sensitized the participants about the series of the Five Quality Management Trainings to be held all over the country. He appreciated the interest of the participants to have traveled from some very remote areas of the country to attend the Training. The workshop facilitators then introduced themselves. This was followed by the detailed introduction of the participants in which they informed about the organizations they worked in, the blood collection procedures followed by them there, equipments they use and the importance of maintenance of quality. A Pre- Course evaluation of the participants was conducted then. Participants included Blood Bank Managers, Medical Technologists and Blood Banks Technicians from in Islamabad including Federal Government Services Hospital, Shifa International Hospital, CDA Hospital, Pakistan Red Crescent Society and Kulsum International Hospital. Outstation participants from Azad Jammu Kashmir included Blood Bank personnel from AIMS Hospital, Muzzaffarabad, CMH Muzzaffarabad, DHQ Hospital Kotli, DHQ Hospital Mirpur, DHQ Hospital Rawalakot and DHQ Hospital Bagh. Participants from Gilgit-Baltistan were from DHQ Hospitals from all over the newly established province Gilgit-Baltistan and City Hospital Gilgit-Baltistan. Outstation participants form FATA included personals from AHQ Hospital LandiKotal, AHQ Hospital Ghallanai, Mohammad Agency, AHQ Hospital Khar, Bajaur Agency, AHQ Hospital LandiKotal, Khyber Agency, AHQ Hospital Khar, Bajaur, and AHQ Hospital Ghallanai Mohmand Agency (Annex 6.3). 6
During the proceedings of the interactive Workshop the participants availed the opportunity to discuss their problems which they faced in their respective centers. The trainings were kept interactive so that the blood bank officials from areas like FATA and FANA could share their issues with the help of the team of facilitators. The Tea and the lunch breaks were also arranged in a way that the facilitators were intermingled amongst the participants to have group discussions. The Workshop concluded with a note of appreciation by Dr. Hasan Abbas Zaheer, National Programme Manager, National Blood Transfusion Programme to the participants for travelling so far and keenly taking interest to attend the Training Workshop. He also hoped that the participants and
the National Blood Transfusion Programme will work together for the betterment of Blood
Transfusion Services in Pakistan. The Participants were also provided with certificates. The outstation participants of this workshop included blood bank staff from some very remote regions of the country. They shared their experiences with the local participants and also discussed the challenges that they encounter in discharging their duties in their respective workplaces. These challenges include extreme weather conditions as well as security situations. For most of these outstation participants this was the first ever training opportunity that they had ever received. Extensive interaction during the workshop period between the participants from such diverse backgrounds provided a very good opportunity for sharing experiences. Many participants had travelled more than 24 hours by road to reach Islamabad. All the participants highly appreciated the training activity and deemed it very useful for themselves.
2.2. Karachi Workshop The Second 3-day “Quality Management Training for Safe Blood Transfusion Services” was held on May 19-21, 2011 in Karachi. The workshop was chaired by Dr. Hasan Abbas Zaheer, NPM, NBTP and attended by Mr. Quaid Saeed, NPO, WHO and Dr. Zahid Ansari, PPM Sindh. The Workshop started with the recitation of the Holy Quran. In his opening remarks, Dr. Hasan Abbas Zaheer, National Programme Manager, National Blood Transfusion Programme welcomed and thanked the World Health Organization, OPEC Fund for International Development and the Provincial Blood Transfusion Programme for providing support to the NBTP to organize the workshop.

Dr. Zahid Hasan Ansari Provincial Programme Manager Sindh, Provincial Blood Transfusion Programme in his remarks appreciated the efforts of the National Blood Transfusion Programme and WHO. He welcomed all the participants and appreciated the 100% attendance. Mr. Quaid Saeed, National Programme Officer, World Health Organization on behalf of WHO welcomed all the participants and acknowledged the efforts of the National and the Provincial Blood Transfusion Programme(s) in successfully organizing the workshop in Karachi for the participants from all over Sindh. Outstation Participants included personnel from Civil Hospital, Mirpurkhas, Shah Bhatti Hospital, Hyderabad, CMCH Larkana, GMMMC Sukkur, Civil Hospital, Mirpurkhas, Civil Hospital, Matairi, PMC Nawabshah, Civil Hospital, Jacobabad, Civil Hospital, Badin, Civil Hospital, Sanghar, Civil Hospital, Badin, Civil Hospital, Shaikarpur and Civil Hospital, Dadu (Annex 6.3). The workshop participants represented very diverse geographical and professional settings. The local participants from Karachi had access to modern facilities in their centers but the participants from the rural settings had many challenges and deficiencies in their workplaces. Deliberations were held on these issues during the course of the workshop and experiences shared. The participants were encouraged to share their problems while working in areas where they do not have trained man power, machines to perform their daily duties and other problems such as power shortages. The rest of the proceedings were then conducted in an interactive manner in a two way learning process in which regular feedback apart from the pre and post coarse evaluations was taken and issues understood were recorded and solutions provided. The participants appreciated this method of training and thanked WHO and the National and Provincial Programme(s) for providing them with such an opportunity.

On the sidelines of the workshop, the meeting of the Provincial Steering Committee was also held which was chaired by Mr. Hashim Raza Zaidi, Secretary, Health Department, Sindh. Other members included NPM, NBTP, Dr. Sarwar Channa, Director General, Health Department, Sindh, PPM Sindh, representatives from departments of Law and Finance and private sector members. Mr. Hashim Raza Zaidi, Secretary, Health Department, Sindh also addressed the workshop participants and appreciated the efforts of the National and Provincial Blood Transfusion Programme and acknowledged the fact that WHO and OFID had joined hands with NBTP to arrange such a Workshop. Mr. Zaidi confirmed the notification of Mr. Zahid Hasan Ansari as the Provincial Programme Manager, Provincial Blood Transfusion Programme and appreciated his dedication to the provision of Safe Blood Products to the public of Sindh. He informed that through the Safe Blood Programme four Regional Blood Transfusion Centers will be built in Sindh and attached with those centers twenty four Hospital Based Blood Banks will be refurbished. He expressed the hope that this will improve the Blood Transfusion Services to a great extent and said that if we join hands with the National Programme it will bring much improvement in the Blood Transfusion Services in the province of Sindh. He advised the participants of the workshop to take notes and clear their minor misunderstandings regarding the working of their respective blood banks. Dedication to a field often over rules the power of money were his concluding remarks to the blood bank officials. Dr. Hassan Abbas Zaheer, NPM, NBTP thanked Mr. Hashim Raza Zaidi, Secretary, Health Department, Sindh for his presence, Dr. Zahid Ansari, Provincial Blood Transfusion Programme Manager for his efforts and Dr. Quiad Saeed, NPO, WHO for their coordination with NBTP.
2.3. Lahore Workshop The third 3-day Workshop on “Quality Management Training for Blood Transfusion Services” was held in Lahore. The workshop was chaired by Dr. Hasan Abbas Zaheer, NPM, NBTP and attended by Mr. Quaid Saeed, NPO, WHO and Dr. Mehfooz-ur-Rehman, Director I.B.T.S, Punjab. The workshop in Lahore started with the recitation of the Holy Quran and the introduction of the participants and the facilitators. This was followed by the opening remarks by Dr. Hasan Abbas Zaheer, NPM, NBTP who welcomed all the participants, appreciated the efforts of WHO and 9
OPEC which have taken interest in provision of Quality Services for Blood Transfusion Services in the Islamic Republic of Pakistan and also thanked the Provincial Blood transfusion Programme for their support. He briefed the participants about the objectives and achievements of the National Blood Transfusion Programme, the scope of the WHO-OFID project and the technical collaboration provided by WHO to the NBTP. Mr. Mehfooz-ur-Rehman, Director, Institute of Blood Transfusion Services, Punjab, acknowledged the efforts of the National Blood Transfusion Programme and its leadership and WHO, who had provided the Blood Bank Officials of Punjab with such a good learning opportunity. Mr. Quaid Saeed, NPO, WHO appreciated the participants for travelling from far, flung areas of Punjab to attend the workshop. The proceedings of the Workshop started with the Presentation by the Director I.B.T.S on Importance of Quality in Blood Supply System. The participants included Blood Bank Managers, Medical Technologists and Senior Laboratory Technicians of Blood Banks in Lahore including Institute of Blood Transfusion Services, Jinnah Hospital, Services Hospital, PIC Hospital, SGR Hospital, M.N. Hospital Yakki Gate, Said Mitha Hospital, Fatmid Foundation, Sundus Foundation and Surriya Azeem Hospital. Outstation participants included participants from M.B.S Hospital, Rawalpindi, DHQ Hospital, Jhelum, DHQ Hospital, Sialkot, DHQ Hospital, Sheikhupura, Allied Hospital, Faisalabad, Civil Hospital, Faisalabad, DHQ Hospital Okara, BHQ Hospital, Sahiwal, Nishter Hospital, Multan, DHQ Hospital, Sargodha, DHQ Hospital, Gujranwala, BVH Hospital, Bahawalpur, Sundus Foundation Gujrat, Sundus Foundation Gujranwala, and Bilal Hospital Rawalpindi (Annex 6.3).
The Workshop format followed the same structure of presentations as the earlier workshops and was kept interactive so that the participants could be kept involved. The Workshop concluded with a vote of appreciation to the National Blood Transfusion Programme and WHO for their efforts in organizing the Workshop. Dr. Quaid Saeed also participated in the concluding ceremony and thanked the Provincial government for their coordination and appreciated the efforts of NBTP Team for their timeless efforts for the provision of better Blood Transfusion Services in Pakistan. Certificates were distributed to the participants in the end.

2.4. Nathia Gali Workshop: The fourth 3-day workshop on “Quality Management Training for Blood Transfusion Services” was conducted for Khyber Pakhtun Khwa province participants in Nathia Gali on July 6-8, 2011. Prof. Fazl e Raziq, Focal Person, KPK, Mr. Ozair-ul-Haq Lodhi, Jr. Programme Officer, NBTP and Dr. Quaid Saeed, National Professional Officer, WHO inaugurated the training workshop in which the participants were briefed about the training objectives, the objectives of the National Blood Transfusion Programme and the importance of such trainings in the future. The training programme started with the recitation of Holy Quran. The facilitator and participants introduced themselves and their respective hospitals and blood banks. Prof Fazl-e-Raziq, Provincial Programme Manager, KPK, in his opening remarks highlighted the importance of quality in blood transfusion services and operations. Mr. Ozair Lodhi, Junior Programme Officer NBTP thanked all the participants and briefed about the National Blood Transfusion Programme, its objectives and achievements. He informed about the objectives of the Quality Management Trainings and that the
NBTP has already conducted three training workshops on quality
management for blood transfusion services in Islamabad, Karachi and Lahore. Dr. Quaid Saeed, National Programme Officer WHO shed light on the activities of WHO in Pakistan and talked about the 23 different programmes which WHO is supporting in different areas of Pakistan. Dr. Sarfraz Jafri, Director Husaini Blood Bank talked on the subject of Screening of blood and WHO trainings and conducted the Pre assessment forms filling. The participants were the staff of the Blood Banks which included Blood Bank Managers, Medical Technologists and Senior Laboratory Technicians of Khyber Pakhtun Khwa and FATA from Lady Reading Hospital, Blood Bank Hayatabad Medical Complex, Hamza Foundation, Peshawar, Fatmid Foundation Peshawar, DHQH Bannu, DHQH Battagaram, DHQH, DI-Khan, DHQH Karak, IKD Peshawar, DHQH Timergara, DHQH Mansehra, DHQH Abbottabad, LMH Kohat, DHQH Mardan, City Hospital Kohat road Peshawar, City Hospital Kohat road Peshawar, Orakzai Agency, A.H.Q Miran Shah, A.H.Q Hospital Wana, Pathologist /BBO ATH Abbottabad, Hematologist Khalifa Gul Nawaz Hospital Bannu, Chief Blood Bank Technician KTH, Blood Bank HMC, SBT HMC (Annex 6.2).

The lectures and discussions of the training programme were based on Importance of Quality in Blood Supply System, Standards of Quality System in Blood Transfusion, Standard Operating Procedures, Hygiene and Safety in BTS, Donor Care Satisfaction and Motivation, Documentation in Quality System, Blood Component Production and Management, Role of Training Programme in Skill Building. The concluding ceremony was conducted by Mr. Rajwal, Focal Person, WHO, KPK who invited the Provincial Blood Transfusion Programme, National Blood Transfusion Programme and World Health Organization to give their concluding remarks. The three partners thanked each other and appreciated the participants to have travelled all the way to attend the training. The Workshop ended with the distribution of certificates. The results of pre-evaluation and post-evaluation tests show the 25% increase in knowledge. Pre and Post evaluation forms were filled by the participants prior to and after the workshop respectively. The results of the Evaluation forms clearly reflected the increase in knowledge of the participants. Percentage of correct answers in the evaluation forms before the training workshop was 45 and after the workshop was 70 which showed 25% increase in knowledge. The participants thanked NBTP, Provincial Health Department and WHO for inviting them and urged to include them in future capacity building workshops as they needed such trainings to support their in comparison remote area of Safe Blood Transfusion.
3. Facilitators: The team of facilitators gathered by the NBTP includes qualified and competent professionals of Blood Transfusion field. Most of the facilitators have specialized academic background in the Blood Transfusion field. These people are serving in both public and private firms. These facilitators travel domestically to different cities where ever the QMT workshops are being conducted with the aim to make the BTS better and for the advocacy of the importance of quality in the Blood Transfusion field. The profiles of the facilitators attached in Annexure 6.1.

4. Participants: The participants of the workshop are selected from the Blood Bank staff which included Blood Bank Managers, Medical Officers, Medical Technologists and Senior Laboratory Technicians. Priority was given to those Blood Banks which will be supplied by the Regional Blood Transfusion Centers from the Safe Blood Project. These participants need such trainings as they will be at the service delivery end once the safe blood project has constructed the RBTC’s. Target group of participants by NBTP are designed in such a way to support their idea to develop human capital for the future needs in the RBTC’s and the HBBB’s. The details of the participants in each workshop are attached in Annexure 6.1.
5. Programme of the Workshops The technical programme of the workshops has been derived from the 15 day Quality Management Programme of WHO and has been adapted to a 3- Day training programme. The workshops started with the recitation of the Holy Quran and followed by Pre- Coarse evaluation which is recorded by the NBTP team. The Inaugural session begins with the welcome note and training objectives from the National Programme Manager, National Blood Transfusion Programme followed by opening remarks by the Provincial Programme Manager, Provincial Blood Transfusion Programme and the National Programme Officer, World Health Organization. First day presentations include Importance of Quality in the Blood Supply System, Introduction to Quality Systems, Documentation in Quality systems, Standard Operating Procedures (SOP’s) and a group activity to develop the SOP’s. The second day include presentations on Principles of good manufacturing practices, Role of Training in Quality Systems, Steps in developing a Quality System, Hygiene and Safety in Blood Transfusion Services, Donor Mobilization and Selection, Blood Collection, Donor care,

Satisfaction and Retention and ends with Developing a Documentation System for Blood Donor Management. The third day presentations include Introduction to Quality Systems in Blood Component Production and Management, Quality Monitoring of Blood Component Production, Storage & Transportation of Blood Components, Evaluation and Use of Immunohaematology Reagents, Evaluation and Use of Test Kits for Transfusion-Transmissible Infections, Developing a Documentation System for the Laboratory, Review of Quality Systems, Post-Course Evaluation and then after recording the post course evaluation the 3- Day training workshop is concluded by remarks from the NPM, NBTP, PPM, PBTP and NPO, WHO. The pre and the post course evaluation forms are evaluated and the percentage differences between the forms of the individual participants are then calculated. This gives (in percentage) the benefit of the Workshop to the individuals and the “Knowledge gained”. 5.1

6. Annexure: 6.1. Facilitators Profile:
6.1.1. Dr. Hasan Abbas Zaheer Dr. Zaheer, a hematologist by training is currently serving as the National Programme Manager, National Blood Transfusion Programme, Ministry of Health, Dr. Zaheer is the pioneer of the blood safety programme in Pakistan. He led the process of conceiving, developing, processing, approval and establishment of this project which envisages universal access to safe blood for transfusion in the country. The project development process was preceded by formulation of a National Strategic Framework on Blood Safety and a National Blood Policy, both of which were also developed under his leadership through an extensive national consultative process. Earlier he has served with distinction as the National Programme Manager of the National Aids Control Programme from where the present project was also developed. Before that he served as the Associate Professor (Hematology and Transfusion Medicine), Pakistan Institute of Medical Sciences, Islamabad.
6.1.2. Dr. Zahid Ansari Presently working as the Provincial Programme Manager, Sindh and the Secretary, Sindh Blood Transfusion Authority. He is the first notified Provincial Programme Manager of the Safe Blood Programme. Prior to this he was working as a Consultant Pathologist, Civil Hospital, Karachi and Chief Pathologist Services Hospital, Karachi. He is also the Advisor / Member Sindh Public Service Commission. Dr. Zahid is a qualified Pathologist and a Transfusion Medicine expert. He

received his post graduate training from Hammersmith Hospital, London and did his DRCP from there. He is also a Masters in Management Sciences from IMS, Karachi.
6.1.3. Dr. Sarfraz Jafri Dr. Sarfraz. H. Jafri is the Administrator, Husaini Blood Bank; He is also the Visiting Faculty Member, Baqai Institute of Hematology, BMU, Karachi. He is also the HBB Head of Thalassemia/Hemophilia Department, Health Management Consultant, Member National and Provincial Steering Committees on Blood Safety, Member Sindh Blood Transfusion Authority Health Department Government of Sindh, Facilitator WHO QMS Blood safety training program, Member Hepatitis Advisory Board (PMRC) Pakistan, Ex-Project Director of Global Fund National Blood Safety Training Programme, Spokesman Thalassaemia Federation Pakistan.
6.1.4. Dr. Saeed Ahmed Presently working as Consultant, Transfusion Medicine, Husaini Blood Bank, Karachi. Earlier, he has served as the Director Development & Services, Pakistan Blood Bank, Karachi. Dr. Saeed has recently completed his MD in Transfusion Medicine from baqai Medical University, Karachi. He has participated in a number of national and international conferences and workshops regarding blood transfusion and is a very expereienced and successful Facilitator of numerous national workshops.
6.1.5. Dr. Sohail Rabbani Dr. Sohail Rabbani is a graduate of King Edward Medical College, Lahore. He did his Masters of Science in Epidemiology from the USA and has extensive experience of this field in USA. For the last four and a half years (May 2007 to Present) he has been in Pakistan working as a freelance consultant on projects related to TB, Hepatitis and HIV prevention and control. Besides doing voluntary work he has also worked on various assignments relating to blood borne viruses and TB control. He has also worked in UK on a number of assignments pertaining to epidemiologic and social sciences.
6.1.6. Dr. Riaz Subhani Dr. Riaz-ul-Haq Subhani is a Consultant Clinical Pathologist and Hematologist with Diploma in Clinical Pathology (D.C.P) and a M. Phil Hematology. Mr. Subhani has an extensive experience of teaching, nursing and other health related disciplines. He has a medical experience of working in Primary, Secondary and Tertiary Health Care system. Along with the experience of working as a

technical expert has also worked in the NGO sector in addition to working in media as a writer, anchor, idea framer and set designer.
6.1.7 Prof. Mehfooz-ur-Rehman Prof. Mehfooz is the Director, Institute of Blood Transfusion Services, Punjab. He is a qualified Hematologist and has served earlier as the Professor of Hemaology in Allama Iqbal Medical College Lahore. He has a number of publications relating to transfusion medicine to his credit. He is also the Focal Person, Blood Safety in Punjab.
6.1.8. Prof. Fazl-e- Raziq Prof. Fazl-e-Raziq is professor of Hematology and currently serving in Hayatabad Medical Complex (HMC) Peshawar. In addition to this he is also performing his services as Focal Person Blood Safety for Khyber Pakhtun Khaw regarding the activities of Safe Blood Transfusion Project.
6.1.9. Mr. Bilal Ahmed Tareen Currently serving as Technical Expert in the National Blood Transfusion Programme, Mr. Tareen is an experienced Laboratory Technician and has served for more than 20 years in the Blood Transfusion Services in Pakistan Institute of Medical Sciences.
6.1.10. Mr. Ozair-ul-Haq Lodhi Working as a Junior Programme Officer, Mr. Ozair is a Management Sciences expert. He has been serving in the National Blood Transfusion Programme since the Programme was established in 2010. He has a good background of managing seminars and workshops including the logistics and other arrangements. He has a special interest in IT and has been coordinating the QMT Workshops in NBTP. Previously he has worked with USAID as a Junior Communications Officer in the TACMIL Health Project.
