Welcome Your benefits help you take care of yourself and lead your healthiest life. You’ll get the most from them when you take an active role. That means knowing how they work and where you have opportunities to keep your costs lower. This guide can help!

Get a Quick Start to Wellbeing for 2016! Start off the new year by earning wellbeing dollars. You’ll learn more about your health and how to improve it. Plus, you’ll earn money in your medical plan account. 1. S  chedule and complete a biometric screening. You can schedule it online at a Quest Patient Center or with your doctor. For more details, see page 17. 2. C  omplete the RedBrick Compass online health assessment. It’s a short online questionnaire that only takes 15 minutes! If you have your biometric screening results handy, you can add them when you complete your Compass questionnaire. For more details see page 17. Don’t forget, if you enrolled in the Silver HSA plan or Bronze HSA plan, you must open a Health Savings Account (HSA) to collect any earned wellbeing dollars. You can open the account on the OneExchange benefits website. For more information about the wellbeing program, see details starting on page 14.

Looking for Your ID Cards? Depending on your carriers, you may or may not have received ID cards in the mail. With most carriers, you can print an ID card from their member website or access it from their mobile app. For a complete summary of which carriers and services provide cards, go to the OneExchange benefits website and review Your 2016 Benefits ID Cards in the Resources section under Health Forms and Documents. Log on to myRioTinto.com, then go to myBenefits > Benefits > Health and Welfare Benefits Or, log on to RioTintoUSBenefits.ehr.com (make sure your browser is Internet Explorer 9 or higher, or Google Chrome).

Important Health Care Terms Annual Deductible

An amount you pay for the care you receive before your plan begins paying benefits for the year.

Coinsurance

After you meet your deductible, this is a percentage of the cost of your care that your plan pays. You pay the remaining amount.

Contribution Amount

What you pay for coverage from your paycheck. The money comes out before taxes.

Out-of-Pocket Maximum

The most you will pay for covered care during the year, including your deductible. If you reach this limit, your plan pays 100% of eligible expenses for the rest of the year.

Preventive Care

Services that help you stay healthy, like annual physicals, well-child visits, immunizations and routine screenings. What’s covered for you depends on your age, gender and family history. Your doctor can help you find the preventive care services that are right for you.

Provider

A doctor, hospital, lab or other facility that provides health care services.

Managing Your Costs No matter what your health care needs are — big or small — you’re bound to have some expenses to manage during the year. It’s important to understand how your benefits work. Here are some tips for keeping your costs down:

“Shrink” Your Annual Deductible with Wellbeing Dollars

Get Your Care In-Network Your medical plan offers in-network and out-ofnetwork benefits. When you use in-network providers, you get your care at a big discount. With out-ofnetwork care, you’ll pay more.

At first glance, your annual deductible might seem like a big number. But you can use wellbeing dollars from the company to lower what you pay out of your own pocket to meet that deductible. You earn wellbeing dollars when you complete wellbeing activities through RedBrick Health (or through Kaiser Permanente if you’re enrolled in a Kaiser Permanente medical plan). Once your earned wellbeing dollars are deposited in the account connected to your medical plan, you can use them to help cover your deductible. (You can also use them for other eligible expenses after you have met the deductible.)

Please note: If you’re enrolled in a Kaiser Permanente medical plan, out-of-network benefits aren’t available, except in an emergency.

Stay Healthy for Free with Preventive Care Annual physicals, routine screenings, immunizations and more are covered at 100% at no cost to you when you stay in-network. Even some prescription drugs are considered preventive. Your carrier has a list of preventive drugs that are covered at little or no cost to you.

Pay Tax-Free for Out-of-Pocket Expenses with Your Account

Use Generics Instead of Brand-Name Drugs

The account (HRA or HSA) connected to your medical plan lets you pay for out-of-pocket costs tax-free when you use the money on eligible expenses. For more details about the accounts and how they work, click here.

Generic drugs are just as effective as brand-name drugs and almost always your lowest-cost choice. If you need a brand-name drug, visit your carrier’s member website to review its preferred drug list (sometimes called a formulary). Brand-name drugs on that list cost less.

What Happens at the Doctor’s Office When you or a covered family member needs care, just follow these steps:

1.

Make an appointment with your provider. In-network providers are always more affordable.

2.

Show your ID card at the provider’s office. If you don’t have your ID card, log onto your carrier’s member website or call their toll-free number for a replacement. Some carriers have convenient mobile apps to access your card information, too. Need your carrier’s contact information? Click here.

3.

Get the care you need. With an in-network provider, you don’t need to fill out a claims form. If your plan includes a copay, you’ll pay it when you get care. If your plan doesn’t include a copay, you won’t pay anything at the doctor’s office. Instead you’ll receive a bill if there’s anything you owe. If you visit an out-of-network provider, you may have to submit a claim form to your carrier.

4.

Review your Explanation of Benefits (EOB). You’ll get this after your carrier processes your claim. You can review it on your carrier’s website, or you may receive it in the mail. Your EOB provides a summary of the services you received, the benefits your plan provided, and what you’re responsible for paying.

5.

Pay any balance you owe. You’ll receive a bill from your doctor if the charges for the visit are more than what the plan covers. You can pay for these expenses out of your pocket or with any money you may have in your HRA or HSA!

What Happens at the Pharmacy This is the place to go if you have a prescription that you’ll need for a short time. Just follow these steps:

Get a prescription from your doctor. Be sure to talk to your doctor about a generic drug if it’s available. It’s your most affordable option.

2.

Show your prescription drug ID card at the pharmacy. (Your prescription drug ID card may be the same or different than the one you use for medical care. For details, click here.) At the pharmacy, you can get up to a 30-day supply of your original prescription plus one refill once the original supply runs out. After that, if you still need the medication, you’ll need to get it through mail order.

3.

Pay for and receive your medication. You’ll pay for whatever you owe at the time you pick up your medication. You can pay for these expenses out of your pocket or with any money you may have in your HRA or HSA. How much you pay depends on the type of medication you get. You’ll pay the least for a generic drug, more for a brand-name drug on your carrier’s preferred drug list, and the most for a brand-name drug not on your carrier’s preferred drug list. If you choose a brand-name drug when an equivalent generic drug is available, you’ll pay your share of the cost for the generic drug plus the cost difference between that drug and the brand-name drug you choose.

Using Mail Order Mail order is a time saver and a money saver. You’ll save time by receiving your long-term medications through the mail instead of having to pick them up at the pharmacy. You’ll save money because you can get a larger supply of your medication for a lower cost than what you’d pay for the same amount at a retail pharmacy. Mail order is the only option for long-term prescriptions. You can find more about your carrier’s mail order program on the member website.

To use mail order, follow these steps:

1.

Get a prescription from your doctor. Remember, generics are your lowest-cost option. Whenever you get a prescription for a new medication, you may want to ask for two prescriptions: one for a 30-day supply you can pick up at a local pharmacy, and then the 90-day mail order prescription, which will take a few days to process.

2.

Send in your mail order prescription. Send it to your prescription drug carrier, along with a completed mail order form the carrier provides in your welcome kit, online on your carrier’s member website, or by calling your carrier directly to request one. Need the phone number? Click here.

3.

Receive your medication. It will come to the address you select. In some cases, you can request automatic refills to make the process even easier.

If you’ve recently enrolled in a Rio Tinto medical plan for the first time, or if you’ve switched carriers, you’ll need to ask your doctor to write you a new 90-day prescription. Your new prescription drug carrier can activate the mail order process for you. Any mail order prescriptions from a previous carrier won’t carry over.

Resources to Save You Time and Money When you enroll in a Rio Tinto medical plan, you have a whole host of tools and resources to help you save money when you need support or care. Why spend more if you don’t have to? Take a quick look through this section to learn about the ways your benefits work to make getting care more convenient and affordable. Telemedicine. Life gets busy, and you don’t always have time to wait for an appointment. That’s where telemedicine comes in. Telemedicine visits work a lot like seeing the doctor in person, but without the travel time and the waiting. You’ll be able to talk to a high-quality, in-network doctor who can discuss your health care concerns, review treatment options and even prescribe medication over the phone or virtually (depending on your location). Contact your medical carrier to learn more about telemedicine options that may be available.

 upport for an ongoing health condition. S Sometimes the health care we need may go beyond routine doctor’s office visits. If you or a family member is managing an ongoing health condition, treating an illness or wanting to ensure a healthy pregnancy, you can get a lot of support from your medical carrier to make the process easier. Reach out anytime to your medical carrier’s member services department to start the conversation.

C  ost look-up tools, so you know before you go. Health care costs can differ widely depending on the provider you choose, location, treatment option and more. That’s why it’s important to know that you can shop around for benefits just like you would any other consumer item. Your carrier can make it easier to shop around with its cost look-up tools. Sometimes they’re called “cost transparency tools” because they can give you a clear view into the cost of different treatment options so you can make smart decisions about your care. Contact your medical carrier to get connected to its cost transparency tools.

 urseline. N If you have a health care question but you’re not sure who to ask, contact your medical carrier’s nurseline. The nurseline can be a great source of advice, and is available 24 hours, 7 days a week.

Don’t forget to visit the OneExchange benefits website throughout the year whenever you need to review or manage your benefits. In addition to learning and enrolling, you can also make benefit updates if you experience a qualified life event, and report it to the OneExchange benefits website or Rio Tinto Service Center within 30 days of the event date. It’s also a good idea to review your beneficiaries for life insurance coverage to make sure they’re up to date.

Your Accounts Each medical plan makes an account (HRA or HSA) available to help you save and pay tax-free for eligible out-of-pocket expenses. Wellbeing dollars you earn through the wellbeing program go into your account. If you’ve chosen an HSA-eligible medical plan, you can grow your account balance in other ways, too.

Health Savings Account (HSA) If you’re enrolled in an HSA-eligible medical plan, you were able to open a WageWorks Health Savings Account (HSA) when you enrolled. If you didn’t open your HSA, it’s not too late. You can open it at any time as long as you meet the eligibility requirements. Don’t forget, opening an HSA is the only way for you to be eligible to receive employer contributions and earned wellbeing dollars from Rio Tinto, as well as any employee contributions you may have elected. You can open an HSA on the OneExchange benefits website. If you need assistance, call the Rio Tinto Service Center at 1-800-778-4313. Here’s how it works.

Tax-Free Money Goes In

Tax-free money can go in three ways:

Tax-Free Money Comes Out Use your HSA money for eligible out-of-pocket medical, prescription drug, dental and vision expenses. If you don’t spend the money in the account, the balance at the end of the year “rolls over.” It’s yours to keep and spend tax-free on eligible health care expenses whenever works best for you, even if you leave the company.

How much can you contribute? Because the HSA is such a great way to save on taxes, the Internal Revenue Service (IRS) sets limits on account contributions. Here are the limits for 2016: 2016 Limit

If you are 55 or older, you can contribute an additional $1,000 in catch-up contributions.

It’s up to you to make sure that total HSA contributions for 2016 don’t exceed the annual limit set by the Internal Revenue Service. That limit applies to contributions from the company, including wellbeing dollars, and any of your own contributions.

When can you start using the account? You can start using your WageWorks HSA as soon as there’s enough money in the account to cover an eligible expense. You can’t use it for any expenses incurred before you opened the account. If you have an HSA with another financial institution, you can use it to pay for expenses you may have had before opening your WageWorks HSA. But as long as you’re enrolled in a medical plan through the company, the WageWorks HSA is the only account that allows tax-free contributions. For more information about the HSA, visit www.wageworks.com. Already have an HSA with WageWorks? Continue to use your current account. Moving? If you change your address after you open an account with WageWorks, you must update your address directly with WageWorks by logging in to www.wageworks.com and clicking on the Profile link at the top of the page.

Health Reimbursement Account (HRA) If you’re enrolled in an HRA-eligible medical plan, your HRA was opened automatically when you enrolled. You don’t contribute to the HRA. The balance grows only through wellbeing dollars from the company when you complete eligible wellbeing activities through RedBrick Health (or through Kaiser Permanente if you’re enrolled in a Kaiser Permanente medical plan). You can use your HRA balance to pay for eligible out-of-pocket medical and prescription drug expenses. It cannot be used for dental and vision expenses. Any HRA balance you have at the end of the year (up to $6,000) will “roll over” for your use the next year as long as you stay enrolled in an HRA-eligible medical plan. Managing Your HSA or HRA WageWorks makes it easy to manage your account. When you visit www.wageworks.com, you can: • Update your account preferences and personal information • Schedule payments to health care providers • Check the complete list of eligible expenses for your account • Order additional WageWorks debit cards for your family • Manage your account while on the go via the WageWorks mobile website • Download the EZ Receipts® app so that you can take care of paperwork from your smartphone or mobile device • Access your HSA bank account (if you’re enrolled in a plan with an HSA) WageWorks gives you several options to use your HSA or HRA money the way you choose: with your WageWorks debit card, directly through online payments to your provider, or via a request to pay yourself by direct deposit or check. But payment from the HRA doesn’t happen automatically. You need to take action by using your WageWorks debit card or submitting a claim for reimbursement.

Other Accounts Health Care Flexible Spending Account

Dependent Care Flexible Spending Account

There are two types of Health Care FSAs.

This account lets you set aside pre-tax money to pay for eligible dependent care expenses for:

 tandard Health Care FSA. This type of Health • S Care FSA is available if you enrolled in an HRA-eligible medical plan, you waived coverage or if you are not eligible for an HSA. It lets you set aside pre-tax money each pay period and reimburse yourself tax-free for eligible medical, prescription drug, dental and vision expenses at any point during the year.

• A ny child under age 13 you can claim as a dependent for federal income tax purposes • Y our spouse/domestic partner or other dependent, regardless of age, who: – Is physically or mentally incapable of self-care, and – Spends at least eight hours a day in your home, and – You can claim as a dependent for federal income tax purposes

• Limited Purpose Health Care FSA. This type of Health Care FSA is available if you enrolled in an HSA-eligible medical plan and you qualify to contribute to an HSA. It lets you set aside pre-tax money each pay period and reimburse yourself tax-free for eligible dental and vision expenses only. For eligible medical and prescription drug expenses, you can use your HSA (if you established one).

The expenses must be for day care while you or your spouse are at work. You may not, for example, claim the cost of babysitters you hire when attending social events. If you are married, your spouse/domestic partner must work or be actively seeking work or be a full-time student. You can contribute up to $5,000 to the account each year. If you are married, the maximum annual contribution between you and your spouse/domestic partner is $5,000.

You can contribute up to $2,550 in these accounts in 2016. But there’s a “use it or lose it” feature so any remaining balance in your Health Care FSA at the end of the year will be forfeited.

Save Your Receipts! For all accounts, be sure to keep detailed receipts and Explanation of Benefits (EOB) for any expenses paid through your WageWorks accounts. From time to time, WageWorks may request documentation to substantiate a claim, and the IRS conducts audits of how these accounts are used.

It’s offered through RedBrick Health (or through Kaiser Permanente if you enrolled in a medical plan with that carrier). The wellbeing program offers lots of ways to earn wellbeing dollar rewards. Whether you want to eat healthier, lose weight, reduce stress, quit tobacco products or something else, there are wellbeing activities that can help you work toward and achieve your goals. And the best part? You’ll get paid for it!

More About Wellbeing Activities RedBrick Compass www.my.blueprintforwellness.com and enter “RioTinto”. A biometric screening received at a lab not affiliated with Quest will not be eligible for wellbeing dollars from the Company.

RedBrick Compass is a short online questionnaire on the RedBrick Health website that gives you a quick way to earn wellbeing dollars while you learn how your current lifestyle (for example, your diet, how often you exercise, how you manage stress) might affect your health. It only takes 15 minutes to complete! It’s the perfect first step in your wellbeing pathway, and can help you focus on other wellbeing activities that can help improve your health.

RedBrick Journeys RedBrick Journeys are online programs that make it easy and fun to focus on a particular health or lifestyle issue. They’re called “Journeys” because they guide you through a series of small steps. Journeys are designed to last a few days to a few weeks and help you shape healthy habits in a broad array of areas including physical activity, nutrition, financial wellbeing, stress and sleep. Journeys can also help you better manage chronic conditions like diabetes, asthma and heart disease.

Biometric Screening A biometric screening is a simple test that gives you some good indicators of your current health. It can help identify risks you might face and gives you a chance to learn what you can do to stay healthy. It’s another activity that’s good to complete early in the year, since you’ll earn wellbeing dollars for completing it and it can help point you to additional wellbeing activities that address any identified health risks. The biometric screening is available at no cost to you, and it’s a great way to start earning wellbeing dollars for 2016.

You’ll find over 60 unique Journeys on the RedBrick Health website, and you’ll earn wellbeing dollars for each milestone you complete along the way (like starting a Journey, completing a Journey stage and evaluating the Journey at the end).

Your spouse or partner is also eligible for the biometric screening if he or she is enrolled in your OneExchange medical plan. You have some options about how to get your screening:

Phone Coaching There’s no one-size-fits-all approach to changing health habits. So, along with the online tools available to you, you can also connect one-on-one with a RedBrick Health coach who can provide personalized support to help you address a specific health risk or make a lifestyle adjustment to improve your health. There are eight different phone coaching programs available, and you earn wellbeing dollars for each coaching call you complete. It’s easy to schedule an appointment with a coach by visiting the RedBrick Health website at http://wellbeing.redbrickhealth.com.

1. Y ou can get your screening at your doctor’s office. You’ll need to complete a portion of the Physician Fax Form, and your doctor will complete the rest after he/she performs the screening, and will then send it to Quest. Need the Physician Fax Form? You’ll find it on the OneExchange benefits website in the Resources section under Health Forms and Documents. 2. G  et your screening at a Quest Patient Service Center. If so, no forms are required. RedBrick Track

Engage in Your Health

RedBrick Track is a daily wellbeing tracker that spans a spectrum of daily health habits: getting active, eating healthy and living well, and it takes just a minute or two per day. You can track your activity and your rewards via web, tablet or mobile phone. You can even sync up your tracking activity with a device like a Fitbit® or Jawbone®. You can earn $2/day for tracking your healthy habits progress. This really adds up over time!

Knowing you numbers – like blood pressure, body mass index (BMI) and blood glucose – is an important part of taking accountability for your health. Taking that one step further by maintaining healthy ranges of your numbers can keep you in tip top shape. Plus it can earn you even more wellbeing dollars. If your biometric screening verifies healthy ranges in those three categories, you can earn wellbeing dollars for each! Care Management Sometimes, you may need more support if you’re managing an ongoing health condition, or looking to ensure a healthy pregnancy. Wellbeing dollars are available here too, but for this program only, you’ll work with your medical carrier directly rather than RedBrick Health. Your carrier will assign a nurse to you, to answer questions, discuss treatment plans and help you navigate carrier resources while you receive care. After you complete the program, you can report your completion on the RedBrick Health website so that your wellbeing dollars can be credited to your WageWorks HRA or HSA. On the website you’ll find helpful tools like the provider search tool, an overview of your benefits and carrier services, the preferred drug list (or formulary) and other tools and resources (like cost transparency tools and telemedicine options).
