Please fill out the registration form neatly. Please make sure that your club name is on the top of each form. Each participant must have an entry for name, rank, age, sex, desired events and fee.

CHECK-IN & RELEASE FORMS:

Upon arrival at the tournament, all contestants must check in at the registration table. No contestant will be registered without signing a release form.

Requests for refunds must be made prior to the day of the tournament.

No contact is permitted when sparring at the JKA Shotokan Karate-Do International Championships and U.S. Goodwill Tournament to be held on October 22, 2016, at APEX Central Gym, Lehman College, 250 Bedford Park Blvd, Bronx, New York 10023.

There will be no contact above the shoulders or to the groin.

·

In the case of any contact sparring, protective gear is required for all who spar, to include headgear, mouthpiece, boots and groin or chest protector. Only light contact to the headgear is permitted.

·

Great care must be exercised by all participants in safeguarding against injury.

·

You must abide by the above sparring rules when participating in the JKA Shotokan Karate-Do International Championships and U.S. Goodwill Tournament to be held on October 22, 2016, at APEX Center Gym, Lehman College, 250 Bedford Park Blvd, Bronx, New York 10023.
