How Do I View My Medical Record? From March 2016, you are able to access coded aspects of your medical record. This is a really useful facility enabling you, through your Patient Services account, to: 

View diagnoses, advice and treatment recorded by your clinician



Check any test results



Check your immunisation history



Check whether a referral has been made



Spot any errors or mistakes and tell your GP or practice



Access key medical information if something should happen to you away from home or when the surgery is closed

In order for you to view your coded medical record, three levels of consent must be met: 

Practice Level - Your GP practice must have Detailed Coded Records (DCR) enabled



Patient Level - You must request access to your detail coded record from your GP practice



GP Level - Your GP has to review your request and enable you access

Please contact your GP practice with regards to consent recording. Once all three levels of consent have been met, My Medical Record is available from the Record Access menu:

The first time you click on My Medical Record, you are asked to set up security details, see Setting up My Medical Record Security (page 3). Once your security details have been set up, to view your detailed coded medical records: 1. Log in to the Patient Services website in the usual way. 2. Click on Record Access and select My Medical Record from the drop down menu. 3. The Enter Security PIN screen is displayed, enter the 4 digit PIN you have set up and click on Submit How Do I View My Medical Record

4. My Medical Record is displayed, click on the available categories to view your records. For details of the records you can view see My Medical Record (page 2).

My Medical Record My Medical Record can consist of any or all of the following sections, depending on your GP's consent decisions:

My Medical Record



Medication - A list of your prescriptions by prescription type



Diagnosis - A list of your clinical diagnosis



Event History - A list of your encounters, referrals, admissions and recalls



Examination Findings - A list of your examinations



Problems - A section defined by your GP surgery, if this option is enabled please check with your surgery for details of this category



Risks and Warnings - A list of your allergies, adverse reactions and contraindications



Procedures - A list of your operations and immunisations



Investigations - A list of tests and results

The following options are available from each section:   2

Export as CSV file - Click to export the selected list to an Excel spreadsheet. Print - Click to print the selected list. Vision 3

Setting up My Medical Record Security Once all consents have been recorded, you must set up an additional level of security in order to view your detailed coded medical records. 1. Log in to the Patient Services website in the usual way. 2. Click on Record Access and select My Medical Record from the drop down menu.

3. The first time you do this the following Additional Security Details screen is displayed and must be completed. This enables you to set a security PIN which you need every time you wish to view your detailed coded medical records, the additional security questions enable you to reset the PIN should you forget it.

4. Complete as follows: 

Security PIN – Enter a 4 digit number that you will remember



Security Question 1, 2 and 3 – Click on the arrow and select a question from the list: - What was your childhood nickname? - What was your mother’s maiden name? - What was the name of the first school you attended? - What was your favourite place to visit as a child? - What was the make of your first car? - What was the name of your first teacher? - What was the name of the street you grew up on?



Answer – Enter the answer to the question you have selected

5. Click on Submit

to save and submit your security settings.

6. The Security Details Updated screen is displayed, click Patient Record to continue to your detailed coded medical records.

What if I Have Forgotten My Security PIN? If you forget the Security Pin you set up to access your online Medical Record, you can reset it: 1. Log into www.patient-services.co.uk in the usual way. 2. Click on My Account

and select Security Pin.

3. Complete as required: 

New Security PIN - Enter the four digit security PIN number you want to use



Confirm Security PIN - Enter the new four digit PIN number again to confirm



Random Security Question - Enter the answer you set when initially setting up the Medical Records security options

4. Click Update to save and close.
