In 2013, People Inc. Finger Lakes completed one year of service: • Provided services to several unique settings, including the only type of home in the area that supports individuals with Prader-Willi syndrome, a complex genetic condition. • Operated the largest In-Home Respite program in the area. • For the first time, People Inc. Finger Lakes was awarded Family Support Services grants, which introduced a range of new services to the region, including behavioral supports and evidence-based practices.

Dear Supporters, As the leading and largest health and human services agency headquartered in Western New York, People Inc. served 12,000 people through various programs and services for individuals with developmental disabilities and seniors. Our 3,000 staff members provided caring and compassionate services north to Niagara County, east to the Rochester and Finger Lakes region and south to Chautauqua county. Through innovative programs and with the support of our funders, People Inc. was able to serve more individuals, implement new programs and expand our services. For more than 40 years, People Inc. has helped individuals in need be more independent and lead productive lives. Over the past year, People Inc. and other area non-profits serving individuals with developmental disabilities monitored state budget cuts and financial challenges that affect our agencies. During this time of budget constraints, human service agencies have looked to pull together their resources for operations to run more efficiently and to collaborate where possible.

An example of this is Agapé Parents’ Fellowship, Inc. becoming an affiliate of People Inc. Agapé (often translated as “unconditional love”) is an interdenominational organization, fostering JudeoChristian principles, dedicated to parents and families of loved ones with disabilities. The main goals during the transition were for Agapé to maintain their identity and their faithbased mission. Similar to People Inc., Agapé also began with a group of concerned parents. The agency serves about 120 individuals through day and community rehabilitation, service coordination, respite and educational advocacy. Agapé Parents’ Fellowship, Inc. is not alone in this transition. In late 2012, People Inc. Finger Lakes was formed serving individuals with developmental disabilities in the Rochester region. The former DePaul Developmental Services merged with People Inc. to become People Inc. Finger Lakes in order to provide quality services to individuals with developmental disabilities, families and the community beyond the Western New York area. While the principals and core values were the same, the transition over the past year has been fulfilling for the Rochester community as the services became part of the People Inc. brand. To ensure that we continue to lead the way in improving quality of life for people with disabilities and older adults, People Inc. pursued and received Person Centered Excellence Accreditation from the Council on Quality and Leadership (CQL). CQL Accreditation is grounded in over 40 years of CQL leadership and peer-reviewed research. Their rigorous process included a review of our systems and practices and their effectiveness, person by person, to facilitate excellence in supports that lead to increased quality of life. We are incredibly committed to continuous improvement and look forward to working with the CQL team.

2013 HIGHLIGHTS Through a variety of services, including residential, vocational and supported employment, health care, senior services and community outreach, People Inc. has helped individuals with developmental disabilities and their families, as well as seniors, live more healthy, independent and productive lives.

Day Services Programs Achieving independence and new skills People Inc.’s diverse day programs enable individuals with developmental disabilities to learn new skills and gain confidence in order to pursue personal goals and reach their highest level of independence. In 2013, individuals who participated in People Inc. Day Services Programs were provided opportunities to enhance their skills, as well as explore new experiences. More than 900 individuals participated in more than 25 People Inc. Day Services programs located throughout Erie, Chautauqua and Niagara counties. Some accomplishments included:

Kenmore/Tonawanda cheerleading squad, delivering mail at Women & Children’s Hospital of Buffalo and sending cards to veterans.

Based on each person’s interest, People Inc. Day Services programs developed and cultivated more than 100 volunteer opportunities for individuals with developmental disabilities to participate in the community. Examples included gift wrapping at Barnes & Noble, washing fire trucks at a local station, hanging clothing for the City Mission Store, making bows and hair ties for the

Young Adult Life Transitions (YALT), a community-based program offering young adults with developmental disabilities an opportunity to experience life on a college campus while developing functional skills, had 19 students graduate from various local college programs. Many students developed the skills to transition to employment and volunteer opportunities.

The Growing Richer, Engaging Everyone’s Needs and Senses (G.R.E.E.N.S.) community opportunity grew to be a success due to collaboration and team work. With the assistance of the farming community near Marilla, an experience was developed for several people who expressed an interest in learning about agriculture.

Through Vocational Services and Supported Employment, People Inc. has helped individuals train, prepare for, secure and maintain employment.

Employment Services were engaged in nearly every kind of industry ranging from manufacturing, health care, retail, hospitality and food service.

Unique Programs People Inc.’s unique initiatives are leading the way People Inc. exists to ensure that individuals with developmental disabilities reach their highest potential. This year, several new People Inc. initiatives supported this vision, including:

Innovative and effective support

As part of a pilot study to prepare for potential changes in how services would be delivered in New York State, People Inc. selected 30 individuals receiving services to participate in an Office for People with Developmental Disabilities (OPWDD) Case Study. Using specialized assessment tools that OPWDD would like to use in the future, the project helped People Inc. establish innovative and effective supports using existing resources.

Person-centered goal attainment

A new dynamic and innovative service, Life Quality Coaching (LQC), helps individuals with developmental disabilities bridge the gap between where they are to where they want to be. Individuals work one-on-one with a life quality coach to develop goals and learn new skills. Coaches focus on enhancing independence rather than interdependence by assisting with problem solving, decision making and goal attainment. This entirely person-centered service has helped with transitioning, future planning, social skills, employment, volunteering, health and wellness and crisis intervention.

Individuals in the Case Study made impressive strides towards achieving goals, including successfully moving from certified residences to their own apartments, securing employment, realizing lifetime dreams and increasing wellness activities.

Health Services Quality healthcare for the whole family People Inc. offered a comprehensive range of health care services for people with disabilities, as well as children, seniors and community members. The Elmwood Health Center, a health care facility that provides a wide range of medical, rehabilitative therapies and counseling services for the general public, as well as specially designed services for individuals with developmental disabilities, had a year of growth and achievement in 2013. The center earned an Allscripts National Outcomes Excellence Award for its advanced use of electronic health information in patient care delivery, and in partnership with HEALTHeLINK, pioneered the online reporting of childhood immunizations to the New York State Department of Health. A valued resource in the community, patient visits climbed to a record 37,319. The People Inc. Certified Home Health Agency made 5,154 visits, providing physical, occupational and speech therapies; social work; nutrition and nursing to 127 individuals of all ages residing in private homes and residential settings throughout Erie and Niagara counties. Skilled Services included physical, occupational and speech therapies, social work, nutrition and nursing.

Ninety percent of individuals receiving services were developmentally disabled. In-home nursing visits were provided through the People Inc. Licensed Home Health Agency with 21,311 hours of personal care and private duty nursing. Services were provided to individuals of all ages living in private homes in Erie county. Enabling people to have more active roles in the selection and maintenance of their home care services, the Consumer Directed Personal Assistance Program provided 159,463 hours of support and training to 156 individuals residing in Erie and Niagara counties, who hired, trained and supervised their own attendants. Participants received services to help them to remain as independent as possible. People Inc. offers Early Intervention Services for children who are developmentally delayed up to 3 years of age through the Erie County Department of Children’s Services. In 2013, the program provided services to 158 children in Erie county. Evaluations by special educators; physical, occupational and speech therapists and individual therapy were provided in the child’s home or day care setting. A total of 627 visits and evaluations were provided.

Respite Services Enhancing quality of life Through People Inc.’s variety of respite services, relief is provided to caregivers, along with temporary care away from home. For many families, respite is crucial to caring for a loved one with a developmental disability. People Inc. offered a variety of respite services, serving ages 4 and up, to suit personal needs. 5

In 2013, our Free Standing Respite programs, Downing Respite House in Bowmansville and at the Northtown Respite House in Wheatfield, provided a combined total of 13,499 hours of care. The programs, starting for individuals at age 10, have made a conscious effort to increase their community outings. Some of the

outings included Comedy Sportz, Mayer Brothers Cider Mill and pumpkin picking. The Northtowns Respite House held a family picnic in the summer to honor the generous and entertaining gift of a new playground contributed by the Nurse family who benefits from respite services.

Senior Services Giving seniors the freedom to live life to the fullest People Inc. values the growing Western New York senior population. The agency expanded its senior services over the past year to give seniors practical solutions to meet the challenges they may face.

People Inc. Senior Outreach provided case management services to seniors 60 years or older who live at home. The program served 786 seniors in Kenmore, Grand Island, North Buffalo and Tonawanda.

In 2013, People Inc. Senior Living celebrated 20 years of providing high quality, affordable and safe housing to seniors. Additional housing continued as People Inc. Orchard Senior Living opened in Springville, which marked the 18th apartment building. A total of 797 tenants were served in the apartments throughout Western New York. Unique to People Inc. Senior Living, the service coordination team provided connections to services as needed to 420 tenants at no extra charge.

The People Inc. Senior Companion Program became part of the agency in the Fall of 2013 through an acquisition from Child and Family Services. The program’s goal is to provide aid and independence to seniors and other adults who are frail or homebound. Volunteers, known as Companions, assisted participating seniors in activities such as: accompanying a shopping trip or doctor’s appointment, light cooking, housekeeping, reading and writing letters.

Headway of WNY Support for people with traumatic brain injuries Headway of Western New York, a support and advocacy agency for people with brain injuries, disabilities and seniors, is an affiliate of People Inc. In 2013, Headway: • Provided support to 1,450 people through various support groups. • Redesigned www.headwayofwny.org. • Offered a series of trainings for professionals entitled “Improving Treatment Strategies for Individuals with Brain Injuries and Behavioral Issues.” www.facebook.com/headwayofwny

Museum of disABILITY History It’s amazing what’s inside The Museum of disABILITY History, a project of People Inc., is dedicated to the collection, preservation and display of artifacts relating to the history of people with disabilities. Its mission is to tell the story of the lives, triumphs and struggles of people with disabilities as well as society’s reactions. The Museum of disABILITY History offers educational exhibits, programs and activities that expand community awareness, along with a platform for dialogue and discovery. In 2013, the Museum of disABILITY History: • Welcomed 2,500 visitors; also resulted in an increase of sales for the Museum Store. • Increased the number of books produced through People Ink Press to 13, including three Ivan the Invacar children’s books, along with an activity book, seven

books in the Abandoned History Series and two new reprints of historical publications. Established by the Museum of disABILITY History and People Inc., the People Ink Press is dedicated to publishing books related to disability history. • Hosted 12 creative and innovative events to attract new audiences, including Kids Day, disABILITIES Film Festival and two new events called Dialogues on disABILITY Speaker Series and the Person First Forum series.

Grateful for National and Local Funding Microsoft Awards $1 Million to People Inc.

People Inc. is proud to have a variety of generous supporters that assisted our agency throughout the year. Grant funds totaled $3,621,495 and helped thousands of individuals and various programs ranging from health care, employment services for individuals with developmental disabilities, services to help children improve social skills, expanding education programs offered by the Museum of disABILITY History and safety measures for People Inc. Senior Living apartments, as well as assisting veterans who have traumatic brain injuries. In addition to funds received for programs and services, People Inc. was honored to have been selected by Microsoft as a key non-profit partner. In November 2013, Microsoft awarded People Inc. $1 million in Microsoft software to upgrade its operating systems and provide special programs to sites. Additionally, as part of the partnership, People Inc. was also awarded $10,000 in hardware, used to purchase Microsoft Tablets for staff working in individuals’ homes and community settings. The mobile devices will help reduce travel time to offices and allow more immediate access to decisionmaking information while in the field. The awards were announced during the grand opening celebration of the new Microsoft Store at Walden Galleria Mall.

2013 GRANT AWARDS People Inc. and all of its affiliate programs are grateful to its national and community partners who provided grant funding to support programs and services in 2013, including: Adult Career and Continuing Education Services-Vocational Rehabilitation (ACCES-VR) of the NYS Education Department (NYSED) Awarded up to $762,040 People Inc. was awarded a five-year contract to help individuals with special needs enter competitive employment. Programs made possible by this grant included entry services, assessment services, work readiness, job placement services, youth employment services and paid competitive employment. Christopher and Dana Reeve Foundation Awarded $13,980 Our Elmwood Heath Center used funds awarded by the Christopher and Dana Reeve Foundation to purchase a pressure mapping system for its wheelchair clinic, which enhanced the physical therapy of patients with mobility impairments. Corporation for National Senior Companions Program Awarded $383,294 Funds supported the People Inc. Senior Companion Program which recruited volunteers age 55 and over to provide assistance and friendship to older adults who have difficulty with daily living tasks, such as shopping or paying bills. The program helped seniors remain independent in their own homes and partnered with 10 non-profits to coordinate more than 60 volunteers. Erie County Cultural Funding Awarded $3,000 Through the operational support received, the Museum of disABILITY

Mary Frawley Grant Awarded $500 Funds supported a horticulture project that allowed individuals from our Orchard Park Day Services program to grow vegetables and learn related gardening and nutrition skills. Microsoft Awarded $1,010,000 Microsoft software funds were awarded to upgrade People Inc.’s operating systems and provide special programs to sites. Additional funds for Microsoft hardware were used to purchase Microsoft Tablets for staff working in individuals’ homes and community settings. New York Council for the Humanities Awarded $1,500 Funds enhanced the Annual disABILITIES Film Festival and Speaker

Funds awarded for the Museum of disABILITY History supported the 20132014 disABILITIES Film Festival and Speaker Series, which explored issues faced by individuals with disabilities. New York State Department of Health Awarded $759,000 Our Elmwood Health Center received a two-year award from the New York State Department of Health. The funding will give more people access to high quality care at the Elmwood Health Center. NYS Office for People with Developmental Disabilities (OPWDD) Family Support Services (FSS) Awarded $57,683 The grant allowed for the innovative Life Quality Coaching program, that assists individuals in achieving life goals. NYS OPWDD Awarded $293,291 People Inc. was awarded funds to develop new supports for individuals with developmental disabilities who are seeking to live in an independent apartment with planned weekly supports. This award is part of the state’s new Medicaid Redesign initiative.

2013 GRANT AWARDS NYS OPWDD Awarded $39,519 This grant allowed for the expansion of our After School Respite program in Rochester. NYS OPWDD Finger Lakes FSS Awarded $15,548 Funds allowed People Inc. Finger Lakes to offer a Social Skill Enhancement program consisting of three, eight-week group sessions for young adults aged 16-24 and their parents residing in Monroe County. The program provided parent group training and support sessions while concurrent young adult sessions focused on improving peer interactions, cultivating friendships and communicating wants and needs in a more socially acceptable manner. NYS OPWDD Finger Lakes FSS Awarded $10,678 Thanks to funding, People Inc. Finger Lakes hosted a 12-week session of Incredible Years Parent Training for families living in Monroe County who have a child aged 3-12 years with developmental disabilities who exhibits disruptive behaviors. The families acquired strategies to address and better manage behavioral problems in their homes. NYS OPWDD Finger Lakes FSS Awarded $78,736 People Inc. Finger Lakes established a hands-on, in-home behavioral support program that worked with Monroe County families of individuals with developmental disabilities who have cooccurring intense behavioral issues.

The funds supported a master’s degree level behavioral specialist and behavior technician to create a modification plan to eliminate or reduce the incidence of problem behaviors and support the person and their family to stabilize the situation, manage complex behavioral issues and recognize and diffuse trigger situations before they may escalate. Peter and Elizabeth C. Tower Foundation Awarded $9,416 Our Elmwood Health Center received funds to purchase resource materials for group and individual therapy sessions, as well as for professional development for staff who provided mental health counseling and support. Peter and Elizabeth C. Tower Foundation Awarded $40,000 The Peter and Elizabeth C. Tower Foundation awarded a planning grant to examine how the agency incorporates information technology toward creating efficiencies in document storage and retention. Peter and Elizabeth C. Tower Foundation Awarded $60,000

Service Collaborative of Western New York Awarded $17,810 Funding allowed Headway of WNY to embark on a partnership with the Service Collaborative of WNY, which helped veterans with brain injuries find services. The Service Collaborative is the largest coordinator of AmeriCorps volunteers in New York state. Headway of WNY will receive assistance from an AmeriCorps volunteer for up to five years. Target Arts, Culture and Design in Schools Awarded $2,000 Hundreds of children received performances and resource materials through the Kids on the Block and kNOw bullying! educational programs to celebrate differences. Variety – The Children’s Charity of Buffalo and Western New York Awarded $5,000 Our Elmwood Health Center was awarded funds to purchase a recording device for a vaccine refrigerator and an audiometer for hearing tests, part of the commitment to provide preventive care in one location for children.

Our Elmwood Health Center received funds toward the “Improved Mental Health = Better Health” project. Funds were used to create a standardized care plan and for training opportunities for primary care and therapeutic staff.
