Welcome to Envy Hair, Nail and Tanning Studio! We truly appreciate you considering us to serve you on your special day! We have a talented, caring and creative staff that wants nothing more than to help make your wedding day as stress free and carefree as possible! We offer an array of services to help you to prepare for your special day; up to and including the day of. Please feel free to check out our services and our wedding policy. This information and agreement will be gone over carefully should you choose to set up a consultation with our salon owner, (Melodie Favinger). Upon completion of the consultation, should you choose to sign the agreement, a deposit will be required to secure your event date and services. You will be provided a copy of this agreement, and you will also be provided copies of scheduled appointments and service itinerary for everyone in your bridal party, leading up to and including the day of the event. *Please read this agreement carefully; this agreement is a contract and acts as a reservation for services provided by Envy Hair, Nail and Tanning Studio staff, to include salon owner, salon stylists, make-up artists, massage therapists, and estheticians. Contract Agreement

I, _________________ (Purchaser/Bride), agree to the above scheduled services to be performed by Envy Hair, Nail & Tanning Salon staff; to include salon owner, stylists, makeup artists, massage therapists & estheticians. I agree to provide a 50% deposit to reserve the services on the day of my event based upon the quoted services price provided by Melodie Favinger, Owner of Envy Hair, Nail and Tanning Salon. _______ Purchaser’s initials I understand fully that I am signing a contract for services scheduled; up to and including any scheduled service in preparation for my wedding / event. Therefore, any cancellation in services for my personal services or the services of my bridal party will be held to the Envy Hair, Nail and Tanning Salon Cancellation policy in the paragraphs to follow. _______ Purchaser’s initials I understand fully that if I cancel my wedding / event 10 days prior to the date of the event, I am entitled to receive 30% of my deposit in refund. If I do not cancel 10 days prior, or I or my bridal parties do not show up on the scheduled event day, I will not receive any percentage refund of my deposit and I will be charged in full for the scheduled services. _______Purchaser’s Initials I understand and agree that if I, or my bridal party, cancel or make changes to services altering the total amount due for services, I / my bridal party am / is still responsible to pay for those services. (i.e. Bonnie Bridesmaid wants a pedicure and facial scheduled 2 weeks prior to the event. The day Bonnie is due in for services, she decides she doesn’t want a facial. Bonnie is still responsible to pay for that service unless that service is replaced with an equal costing service, or the difference in service cost is paid). (i.e. Mary Maid of Honor decides instead of her scheduled pedicure, she would like a spray tan instead. Because the spray tan costs more than the pedicure, she is responsible to pay for the difference in service cost). ________ Purchaser’s Initials I agree that the day of the event, no last minute changes can be made to the schedule. (i.e. Franny the Flower Girl was not scheduled for makeup, but since she is so small, can we do it anyway?). Hair Trim Hair & Makeup Trial-Bride comes in for the hair & makeup. Wear a white button down shirt, headpiece if any, photos of makeup or hairstyles that you may like to show stylist & makeup artist, and bring a camera to take photos, before & after.
