FESS course development CLINICAL LEAD Mr Paul White, FRCS(Ed) FRACS Consultant ENT Surgeon and Rhinologist At Cuschieri Skills we offer a new and innovative approach to training in endoscopic sinus surgery and anterior skull base surgery. Our core programme offers a progressive step wise approach to the acquisition of skills using the Thiel fixation human cadaver simulation platform. This is based on 22 years of experience in FESS training which has led to the evolution of a curriculum of 5 different surgery skills modules, FESS 1 to FESS 5. These modules are best taken in sequence and any may be repeated as often as a trainee/delegate may wish. Our emphasis is to allow trainees/ surgeons to practise these demanding skills in a structured manner, and in a safe environment devoid of risk to patients. Our educational content is linked where it can be to the Intercollegiate Surgical Curriculum Project and delegates registered with ISCP will be able to have some exercises registered as PBA’s (simulation) if they wish. Each module builds on skills attained during the previous session with FESS 1 and 2 providing a foundation in the practical competencies required by ENT surgeons to attain proficiency in endoscopic sinus surgery and includes modules on dealing with surgical complications. FESS 3 to FESS 5 comprise optional more advanced modules in anterior skull base and frontal recess surgery (FESS 3), orbital and lacrimal surgery for ENT surgeons and ophthalmologists (FESS 4), and endoscopic skull base and neurosurgery (FESS 5).

FESS 1

THIEL CADAVERIC ENDOSCOPIC SINUS SURGERY This course of individual work station based surgical dissection using cadaver specimens aims to provide trainees with a chance to become familiar with nasal endoscope and instrument handling. Understanding of the detailed anatomy of the lateral wall of the nose and the anterior paranasal sinuses will be gained by practical surgical dissection exercises. EDUCATIONAL AIMS Gain an understanding and experience in: • Uncinectomy • Bulla resection • Defining the ground lamella • Anterior ethmoidectomy • Middle meatal antrostomy • Sphenopalatine artery ligation

THIEL CADAVERIC ADVANCED AND OPEN SINUS SURGERY This course of dissection aims to build on core skills and provide training in some of the more challenging aspects of endoscopic sinus surgery, including those necessary for emergency safe practice and those necessary for dealing with complications of endoscopic ethmoidectomy. We aim to provide trainees with a chance to develop competency at managing rhinological emergencies and the acute complications of FESS such as orbital haematoma and csf leak. EDUCATIONAL AIMS Gain an understanding and experience in: • Posterior ethmoid and sphenoid anatomy • Surgical approaches for dealing with epistaxis • Intra-operative management of FESS complications • Emergency orbital decompression surgery • Frontal recess anatomy • Repair of anterior skull base CSF leak

THIEL CADAVERIC ADVANCED FRONTAL SINUS SURGERY Advanced Frontal Sinus Surgery Course is a one day intensive comprehensive handson cadaver dissection course covering all aspects of frontal sinus surgery. Dissection includes: Frontal Recess Surgery (DRAF I), Advanced Frontal Sinus Surgery (DRAF III), Frontal Balloon Sinuplasty, Frontal Osteoplastic Flap, Open Sinus surgery and Frontal Trephination. The course features hands-on dissection on Thiel soft-fix cadavers in a well-equipped modern surgical skills centre. Powered instrumentation and the latest balloon technology will be made available on this course. The course aims to provide high quality training to Consultants, Fellows and Senior Trainees with a special interest and practice in Frontal Sinus Surgery.

THIEL CADAVERIC ENDOSCOPIC AND OPEN ORBITAL AND LACRIMAL SURGERY Dissection Orbital/Oculoplastic Surgery Course is a one day intensive comprehensive hands-on cadaver dissection course covering all aspects of Orbital surgery. Dissection includes: Endoscopic DCR, External DCR, Endoscopic Orbital decompression, External Orbital decompression, Endoscopic Optic nerve decompression, External Ethmoidal artery ligation and Canthoplasty.

THIEL CADAVERIC TRANSNASAL ENDOSCOPIC SKULL BASE SURGERY This comprehensive one day course is a presentation of minimally invasive techniques for endoscopic endonasal surgery of the pituitary fossa and cranial base. Experts on the subject will present the anatomical and technical aspects of this procedure along with risks, benefits and outcomes. The course features hands-on lab dissection on Thiel soft-fix cadavers, including state of the art equipment with image guidance in a well-equipped, modern surgical skills centre. Participants will have the opportunity to enhance their knowledge regarding endoscopic surgery of the cranial base and pituitary fossa using minimally invasive techniques.
