BENEFIT PAID TIME OFF (PTO)

BENEFIT DESCRIPTION PTO is an employee benefit program that allows employees to have more flexibility in scheduling time off for personal need. The PTO bank includes hours typically found in vacation, sick pay, personal and national holiday banks. Employees are encouraged to accumulate enough PTO hours to provide adequate coverage for those events, as well as short-term personal illness or injury. For longer-term illness or injury, PTO interacts with the STD program. Contact the Human Resources Department for your specific PTO accumulation rate.

PTO CASH OUT

Employees are always encouraged to use their PTO time throughout the year, but we are aware that there are occasions when not all available PTO can be used. Employees have the opportunity to sell back up to 40 hours of PTO on their birthday and up to 40 hours of PTO on the anniversary of employment date. Enrollment for this benefit is in the fall of every year.

SHORT TERM DISABILITY (STD)

STD is a program that allows employees to build a bank of hours to bridge the period between short-term illness or injury and the Long Term Disability Insurance benefit. The STD bank is coordinated with PTO. Full-time employees must use 40 consecutive hours of PTO (elimination period) before using STD. Part-time employees must use 24 consecutive hours of PTO (elimination period) before using STD.

BEREAVEMENT LEAVE

Up to three days (24 hours) off work with pay to attend funeral of immediate family member, defined as mother, father, brother, sister, spouse, child, father-in-law, mother-in-law, daughter-in-law, son-in-law, grandparent, grandchild, stepparent, stepchild, and legal guardian.

BENEFIT DESCRIPTION An extended period of time off may be requested for medical, personal, military or education. Applications are available in Human Resources. LOA status has considerable impact on participation in benefit programs. Employees should be sure to check with HR. Eligible employees may apply for a leave under the Family Medical Leave Act.

Choice of three medical plan options; all include prescription drug coverage. Four-tier employee premiums. Guaranteed enrollment if application received within eligibility deadlines. A 12-month waiting period may apply to pre-existing conditions with the PPO option. The prescription drug plan provides a benefit for employees and eligible dependents through the Blue Cross Blue Shield of KC pharmacy network. This benefit utilizes a co-pay structure. Mail order services are also available. Dependent children may be covered until the end of the calendar year of their 26th birthday.

Choice of two dental plans, a PDP (Preferred Dental Provider Co-Pay Plan) with the choice of either a High Plan or a Low Plan, with the difference in coverage and out of pocket expenses. Four-tier cost sharing with full and part-time premiums. Dependent children may be covered until the earlier of the end of the calendar year they reach age 25 regardless of student status.

The special 30-day eligibility rule applies here as well.

EYE MED

Employees may apply to purchase this vision benefit coverage through EyeMed Vision Care. The EyeMed Vision Care Network consists of private practicing optometrists, ophthalmologists, opticians, and optical retailers. The plan pays both in network and out of network, with greater benefits in network. Four-tier employee premiums.

The special 30-day eligibility rule applies here as well.

BENEFIT DESCRIPTION Premiums for health and dental insurance may be paid on a pre-tax basis. Pre-tax dollars withheld from paychecks may be set-aside in Medical Reimbursement and/or Dependent Care Reimbursement accounts. Elections are made on a calendar year basis and enrollment in the Reimbursement accounts must be made ever year. Dollars not used by the end of each calendar year are forfeited.

Term life insurance is employer-paid. Full-time coverage is equal to the employee’s base annual salary rounded to nearest $1,000 up to $50,000 and up to $25,000 for part-time employees. An equal amount of accidental death and dismemberment insurance is also provided.

Employees may apply to purchase term supplemental life insurance amounts in increments of $10,000 by paying the full monthly premium through payroll deduction. If at first eligibility the employee passes a short health screen, issue is guaranteed up to $130,000, without additional proof of good health. Employees who do not apply when first eligible or wish to increase coverage must provide proof of good health.

If you enroll for Supplemental Life coverage for yourself, you may also elect coverage on the lives of your spouse and dependent children. The following are guaranteed issue (unless a late entrant): Option 1: $30,000 Spouse/$10,000 Child Option 2: $20,000 Spouse/$5,000 Child Option 3: $10,000 Spouse/$2,000 Child Option 4: $5,000 (Children only)

Employee may apply to purchase supplemental Critical Illness Insurance coverage through MetLife. Policy provides a lump sum payment if you are diagnosed with one of the six covered conditions for the first time ever in your lifetime. You may apply for up to $50,000 in coverage for you and your family. A medical questionnaire must be completed and the coverage is subject to acceptance by MetLife.

Employer-paid long-term disability insurance is equal to 50% of regular earnings (to a maximum of $5,000 per month), with a 90-day elimination period. The waiting period for coverage of a disability resulting from a pre-existing condition is 6 months from the coverage effective date or after 3 months in which the employee is treatment-free, whichever is less.

BENEFIT DESCRIPTION Life insurance valued at $100,000 provided while traveling on approved business. Coverage reduces beginning at age 70 and at five-year intervals thereafter. Coverage is fully paid by the corporation.

RETIREMENT SAVINGS PLAN

All employees are automatically enrolled upon employment at 3% contribution level.

Services at no cost to the employee include: Required post offer physical exam, including TB skin test, Rubella screen and drug screen; annual flu vaccination, and examination and some treatment for work-related illness or injury while on duty.

Short term, strictly confidential counseling services for employees and their eligible dependents. Professional services provided through New Directions Behavioral Health, and is available 24 hours a day, 7 days a week.

Up to $1,500 per calendar year is available for full-time employees and up to $750 per calendar year for part-time employees for approved college credit courses related to the employee’s current job or a job to which the employee can potentially advance. Tuition reimbursement may be subject to taxation as income.

MO$T is an educational savings plan offered by the State of Missouri that allows an employee to open a savings account for higher education expenses. May deduct a defined amount yearly from state tax return. This is done on a post-tax basis through payroll deduction.

Learning Quest is an educational savings plan offered by the State of Kansas that allows an employee to open a savings account for higher education expenses. May deduct up to a certain amount yearly from state tax return. This is done on a post-tax basis through payroll deduct. Authorization forms must be received from Learning Quest before payroll deducts may start.

Employees may contribute between 1% and 50% of salary each pay period, up to the IRS maximum of $17,000 for 2012, with age 50 and up eligible to make catch-up contributions to a maximum of an additional $5,500. Contributions are made on a tax-deferred basis to any of the investment funds available and are fully vested immediately.

RETIREMENT SAVINGS PLAN MATCH

Employees age 21 or older become eligible on January 1 or July 1 after completing one year of service, defined as the 12-month period from the hire date in which 1,000 hours are credited.

The employee’s contribution to the Savings Plan is matched on a discretionary basis by the corporation at 50% up to the first 6% of pay (up to 3% employer match). Employees not regularly scheduled to work 40 hours in a bi-weekly pay period must complete 1,000 hours of service during the year to receive the match. Employer-matching contributions are fully vested after earning 3 years of service.

FRIEND-INNEED PROGRAM

Employees may donate PTO hours to the Friend-in-Need bank. Benefit eligible employees who contribute may apply for withdrawals from the bank in case of personal or family hardship. Non-contributors are eligible for a lesser amount of benefit.

Rest and meal periods will be scheduled based upon hours worked. Scheduled meal periods are 30 minutes in length and are unpaid. Employees not receiving an uninterrupted 30-minute period will be paid for the meal period. The employee must mark “no lunch” on timecard or enter into electronic time-keeping system.

The corporation matches social security contributions made by the employee. These contributions provide income at the time of disability, retirement, or the employee’s death, if Federal eligibility requirements are met.

BENEFIT DESCRIPTION

Fund developed by employees, for employees, to help employees who have no other place to turn for financial assistance in an unforeseen emergency. Assistance is designed to meet emergency needs rather than problems resulting from ongoing expenses greater than income. Emergency assistance is provided on the basis of verified need. The total amount of assistance, from the Employee Emergency Assistant Fund, can not exceed $1,500 and is approved as a loan.

RECREATIONAL ACTIVITIES

All employees are eligible upon hire.

Tickets to local sporting and recreational events are available to employees and their families throughout the year at discounted prices. AMC theatre discount passes (gold and silver) are available all year long.
