Jam Hops 2016-2017 After School Care/Day Camp Parent Handbook Jam Hops welcomes you to our programs! We are looking forward to an amazing year with your children! Please review the following policies and procedures for our After School Care and Day Camp. Please sign and return the agreement page within two weeks of your registration.

Jam Hops Mission Statement At Jam Hops we have a professional, enthusiastic team of caring people. Our staff is dedicated to the pursuit of nurturing children so they can realize their dreams. We are leaders who believe that by teaching through example, we build self-confident, productive children who will become life champions. About the After School Care/Day Camp Programs Trained Jam Hops staff will be responsible for the care of the children. Our coach to student ratio is 1:15. The children will participate in a number of activities while they are at Jam Hops:        

Open Gym in our gymnastics facility Arts/crafts/coloring Indoor games (board/group games) Outdoor activities (jump rope, games, chalk, etc.) Homework time if they choose (for After School Care) Movies Snack and drink each day Additional activities vary

What to bring/wear  

A lunch and water bottle for Day Camp Comfortable work out clothing or gymnastics apparel

Transportation After School Care Drop Off – A bus will bring the children from Davinci and drop them off at our facility for After School Care. Jam Hops staff will meet the children as they get off the bus. Day Camp Drop off - Parents/guardians will drop children off for Day Camp. Parents/guardians must sign their child in on the sign in sheet. After School Care Pick Up - Parents/guardians will be responsible for picking children up. Parents must sign their child out on the sign out sheet. We will not release a child to a non-custodial parent without the custodial parent’s consent. We will need a copy of the driver’s license on file for each person on the authorized pick-up list (see last page of this handbook). Day Camp Pick Up - Parents/guardians will be responsible for picking children up. Parents must sign their child out on the sign out sheet. After School Care Hours 3:15pm-6:00pm; children must be picked up promptly by 6:00pm. For every 15 minutes they are here longer than that your account will be charged $5. Day Camp Hours 9am-4pm; with before and after care available from 8am-9am and/or 4pm-5pm for an additional fee of $5 per hour. Children must be picked up promptly by 5:00pm. For every 15 minutes they are here longer than that your account will be charged $5. If you did not sign them up for after care and they are here past 4pm you will be charged $5. After School Care Dates for the 2016-2017 There will be After School Care available for your children to attend every day school is in session; August 9th – June 15th. There will be After School Care for the week of January 3rd – 6th (Intersession week). Day Camp Dates (for Davinci school release dates) September 6; October 17-21; November 4, 21-23; December 22-23 and 26-30; January 2, 16; February 10, 13-17; March 2-3 and 20-24; April 13-14 and 17; May 25 and 26; June 2 and 9. Closed Dates Jam Hops will be closed and will not have After School Care or Day Camp on the following dates for 20162017: September 1st, 2nd and 5th, November 24th and 25th and May 29th. Cost/Payment for After School Care $12 per session ($240 for one punch card of 20 sessions). Each family will purchase one punch card at the time of registration. When all 20 sessions have been used, $240 will be automatically billed to the account we have on file. You will receive an email confirmation when a new punch card is purchased and billed to your account. We will make every effort at the end of the school year to adjust the charge for new punch 2

cards to reflect the number of days your child has remaining for After School Care. Any punches remaining on your card at the end of the year will either be credited to your account (you may apply those funds to any program, activity or camp at Jam Hops) or be saved to use for the following school year. We will contact you if you are in this situation. Cost/Payment for Day Camp $30 (plus tax) per day for members/$35 (plus tax) per day for non-members. $5 per hour for before/after care from 8am-9am and/or 4pm-5pm. This will be paid at the time of registration. Member Fee Family member fee of $42.85 is due at time of registration. Member fee is valid September 2016 – August 2017. This fee will allow you to sign up your children for Day Camp and other events/activities at a lower cost. Registration Information 

Signed handbook – return the signature page of this handbook to confirm that you agree with our terms for the After School Care and Day Camp programs. You may scan and email to [email protected]. Permission form – complete and sign the policies page available through your Customer Login on our website. Waivers will need to be signed and updated at registration and September 1st of every year. To finalize your registration for the After School Care Program, please call or stop in and speak to our customer service representatives to register, purchase your first punch card for $240, and set up billing information for future purchases. Registration for Day Camp can be completed through your Customer Login or with one of our customer service representatives. After School Care and Day Camps are available on a first come, first serve basis. Pre-registration is required for both programs at least one day prior to the date your child will attend.

Customer Login All families will need to create a customer login. Through the customer login you will be able to:      

To create your customer login or if you have forgotten your password:        

go to our website, www.jamhops.com click on the “customer login” which is red, white and blue with our logo above the words “customer login” located in the center of the page (ignore the “login” tab at the top with the black background) click on “forgot password” or “need to get started” enter email address and then submit retrieve the temporary password sent to your email address go back to the “customer login” enter your email address and temporary password to sign in change your password under the “my account” tab

Cancelation Policy After School Care - A minimum of 5 children enrolled by September 30, 2016 is required to run this program. Day Camp - We must have at least 4 children registered for Day Camp for each day. We will contact you the day before if camp will be canceled. Scheduling Jam Hops provides individual scheduling for each student that attends After School Care. You will receive an email approximately 2 weeks before the 1st of each month asking for the dates your child(ren) will attend our After School Care. We do not require your child to attend our program for a specific number of days each week. You pay for the days that your child attends. You must let us know the dates your child will attend each month at least one week before the 1st of the following month. Attendance/Canceling a scheduled day After School Care - If your child is scheduled for After School Care and they will be absent for any reason, you must contact Jam Hops by 12:00pm that day. Day Camp – If you decide to cancel a registration it must be done at least one week prior to that date for an account credit to be given. Clothing change Children may bring clothing/gymnastics attire to change into when they arrive for After School Care if they prefer not to wear their school uniform in the gym. It is best if the children have something comfortable for them to do gym activities in. Children must be barefoot for safety purposes. Snacks All snack items will be commercially processed. No home prepared food will be distributed to children for their safety. Let us know on the “My Students” tab of your customer login if your child has any food allergies and we will accommodate their snack options. Feel free to send a snack of their own to have while they are here if they have special needs or allergies. Purchases made by children from the Jam Hops vending machine will not be allowed during After School Care or Day Camp. Sick Children/Injuries If a child becomes sick while in our care we will notify the parents/guardians. It is the parent’s responsibility to provide transportation immediately if their child becomes sick while at Jam Hops. If a child receives a serious injury, the parent or alternate person listed on the permission form will be contacted. In the event the gym cannot reach someone, medical personnel will be called and decisions will be made accordingly. Please follow these guidelines when determining if you should not bring your child to Jam Hops for After School Care or Day Camp:    

Temperature that is higher than 100 degrees Vomiting or severe nausea Eyes are pink or crusty An undiagnosed rash

Child needs to be symptom free or on antibiotics for at least 24 hours before sending them to Jam Hops. Ice packs and band-aids are available in our customer service office. Ice packs are administered by a coach if needed. If an ice pack is administered, an accident report will be filled out and placed in the student’s file. Inclement Weather In the event that Jam Hops is closed due to weather, we will post it by 3:00pm on our website at www.jamhops.com. If the RED bus will be bringing your children to us when we are closed, a staff member will be present until each child has been picked up. We appreciate on these days that you would pick up your child as soon as possible so our staff will be able to get home before the weather becomes more dangerous. 5

Medication Jam Hops will not be responsible for distributing medication to children. We will keep an asthma inhaler at Jam Hops for your child if necessary. Discipline – Child Expectation At Jam Hops, the focus in our programs is on self-discipline and learning to take responsibility for one’s actions. A child’s right to participate and a staff member’s right to provide equal care to everyone should not be denied because of disruptive behavior. Expecting and encouraging appropriate child behavior is the responsibility of all employees who have direct contact with children attending Jam Hops. Children will be re-directed into appropriate activities whenever possible. In the event a child’s behavior becomes dangerous to themselves or others, or so out of control that the staff member is not able to diffuse the situation, they will be removed from the situation and the parents will be contacted. The parent will be expected to pick up the child as soon as possible. At that time a discipline management program may be put in place for the benefit of the child and others. Lost and Found The lost and found for clothing and other items is located in our cubby room. You can help your child retrieve lost clothing more easily by labeling items with his/her name. Our lost and found is donated at the end of every month, so please check this area often. Water bottles left at Jam Hops will be thrown in the garbage at the end of each day. Personal Property of Students Jam Hops is not responsible for valuables which children bring to school. It is recommended that children leave valuables, toys, etc. at home. If special circumstances make it necessary for a student to bring money or other possessions to Jam Hops, please have your child let his/her coach know and they can put it in a safe place. I understand that this is a commitment for the 2016-2017 school year, that I will have a credit card, debit card or e-check set up for future payments, and that I will automatically be billed for a new After School Care punch card to the account on file when all twenty sessions have been used – or be automatically billed for Day Camp that is done through online registration.
