On behalf of our staff at West Cobb Funeral Home and Crematory, we want to express our gratitude for allowing our family to serve your family during this very difficult and emotional time. Our promise to you is to listen to your wishes and desires, and to carry them out in a compassionate and professional manner. Enclosed in this packet you will find information to help you make good decisions based on what is best for your family. We will assist you during the arrangement conference to bring all of your needs together and celebrate the life of your loved one. We have provided you documents that will help expedite our services to you. If you wish, you can fill out the basic Vital Statistic and Biographical information privately before we meet with you. This will assist us in completing the Obituary and Death Certificate. You will also see our Consumer’s Guide to Selecting a Funeral and General Price List, along with payment options that will fit any of your financial needs. Other services include floral arrangements, monuments and bronze memorials to be placed at the site of final interment. When you come to the arrangement conference, depending on what is applicable to your funeral requests, you will need to consider bringing clothing, veteran discharge documents, insurance policies, social security card and birth certificate of the deceased, jewelry, and a picture of your loved one. We will stay in close contact with you during the entire process to help you with anything that needs to be taken care of. Again, thank you for allowing us to serve you and placing your trust in us during this time. You will find our facilities warm and accommodating, with large state rooms, plenty of off street parking, and a new Family Reception Room with a fire place, satellite television, and tables and chairs, designed for you to bring food or have it catered in to the funeral home. Payment Options Policy To Every Family That We Serve One of the most important responsibilities that we have to each family we serve is to bring your emotional and financial needs together, whatever the situation may be. As part of our consultation with your family, we will discuss the different options we offer to help you with the funeral expenses. Our goal is to insure that the services and merchandise you choose will fit into the financial situation you are in. Payment arrangements must be discussed and made during the final arrangement conference. Payment for all services, merchandise and cash advances are due in full by the day that the funeral services are rendered.

THE USE OF AN INSURANCE ASSIGNMENT IS AT MANAGEMENT’S DISCRETION. Life Insurance Policies are funded and advanced through American Funeral Financial, LLC. In most cases this can allow the funeral home and the beneficiary to receive insurance proceeds within 48 hours from completion of the claim, subject to approval. The assignment fee is 3.98% added to the total assigned amount of the funeral contract, with a $150 minimum. Note * Insurance Assignments require the following criteria:

Original Insurance Policy Verification of the Beneficiary Verification that the policy is in force Verification that the policy is not contestable Verification that the policy is assignable to the funeral home Verification that there are enough funds to cover the funeral expenses

General Price List, Effective August 20, 2014 This guide is designed to familiarize you with the services and products that normally compose different types of funeral arrangements. Enclosed you will find information on Traditional Services, Cremation, Forwarding or Receiving of Remains, Direct Burial and various alternative dispositions for limited services. Each category offers a different level of use for our staff, facilities and equipment. You may choose only those services you want directly from the General Price List. The consumer’s guide is another way to make things easier for the selecting party to make an informed choice. The Goods and Services shown below are those we can provide to our customers. You may choose only the items you desire. However, any funeral arrangements you select will include charge for our basic services and overhead. If legal or other requirements mean you must buy any items you did not specifically ask for, we will explain the reason in writing on the statement we provide describing the funeral goods and services you selected. Keep in mind that a funeral is a meaningful event and one that properly planned, can help ease the pain of separation that naturally accompanies a death. It is a time of sharing good memories and experiences with friends and family. Our professional staff can help you in coordinating all of the details and will explain all of the options. Our primary objective is to provide a superior service for all clients we serve.

If you want to arrange a direct cremation, you can use an alternative container. Warranties - Our funeral home makes no representations or warranties about the protective value of certain caskets or outer burial containers other than those made by the manufacturer. The only warranties, expressed or implied, granted in connection with goods sold with this funeral service are the written warranties, if any, extended by the manufacturers thereof. No other warranties of merchantability and fitness for a particular purpose are extended by the seller. In most areas of the country, state, or local law does not require that you buy a container to surround the casket in the grave. However, many cemeteries require that you have such a container so that the grave will not sink in. Either a grave liner or a burial vault will satisfy these requirements.

Death Notification Checklist When a family member or friend has died, it is important to notify various government agencies, banks, creditors and credit-reporting agencies. To reduce the risk of identity theft, these notifications should be made promptly after the death. To expedite notification, make initial contact by telephone and follow up with written notification. For many government agencies and financial entities, you will need the decedent’s Social Security number, a copy of the Death Certificate and, if you are a personal representative (executor) of the estate, your appointment form from the probate court. Make sure to retain copies of all notices sent. Below is a checklist of possible agencies and businesses that should be notified of the death. Because each case is unique, the list may not be complete. Also, the funeral home may have notified some of the government agencies on your behalf.
