HEALTHY 4 LIFE The Healthy 4 Life individualized health incentive program is designed to motivate actionable healthy lifestyle behavior change that results in improved health risk status of the participants in the program. The program is self-directed by the participant, but involves the direction and support of the City Health Promotion program. Healthy 4 Life provides a financial reward linked to benefits upon successful completion of the program. Success is determined by measurable changes in health status that occur over a one-year time frame. Alternatives will be provided for those in which it is unreasonably difficult to achieve the standard. Successful participants are rewarded through a subsidized premium for health insurance. All full-time or part-time employees covered by the City health insurance plan are eligible for the program. For additional requirements, eligibility, process, criteria and incentive information, contact the Health Promotion Coordinator. 15-16
