First and foremost — the general insurance policy Everyone living or working in the Netherlands is legally obliged to take out a general insurance policy, which reimburses the most medically necessary healthcare costs. The government decides what is reimbursed. The general insurance policy includes things like healthcare provided by your general practitioner and medical specialist, as well as transport by ambulance, medicines and hospitalisation.

General insurance policies have a term of 1 calendar year. CZ has 3 general insurance policies and they all provide the same kind of reimbursement. However, the reimbursement percentages vary, as do the extra services you receive. The differences between the 3 general insurance policies are summarised on the next page. Which healthcare providers can you use?

Why does CZ make agreements with healthcare providers?

CZ lets you choose between some 45,000 contracted healthcare providers and suppliers. The reimbursement for care from contracted providers is the same, regardless of which CZ general insurance policy you have taken out. To find out which providers we have agreements with, simply visit www.cz.nl/zorgvinden (information in Dutch). You can, of course, use a healthcare provider with whom we do not have a contract, but this will usually mean that your reimbursement is lower.

It is very important to CZ that healthcare providers such as hospitals, pharmacies and physiotherapists meet certain quality requirements. This is why we make agreements about treatments, so we can rest assured that our customers receive high standards. We also make agreements on the prices that healthcare providers are allowed to charge for their services. This helps us to manage healthcare costs and keep the Dutch healthcare system affordable. 5

More extensive cover for your healthcare costs STEP 2

Additional insurance Some costs are not reimbursed under the general insurance policy, or are only reimbursed in part. You can insure these costs separately by taking out an additional insurance package. CZ has a range of additional insurance packages for you to choose from. Our ‘Start’ package has a low premium and insures you against unforeseen healthcare costs in the event of an unexpected incident, such as urgent medical care abroad or dental costs following an accident.

Alternatively, you could take out a more extensive additional insurance package such as ‘Basis’, ‘Plus’ or ‘Top’. These packages include reimbursements for things like physiotherapy and alternative treatment methods, and provide progressively higher amounts of reimbursement. CZ also has several additional insurance packages that have been put together especially to suit your stage of life. Select our ‘Jongeren’ (young people), ‘Gezinnen’ (families) or ‘50+’ package and you will only pay for the things you need at this point in your life. To find out which additional insurance package is right for you, simply visit www.cz.nl/additional-insurance or take a look at the package comparer. Premiums for the additional insurance packages are payable from the age of 18. Co-insured children under 18 do not need to pay a premium and they are given the same package as the parent/carer with the most expensive additional insurance package. The packages have a term of one calendar year, so you can change your package each year if your needs change. CZ will accept you without carrying out a medical assessment first.

STEP 3

Dental insurance The general insurance policy covers dental costs for children up to the age of 18. Once you turn 18, you need to pay these costs yourself, but you can take out dental insurance to cover them. The policies have a term of one calendar year. CZ has two dental insurance packages to choose from — ‘Tandarts’ and ‘Uitgebreide Tandarts’ — and the difference between the two is the maximum amount we will reimburse each year. Both packages also provide reimbursement for orthodontic care. If you take out the ‘Jongeren’ or ‘Gezinnen’ additional insurance package, you do not need to take out separate dental insurance, because CZ already reimburses dental costs under these packages. Anyone can register for the ‘Tandarts’ dental package — CZ will accept you without carrying out a medical assessment first. Before deciding whether to accept your application for the ‘Uitgebreide Tandarts’ dental package, CZ will assess your teeth based on a statement from your dentist.

Here are some tips on how to save on your health insurance premiums: • If your healthcare providers are contracted with CZ, you might want to select the ‘Zorgbewustpolis’, as this policy has the lowest monthly premium. • Think about whether a voluntary deductible would be a good idea for you. If you expect your medical costs to be low, it could work in your favour. However, you need to be able to pay the deductible if you do incur medical costs after all. • You might be entitled to receive the healthcare allowance, which is a contribution towards your healthcare costs through the Dutch tax authorities (‘Belastingdienst’). You can request the healthcare allowance at www.toeslagen.nl (information in Dutch) or by calling the ‘Belasting Telefoon’ on 0800 0543. • If you opt to pay your premiums annually, biannually or quarterly in advance, we will give you a discount of up to 2% per year.

Deductibles — what you need to know What is a deductible? Everyone aged 18 or above and living or working in the Netherlands has a compulsory deductible of €385 in 2017, which is set by the government. The deductible is the amount you have to pay yourself before you start receiving reimbursement and it only applies to reimbursements under the general insurance policy. The exceptions are appointments with your general practitioner, midwifery care, obstetric care, multidisciplinary care, check-ups for kidney and liver donors, donor transport costs in the event of an organ transplant, medical aids on loan and district nursing.

Paying the deductible in instalments If you expect to incur healthcare costs during the year and prefer not to pay the deductible all at once, you can opt to pay it in instalments. This will prevent you suffering a financial setback. To find out more, take a look at www.cz.nl/spread-payment-deductible.

Voluntary deductible You can also opt for an additional voluntary deductible on top of the compulsory deductible. The higher the deductible you choose, the lower your premium will be. The voluntary and compulsory deductibles both apply to the same reimbursements.

Help while on holiday

Premiums

Accidents do happen. If you need medical assistance while on holiday, call the CZ Helpline. A Dutch healthcare expert will give you advice and information, such as whether you should see a doctor and where the nearest hospital is. If you request a European Health Insurance Card (EHIC) at www.ehic.nl (information in Dutch), you will not need to pay upfront any healthcare costs you incur in most countries within Europe. CZ will settle these costs directly with the healthcare provider.

The premium is the amount you pay for your general insurance policy and any additional and/or dental packages you have taken out. The premiums for the different general insurance policies are listed on your quote and in the premium overview. Children up to the age of 18 are co-insured without having to pay a premium or a deductible.

CZ’s additional benefits • If you need to submit bills to CZ yourself, you can do so using the app on your smartphone, online through ‘Mijn CZ’, or by post if you prefer. We will pay out the reimbursement within 10 days. • You can use ‘Best Doctors®’ in consultation with our healthcare advisers. ‘Best Doctors®’ is an international network of specialists who can look at your file and give a second opinion about the best treatment for your illness or condition. • As a CZ customer, you can get a discount on products and services that help you to stay fit and healthy, such as gym membership, glasses and contact lenses. Thanks to CZ’s collaboration with OHRA, you can also get a discount on products like travel, cancellation, life and car insurance. For a summary, visit www.cz.nl/klantvoordeel (information in Dutch). • If you need to know how to arrange care for your condition or you want to know where to find the best place to receive treatment, talk to a member of the CZ Healthcare Team (‘Zorgteam’). All of our healthcare advisers have a medical background and can assess your situation. They will advise you if you are on a waiting list, for instance, or if you would like a second opinion from another doctor. This will help you get to grips with the healthcare system, with a dedicated contact person you can rely on.
