Fully enclosed compartments keep dust, dirt, and debris away from fully organized inventory. Lockable cabinets restrict access to inventory. High-density cabinets multiply storage capacity, save space and maintain organization. Interior drawer dividers provide instant identification of implants. Drawer interiors house up to 120 implant compartments. Adjustable dividers accommodate changing inventory requirements. Labeled plastic dividers separate product compartments. Divider labels are printed with product number, implant description, bar codes, and par levels to simplify inventory control. Compartments configure to accommodate implants from DePuy Synthes, DePuy Synthes Spine, and DePuy Synthes CMF. As inventory needs change, compartments adjust to accommodate the hospital’s needs.

How to Select Storage Cabinetry The most common storage configurations are featured in this catalog. To determine suitable cabinets for your hospital, please use the guidelines below. Keep in mind that future inventory growth will occur and while a hospital may have a set, backup inventory for it may not be maintained. It is better to have extra space in a cabinet than too little. If you have any questions, please call a SIMS team member. Determine what implants will be stored On the next page is a chart of product groups and the number of drawers necessary to store backup inventory for that group. Use the chart to review the product lines that have backup inventory at the hospital (Spine and CMF storage recommendations are on page 9). Determine the number of drawers needed This recommended number is an average number of drawers needed to hold a product group. Total the number of recommended drawers and find a cabinet configuration that meets the drawer requirements. The cabinet configurations in this catalog are organized by number of drawers. (Keep future growth in mind!) Helpful hints Consider cabinet location: – We recommend measuring the hospital’s space restrictions. – Consider that standard drawers extend approximately 25” beyond the front of the cabinet when opened. It is easier to use SIMS when standing in front of the open drawer rather than at the side. – A computer placed on top of a cabinet needs an electrical outlet and network /phone jack. – Consider cabinet height. There may be electrical outlets, telephone jacks or gas valves that cannot be obstructed by a storage cabinet.

How to Set Up and Assemble Storage Cabinets Most cabinet configurations are preassembled. You may only have to lay a countertop surface onto the cabinet. If maintenance or engineering personnel assemble the cabinetry, they may try to assemble them in the engineering department or on the receiving dock. Make sure assembled cabinets will fit into elevators and around corners, en route to their final destination. Assembling a cabinet requires only a few tools and instructions supplied by Stanley Vidmar.

How to Place Implants into Storage Cabinets Following these guidelines and suggestions will simplify a SIMS installation. Hospitals usually have some implants not organized into any product category within their current storage arrangement; find all these implants and separate them into product groups. If you do not, you will spend excessive time later shifting storage compartments and implants while setting up. Most hospitals store their implants by product group in numerical order. Some hospitals include drill bits, taps, and guide wires in the same product group drawers. Others prefer drill bits, taps, and guide wires in separate drawers. Determine your preference before starting.

Create implant compartments by placing a labeled plastic divider in the first drawer slot. Find the implants for that compartment label and place them in front of the divider. Then place the next divider two slots behind the first one. This “every other slot” setup will work for most implants. As you get into longer screws and plates, you will need to adjust the dividers accordingly. Longer implants may take six, seven or more slots between dividers. Drawer handle labels are long, colored labels that describe the product group stored in the drawer. These labels slide into an opening slot on the LEFT side of the drawer pull.

Price List May 2012 Synthes Inventory Management Solution (SIMS) is a partnership program in which DePuy Synthes offers to split the cost of Stanley Vidmar Storage Cabinetry 50/50 with the hospital. Because DePuy Synthes shares the cost of the storage cabinetry, we ask that the hospital sign our 50/50 Partnership Agreement form which validates that only DePuy Synthes instruments and implants will be stored in the SIMS cabinetry. A copy of this Partnership Agreement is found at the back of this catalog. This price list includes product numbers, cabinet descriptions and current pricing. The prices1 show the list price of the cabinets, the 50/50 split, the cost of Set-Up supplies,2 and the total cost to the hospital.

Synthes Inventory Management Solution—50/50 Partnership Agreement.

Fax to: (610) 695-2494 50/50 Partnership Agreement This Agreement is made this _______ day of ____________ , in the year_______ by and between _____________________ (“Hospital”) - Customer Number _______________________ and DePuy Synthes Sales, LLC (”DePuy Synthes”), having a place of business at 1302 Wrights Lane East, West Chester, PA 19380. DePuy Synthes and Hospital are referred to herein as Parties to the Agreement (“Parties”). WHEREAS, DePuy Synthes owns certain storage cabinets which are used exclusively for maintaining, organizing and tracking DePuy Synthes’ products (the “Storage Units”); WHEREAS, DePuy Synthes desires to loan a Storage Unit to Hospital solely for the purpose of maintaining, organizing and tracking DePuy Synthes’ products; NOW THEREFORE, in consideration of the foregoing, and intending to be legally bound, the Parties agree as follows: Cabinet Item Number:_________________________________ Cost to Hospital: $____________________________________ FMV of Cabinet: $____________________________________

8. The hospital will permit DePuy Synthes and its representatives to audit the storage unit inventory during normal business hours. Depreciation and Buy Out 9. DePuy Synthes shall retain sole ownership of the Storage Unit until five (5) years after the sale of the Storage Unit to Hospital. The Storage Unit will be depreciated over a period of five years. (Depreciation schedule furnished upon request.) After five years’ time, the Hospital shall own the Storage Unit. 10. The Party requesting the buy out must be an original party to this Agreement. 11. In the event the Hospital requests a buyout of the Storage Unit, the Hospital shall pay to DePuy Synthes the current depreciated cost for the Storage Unit. In the event DePuy Synthes requests a buyout of the Storage Unit, DePuy Synthes shall pay to Hospital the current depreciated cost for the Storage Unit. 12. For purposes of compliance with Medicare/Medicaid requirements, the fair market value (see above) provided by DePuy Synthes (in terms of the percent of the partnership program) of the SIMS Cabinet may constitute a discount or reduction in price relative to DePuy Synthes products.

Storage Unit Purchase and Set-Up 1. DePuy Synthes and the Hospital will share the list price of the Storage Unit (cabinetry housing the inventory of DePuy Synthes products). 2. The Storage Units will be initially set up and organized by DePuy Synthes.

IN WITNESS WHEREOF, the parties hereto have executed this Agreement effective as the date first written above:

Storage Unit Inventory and Maintenance 3. The Storage Units will house only DePuy Synthes products. 4. DePuy Synthes will assist in the organization and management of the Hospital’s DePuy Synthes inventory at the Hospital’s request. 5. The replenishment and /or increase of the inventory will be the responsibility of the Hospital through Hospital purchase orders. 6. The continued maintenance of the Storage Units shall be the joint responsibility of DePuy Synthes and the Hospital. Any missing Storage Units or components will be the responsibility of the Hospital. 7. Hospital will be responsible for any damage to Storage Unit except what would be considered normal wear occurring through everyday use.
