Over the past few years, we have taken significant steps to respond to the unmet healthcare needs of Cape Cod and our surrounding communities, expanding our geography and diversifying our services. First, we brought our hospice services to Martha’s Vineyard, next, to Plymouth, Bristol, Norfolk and Suffolk counties. Then we

A Return to Earlier Times: Hope HouseCalls The time-honored method of doctors making house calls is enjoying a renaissance, acknowledged as a cost-efficient, medically effective and personalized way to deliver medical services to those with serious illness. Launched in August, Hope HouseCalls is our home-based, primary care service for those with physical or mobility limitations, cognitive impairments or other medical conditions that make it challenging to access primary care on a regular basis. Our primary care physicians provide high-quality, non-emergency medical care and personal attention in the convenience and comfort of the patient’s home. In 2012 donations to Hope HouseCalls helped to fund the start-up costs and build the infrastructure needed to support this innovative program. Hope HouseCalls is especially grateful to Charles and Shirley Rogers for their generous gift. Ongoing community support will help ensure that Hope HouseCalls is able to meet the growing need for this critical service.

Hope Hospice Improves Patient’s Quality of Life When Richard Grace was diagnosed with bone cancer, he was referred to HopeHealth by his doctor at Dana Farber. He thought he was in big trouble because the doctor recommended hospice. But nine months later, he is feeling better than ever. During his treatment at home, Richard was in pain all the time, on so many medications he was falling frequently and having difficulty caring for himself. When he was admitted to Hope Hospice, they suggested he go to McCarthy Care Center (MCC) to stabilize his symptoms. Richard said, “I didn’t want to go to a facility. But after two days, because of their great care, I was able to walk with no pain at all.” Richard couldn’t believe his good fortune in receiving care at a place designed to meet all his individual needs. MCC’s medical staff assessed and managed his medications so that he was able to gain weight for the first time in months. It was the extras that really made the difference, improving his quality of life enough to move to a long-term care facility. Richard said, “Everyone at MCC was so nice to me—they do everything for you, from

Donations Help to Enhance McCarthy Care Center Your donations through the years have helped ensure that patients at McCarthy Care Center (MCC) receive the best possible care. Indeed, more than 1,200 have received care since it reopened in 2008 as a specialized inpatient unit for short-term acute care. And in that time, thousands of family and friends have visited to be by their loved ones. Now we are enhancing MCC in response to thoughtful feedback from patients, families and staff. Enhancements include: • Creation of private space for families for counseling, conversation and retreat • An inviting children’s room for play and play therapy • Improved medication room and staff areas • Much-needed parking expansion With your help, we are updating MCC to answer these significant needs. Construction is set to begin in 2013 and the project is expected to cost close to $1M. Hope Dementia & Alzheimer’s Services Programs Donors like you understand that caring for someone who has Alzheimer’s disease or a related dementia can be a difficult physical, emotional and mental challenge. Your generous support to Alzheimer’s Services of Cape Cod & the Islands, now Hope Dementia & Alzheimer’s Services, provides a wide range of programs for those living with dementia, for healthcare professionals, community agencies and caregivers. This includes a telephone helpline and email support, memory screenings, various support groups, respite grants to provide short-term breaks for caregivers and accredited education for healthcare professionals. • CARES provide an opportunity for people with memory impairment and their care partners to participate in two separate support groups—held simultaneously • Young Onset support group is designed for individuals who have been diagnosed before the age of 65 • Caregiver support groups provide a mutual support system that can help caregivers and loved ones maintain their own health and well-being • Arts & Conversation acts as a gateway to past stories and memories to help stimulate memory and enhance conversation among individuals who have dementia

Hope HouseCalls Provides Specialized Services for Alzheimer’s and Dementia Dementia-related caregiving is unique and stressful. The sooner caregivers connect to services they will be better at coping with the challenges of this disease. This service addresses many of the challenges caregivers experience on a daily basis. In the comfort of home, our specially trained social workers provide a range of services including: • Care plan development

• Communication strategies

• Education regarding dementia and its progression

• Support in development of a long-term care plan

• Behavioral management techniques

• Safety awareness

Whether you attended, bid, walked, plunged or sponsored, you’ve made a difference. Here’s a recap of events: The year started off with a splash as hundreds braved the chilly temperatures on New Year’s Day at the third annual Polar Plunge. The 20th Miles of Memories walk took place in May at six locations, with the warm spring sun shining. Nearly 1,000 walkers helped raise more than $100,000. Thank you to Mark Novota, managing partner of Wequassett Resort and Golf Club, who hosted our second annual Celebrate Summer Memories gala auction. About 170 guests enjoyed a perfect summer evening and helped raise more than $70,000. In October, we honored noted journalist Greg O’Brien at the 11th Reds, Whites & Blues gala held at Willowbend Country Club. Miles of Memories

This organization is with me on the front lines. Through thick or thin, it is such a comfort to know they are there.”

Volunteers Share Personal Reasons to Support HopeHealth Brenda Danovitch – Hope Dementia & Alzheimer’s Services Brenda Danovitch’s warm voice answers the phone at Hope Dementia & Alzheimer’s Services on a weekly basis. Retired from a successful purchasing career, she chose to volunteer for a very personal reason. Her husband Steve, was diagnosed with Alzheimer’s in 2007. The organization’s help proved invaluable as she struggled to cope with this greatest of all caregiving challenges. “I didn’t know anything about the disease or the services available. I came looking for answers, and found them here. They guided me through every step of the way.” With Alzheimer’s Services of Cape Cod & the Islands, now Hope Dementia & Alzheimer’s Services’ help, Steve started going to day care, which provided him with needed structure and Brenda with a much-needed break. As the disease progressed, Brenda attended support groups, which she found invaluable. “Everybody has a story to tell because we’re all in the same boat. You laugh, you cry; it’s the best thing that’s ever happened to me. And now I get to give back!”

Carol Williams – Hope Hospice Carol Williams is the proverbial busy professional woman — she runs a thriving catering service and teaches cooking — while pursuing a supplementary degree in health and nutrition. Yet she also volunteers weekly for HopeHealth. Carol has done everything from patient companionship in long-term care facilities to driving patients to medical appointments to making follow-up calls in the office. She’s been making this a priority for the past five years, ever since her sister, Jean, received Hope Hospice’s services both at home and in McCarthy Care Center. “I am so thankful for the comfort my sister experienced in HopeHealth’s care. I remember looking at her face and knowing she was at peace. I can’t think of anything better to do. I usually leave a patient’s home completely humbled by how grateful everyone is— they all say Carol Williams is grateful to volunteer for HopeHealth

In 2012, 176 volunteers donated a total of 12,162 hours of service to Hope Hospice for a wage equivalent of $273,159. Additional volunteers donated 2,277 hours to support Hope Dementia & Alzheimer’s Services and provide administrative support to Philanthropy and other HopeHealth departments. 13

Helping Families in Times of Stress When Kelly DaSilva-Mafra’s daughter Chloe, was born with severe heart, lung and digestive issues in 2009, she weighed less than two pounds, remaining in intensive care for eight months. When she was referred to Hope Care for Kids, she was on oxygen and many medications and had undergone many surgeries. The strain on the family was huge, especially on Chloe’s older brother, Thiago. “The sick child gets all the attention. Hope Care for Kids saved me,” said Kelly. Hope Care for Kids also helped the parents weather the stress on their relationship and secure needed resources. Kelly said they “saved my sanity, saved my marriage, and saved my kids.” Now, four-year-old Chloe is healthy and medication-free. Thiago is a well-adjusted eight-year-old who adores his sister. And Kelly is inspired to consider returning to school to become a social worker. Kelly added, “If I had to go through it all again, I would, because there was so much joy in it.”

Grant Funding Results in Strong Community Impact In 2012 grants from Elder Services of Cape Cod & the Islands, Cape & Islands United Way, Cape Cod Healthcare, CHNA, Community Foundation of Nantucket and others, made it possible for hundreds of caregivers and their loved ones living with Alzheimer’s disease or a related dementia to participate in CARES—unique support groups held simultaneously to provide respite for caregivers. Hope Dementia & Alzheimer’s Services has offered CARES groups for years, recognizing the different needs for support between the caregiver and their loved one. These grants allow the organization to expand CARES, offering 12 groups per month at locations from Mashpee to Wellfleet and Nantucket.

Hope Palliative Care Launches in November Hope Palliative Care consult service provides relief from pain or symptoms associated with a serious illness — whatever the diagnosis — at any age and stage of an illness. The consults are generally provided in acute care hospitals or clinics, with a physician’s order. The consults can be provided in combination with curative treatment. Hope Palliative Care helps to alleviate a broad range of symptoms such as pain, shortness of breath, fatigue, nausea, loss of appetite, depression or anxiety. It can improve the ability to tolerate certain medical treatments and help people gain the strength to carry on with daily life. Hope Palliative Care launched in November, working in partnership with acute care hospitals, and will be expanding to additional clinical settings during 2013.
