JOSE ALEJANDRO Campaign Statement My national experience includes serving as President (2012-2014) of the National Association of Hispanic Nurses. I led the efforts in which the organization’s transformation saw membership growth, over $600,000 in scholarships and close to $2 million in grants. My contributions to professional associations and case management were recently recognized by my induction as a Fellow of the American Academy of Nursing.

CMSA continues to be the leading organization in case management/care coordination because of its membership and the services they provide to patients throughout the healthcare continuum. I have faithfully served for five years on the CMSA national board of directors; 2009-2010 – Chair, Chapter Presidents Council and 2010-2014 Treasurer. My service to the board included leading the subcommittee, which was charged with redesigning the years assigned to each board of directors’ position; the creation of finance-audit committee; working with the association management team in establishing the budget on a yearly basis; approval of all expenditures above policy amounts and advising the board of directors on financial matters.

If elected as CMSA president, my goals include: 1. 2. 3. 4.

It is an honor to run for President Elect of the Case Management Society of America. I have held the Commission for Case Management Certification since 1992 and my career in case management encompasses physical rehabilitation, managed care, outpatient case management and, Director of Care Management for hospitals within the Cleveland Clinic Health System. It is my privilege to serve as the Secretary of CMSA, I am a member of the board for the Cleveland Chapter of CMSA, and I was on the CMSA Public Policy Committee and currently serve on the CMSA Foundation Board of Directors.

My name is Ellen Fink-Samnick and it is my desire to serve as your Secretary on CMSA’s Board of Directors. The health care industry is experiencing profound growth. Each day brings innovative technologies, new care delivery and reimbursement models, plus diverse populations warranting intervention. Case management is well positioned to facilitate interprofessional care coordination that is quality driven and patient-centered. Yet, as I monitor the developing trends, I am increasingly concerned about the proficiency and sustainability of case management’s transdisciplinary workforce moving forward. The industry’s robust opportunities are a profound reminder of those vital issues yet to be reconciled for case management to evolve: •

Development and consistent implementation of formal education across disciplines, at the baccalaureate, masters’ and doctoral levels

Being a case manager and a member of CMSA has brought me significant growth personally and professionally. I believe that case management is the most integral piece in the health care puzzle. Our involvement in care coordination has been proven to significantly impact the quality of services received and outcomes achieved by our patients. CMSA has been so fortunate in its leadership. Each Board of Directors under the president’s guidance has continued to move our organization forward through the development of numerous initiatives that not only help us to better serve our patients but have brought us recognition so that we are no longer, as quoted by Nancy Skinner, ‘the best kept secret.’

It would be my honor and privilege to serve on the Board of Directors to continue the efforts of current and past leaders. It is also my hope to make our work better known and understood by physicians so that a more collaborative relationship will result. Gaining recognition and acceptance by our physicians will significantly contribute to the improved health and well-being of those we serve. My name is Deborah Gutteridge, and it is with great honor and respect for each of my fellow case managers that I am asking for your support in my bid for the National CMSA Board of Directors. I am proud to have been in the field of case management for the past 28 years. I possess a Master’s of Science Degree in Counseling Psychology, and I am an “in-the-trenches” case manager with daily contact in addressing and advocating for the needs of the persons we serve. I believe the diversity of our practice settings, educational experience, and expertise that make up our organization serve as a catalyst to propel CMSA into the role of the most knowledgeable and respected professional entity assisting patients as they transition through the multiple stages of the healthcare process. I am proud to have been able to serve on our national board in the past as CPC Representative and Vice President. I am hopeful to have the opportunity to serve as Director to bring experience,

creativity, and forward thinking to the position. Allow me to introduce myself, I am Patricia Heinzman and I am running for a Director Position on the CMSA National Board. I will complete my first year on the Board as the CPC Representative in June, this board experience has exposed me to a new facet of case management and non-profit management. Having worked alongside the talented members of the Board and CMI staff, I have gained new skills and experience has given me a renewed sense of the practice of case management. I would like to build on this foundation and use the momentum to serve the CMSA membership in the practice of our profession. This is an exciting time for case managers across the country and the world, as the delivery of healthcare changes on a daily basis. As we are

asked to do more with less, I believe the case managers are the leaders of this movement. If elected as Director, I will work side by side with the new CPC Representative to address the concerns and needs of chapter leadership and members. I believe communication is a key component of leadership and mentoring. EXPERIENCE

Derenda Lovelace has been a Registered Nurse for over 36 years and a Case Manager for 21 years. She has a BSN from the University of Virginia (1978) and MSN from University of Alabama (2008)in Nursing Case Management of Rural Populations.

She has worked in a variety of hospital and community health settings. For the past 21 years, she has worked as a case manager for the Federal government in the Department of Defense (DoD) as well as the Veterans Administration (VA) health care systems serving Tricare beneficiaries and Veterans. From 2007-2010, she served as the chair for the Military/VA CM Conference Planning Committee. After serving three years at the executive level of DoD as the Defense Health Agency Case Management Nurse Consultant (Jan 2011- Mar 2014), she has returned to direct patient care and is an RN case manager at Hunter Holmes McGuire VA Medical Center in the Home Based Primary Care Transitional Care Program.

She is an ANCC Certified Case Manager (since 2005) and a CMSA member since 2004 and is active in the Central Virginia Chapter (has served as Past President, Vice President and Board member) as well as serving on National CMSA Committees (Awards and Conference Planning). She has been a presenter at both the local and National CMSA levels and in 2010, received the National CMSA Award of Service Excellence.

DERENDA LOVELACE Hello CMSA members. I am Derenda Lovelace, an ANCC certified case manager with an advance practice degree in Nursing Case Management (CM). I have been an RN for over 36 years and have spent the past 21 years blazing a CM trail in the Department of Defense (DoD) and Veterans Administration (VA) health care systems serving TRICARE beneficiaries and Veterans. I am very excited for this opportunity to be on the ballot for National CMSA Board Director. I am a published author, innovative award winner, experienced speaker, active CMSA chapter Leader and National CMSA committee member with executive level and frontline CM experience. I am running for CMSA Board because I would like to share my CM expertise, knowledge, leadership abilities and passion with National CMSA.

In March 2014, fueled by a desire to directly impact patient lives and well-being, I voluntarily returned to full-time frontline CM which not only places me back in the direct patient care arena, but also affords me the opportunity to dedicate time and my CM expertise to the National CMSA Board of Directors. I come highly equipped, executively mature, incredibly devoted and definitely ready to serve as a National CMSA Board director. I know I can easily transfer my executive leadership skills and other diverse experiences to benefit the National Executive Board and CMSA members. My goals for CM are to grow and develop a pipeline of well-trained, highly competent case managers and to support and improve formal award programs to recognize, incentivize, and reward case managers for their outstanding contribution to health care.

Laura Ostrowsky, RN, CCM, MUP, is currently the Director of Case Management at Memorial Sloan-Kettering Cancer Center (MSKCC) and the 2012 CMSA Case Manager of the Year. Her program has been nominated by Case in Point for excellence in Utilization Management, Discharge Planning and Transitions in Care. She created a patient advocacy program to assist patients in obtaining access to specialty care despite network restrictions. The program has been featured in articles in Advance for Nursing, The Wall Street Journal, Case in Point, and Case Management Monthly.

Laura has over 30 years of health care experience, including time as a staff nurse, QA Coordinator, Director of UR and QA, followed by directorships in CM at New York Presbyterian (NYP) and currently Memorial Sloan-Kettering Cancer Center. She also spent 3 years in information services at the NYP Network overseeing the selection, acquisition and implementation of an integrated hospital information application for UR, QA, Credentialing and Risk Management at 5 network hospitals. Laura has published articles on Case Management in CMSA Today, Professional Case Management, The Patient Flow Journal and Case in Point. She has presented locally and nationally and In 2014 she was a presenter at several national conferences including the Patient Advocate Institute, Central Logic’s Patient Flow Summit and the Health Plan and Hospital Collaboration Congress.

Laura holds a master’s degree in Urban Planning (MUP) with a concentration in Health Planning and Policy, has been a CMSA member since 2005, and has served on the Board of Directors of the NYC Chap-

Campaign Statement

As Director of CM at Memorial Sloan-Kettering, I have created an award winning case management program which has been ahead of the curve in the areas of transition planning, patient advocacy for access to specialty care and the application of utilization management principles to optimizing case management resources. I have grown the department from an inpatient only CM program in 1999 to one that provides support across the continuum with RN CMs in inpatient, outpatient and the ED, a successful denials and appeals program and a proactive program to assist patients with obtaining authorization for covered out of network care. I am a certified case manager, a CMSA member since 2005 and was the 2012 CMSA Case Manager of the Year. For the last 3 years I have served and continue to serve on the board of directors of the NYC Chapter. I hold a master’s in health planning and policy and am a voluntary guest lecturer to the School of Nursing at the College of New Rochelle where I present an overview of case management to the senior RN students and to the RNs in the RN to BSN program.

CMSA has been pivotal in my career. Case Management is rewarding work but Case Managers sometimes feel underappreciated, misunderstood and overlooked. That is why CMSA is so important. CMSA recognizes our work, provides national recognition for our profession and a platform for sharing knowledge and expertise. Local chapters and our national organization nurture and promote CM professional development. CMSA takes this a step further by publicizing our role nationally, defining us as a profession, educating not only the public but regulators and other healthcare professionals about our role in the delivery of quality, cost effective health care. I truly feel that it is important for me to support this organization to continue to expand on the work we do as advocates and influencers. I want to give back to CMSA by serving on the board and representing case managers on a national level.
