MFA Workplace Apparel Standards

Expectations and Guidelines for Clinical Uniforms As of 5.2013 This document is intended to supplement information regarding MFA Workplace Apparel Standards found in the Employee Handbook and in the Dress Code Policies. In instances in which there appears to be discrepancies between expectations and/or guidelines, documents with the most recent effective dates will override former documents. The Medical Faculty Associates (MFA) clinical healthcare professionals are required to wear a uniform. Clinical healthcare titles for the purposes of the uniforms are defined below as Registered Nurses (RN), Licensed Practical Nurses (LPN), Technologists, Medical Assistants (MA), Phlebotomists, and Technicians. Clinical healthcare professionals working at U.S. locations are required to wear a scrub top and matching scrub pants. A matching warm up jacket is optional. In addition an optional white undergarment/undershirt can be worn under the scrub top but cannot be worn alone. Below outlines additional information. (1) The color of the scrub is defined by the clinic staff’s role/position. RN - Caribbean Blue LPN - Wine Board Certified Technologist - Navy Blue MA - Royal Blue Technician - Pewter (2) Every scrub top and jacket will be embroidered with the employee’s full name and credentials. Nicknames are not permitted. (3) Clinical supervisors/managers may choose to wear scrub uniforms or dress clothes and a white lab coat. (4) The MFA has developed a brand identity standard for the workplace apparel. Caskey Group is the approved apparel line partner for the MFA. Available catalogs can be provided by your supervisor/manager or can be picked up from the MFA Marketing Department. Initial fitting must be performed by a Caskey Group representative. The apparel will be embroidered and is not returnable. After your personal fitting, future orders may be placed at http://mfaclothing.caskeygroup.com.

(5) The MFA will purchase the following: New Clinic Hires - Two scrub shirts and two scrub pants at the time of hire - Three scrub shirts, three scrub pants, and one optional jacket at the end of a new hire Introductory Period Full Time Clinic Staff - Five scrub shirts, five scrub pants, and one optional jacket Part Time Clinic Staff - Depending upon the number of days worked per week, two to three scrub shirts, two to three scrub pants, and one optional jacket Note: You need to speak with your supervisor/manager prior to ordering (6) Clinical staff will wear their MFA/GWorld identification badge at all times. (7) Clinical staff will wear clean, closed-toed shoes with their uniform.

If the employee would like to order a different size or style, they will need to put the new size and style on the order form. Please fax all new order forms to (717) 764- 5003.

WHAT TO DO IF THERE IS AN ERROR WITH AN EMPLOYEE'S ORDER Manager/Supervisor should send an email to the following: John Pangborn - [email protected] Cc: Jason Martin - [email protected] Please indicate the employee's name and contact information in the email. They will compare the delivered garment (color, size, style, etc.) with the order form completed by the employee.
