Every year, on the 2nd Thursday of March, we join the world in celebrating World Kidney Day. Our goal is to promote and prevent the progression of kidney disease; to educate and to raise kidney disease awareness in the community. We are pleased that so many have answered our call. Friends from the different LGUs, pharmaceutical industry and several dialysis centers pledged their support in our campaign. The theme for this year is: Kidney Health for All. Different nephrology training institutions already opened booths and displays in their respective hospitals. For this year's celebration, we laid out several exciting activities. To promote the celebration, A 'Water Drinking Challenge' was launched. This was a spin off from the recent 'Ice Water Bucket Challenge'. Just to give everyone a nudge; our way of saying, “Hey, this is happening. Come join us.” We had a Poster-making Contest. This was opened to all --and must illustrate, interpret and emphasize this year's theme. On March 15, 2015, we ended the festivities with bang. We started off with an Alay-Lakad in Makati City Hall at

5:30am and ended at the Makati Coliseum with a Sayaw Zumba. At the end of the celebration, we hope to have reached out to as many people as possible. We hope to have motivated people to change their lifestyle; 1) to keep fit and active, 2) regulate and control their blood sugar and blood monitor, 3) eat healthy, 4) maintain adequate fluid intake and 5) stop smoking. Curbing the current trend of kidney disease is a process that takes time. The difficult part is the commitment and the follow through. We started with setting small goals and taking things one step at a time. We will consider this campaign a success if we were able to save even one kidney. You can access our Facebook page at World Kidney Day Philippines 2015.

OUTCOME BASED EDUCATION IN NEPHROLOGY TRAINING: WHERE ARE WE?

President Philippine Society of Nephrology

by: Lynette Fillone- Alcala, MD, FPPS, DPSN, DPNSP

Dear Colleagues, As I look back, I thank the Lord for sparing the Filipino people from the wrath of nature in the past year. In spite of the uncertainties and loss of trust in our country's leadership, I believe that we Filipinos still need to try to lay down a foundation of trust in those that govern us. We still need to keep on providing our government with feedback by participating in various fora to make our collective voice heard. I thank you for giving your PSN Board of Trustees this year that trust to steer our society in the right direction. The Philippine Society of Nephrology (PSN) envisions itself to be able to attain the highest standards of excellence in training, research and service in the field of nephrology for the nation and for the world. It has mandated itself to ensure the highest quality of patient care and produce, promote and disseminate up to date knowledge, training and research in nephrology. At the core of every specialty is its training program, the source of future specialists of the country. This year the PSN Training Program Accreditation Board (TPAB) approved the re-accreditation of two adult training programs in nephrology (The Medical City and Veterans' Memorial Medical Center). We have received a request to review a proposed pediatric nephrology training program in Davao, which we have endorsed to the Pediatric Nephrology Society of the Philippines (PNSP) for review. It is about time that PSN spreads its wings to the provinces. Our Committee on Continuing Medical Education has revived the Nephrology Forum to help us keep up-to-date on the latest topics in Nephrology. We hope to see a greater number of diplomates and fellows attend and participate in these fora. The CME Committee has likewise conducted the 2nd in-service examinations, this time online. We hope that these innovations help prepare our trainees for their specialty board examinations. We have yet to see the impact of these endeavors on passing rates for the specialty board examinations in the coming years. For the first time last year, we conducted the specialty board examinations in August to allow our trainees some time to review after the end of their fellowship training. Our Research Board also conducted a research workshop for our nephrology trainees to beef up their capacity to conduct quality researches. Our Research Board continues to support the Lipat Kalinga program as a flagship project that aims to smoothen the transition of care of a renal patient who moves from the care of a pediatrician or pediatric nephrologist to the care of an internist or nephrologist. The APKD registry remains a flagship project of the PSN. This year we have partnered with the Philippine Council for Quality Assurance in Clinical Laboratories (PCQACL) to develop a study protocol for the determination of normal ranges for standardized serum creatinine determinations throughout the country. This will pave the way for the standard reporting of eGFR by laboratories in the Philippines. The PSN BOT has approved all requests for travel grants for attendance in

2

international conferences in the past year especially for those presenting a lecture or poster. We are excited about a new partnership at the community level. The PSN has partnered with the College of Nursing of the University of Santo Tomas to adopt Barangay Balucuc in Apalit, Pampanga as our community laboratory to develop and conduct educational modules on the prevention of kidney diseases and to provide continuing care for those at risk for the development of kidney diseases in the barangay. We thank the PSN Central Luzon Chapter for agreeing to assist us in this endeavor. We continue to participate in the activities of the Philippine Medical Association (PMA) and the Philippine College of Physicians (PCP). We continue to work with the Department of Health and Philhealth to improve hemodialysis, peritoneal dialysis and kidney transplant facilities and packages that will benefit our renal patients. Our Committee on Hemodialysis (HD) has been very active this year as they have conducted 4 HD courses for physicians on duty, 1 HD course for nephrologists, 1 HD course for nurses, 1 HD course for dialysis technicians, and the second Hemodialysis Summit, all in the past year. Our Family Day during kidney month last year was well attended and we awarded the first Miss Kidney to a patient with poise, beauty and strength of character while undergoing regular hemodialysis treatments. We thank all the Dialysis Units that participated in this event. We had a very successful conduct of World Kidney Day this year with a glass of water challenge, poster making contest, Alay Lakad and zumba party. This was a partnership with the Makati City government as the culminating activities were held in the Makati City Colisseum. Our Facebook account for World Kidney Day had a reach of over 10,000 people. For all these endeavors, we continue to receive support from our media and pharmaceutical partners, partners in the campaign for the prevention of kidney diseases. The various PSN chapters have also had an abundant share of activities that help promote kidney health for our patients. On the international scene, we have submitted a bid to host the Asia Pacific Congress in Nephrology in 2018. We continue to be represented in the International Society of Nephrology and the Asia Pacific Society of Nephrology. We have recently contributed to the development of a Handbook of Kidney Diseases sponsored by the ISN. We culminate the year with our annual convention with the theme, “Cutting edge nephrology.” It is a fitting way to end a busy year. Time just flew. I thank the officers and members of the PSN Board of Trustees and all those Committee heads and members who continue to serve the PSN selflessly and who have traveled with me in this one year stint. Mabuhay ang PSN! Volume 11, Issue 1 - April 2015

ASEAN integration in 2015 has compelled the country to adapt to drastic changes in education. It will open our doors to foreign students, align our opening of classes to international standards and revise our curriculum to be at par with our ASEAN neighbors. With the adaptation of the K -12 Curriculum in the elementary level and cascading all these changes in the senior high school, baccalaureate and higher education degrees, as well as fast-tracking the Doctor of Medicine degree to be outcome based , are we in the different training programs like NEPHROLOGY ready to accept these batch of trainees? The Commission on Higher Education, Office of Programs and Standards Development, Technical Panel for Health Professions Education conducted its National Public Hearing /Consultation on the Policies, Standards and Guidelines for the Doctor of Medicine Program last December 19, 2014 at the Philippine Medical Association (PMA) auditorium in Quezon City gathering all thirty eight (38) medical schools in the country discussing the different guidelines in the revision of the curriculum. All private higher education institutions (HEIs) with an existing Doctor of Medicine program are required to shift to an outcome –based approach as mandated by Republic Act No. 7722 as stipulated under CMO No. 46 series of 2012. Several institutions of higher learning have already adjusted their opening of classes to August 2015 this year and the rest of the colleges and universities by 2016 in preparation for this reform. The Commission of Higher Education and the Philippine Regulation Commission (PRC) will soon implement the changes in the medical board examination schedule and the transfer of internship program to July. With the last batch of 4th year high school students graduating this year and the next batch pursuing senior high school the following year there is no other way to go but follow the rest of our ASEAN neighbors.

Nephrology has been in the forefront of medical advances in so many years. Its basic anatomy remained the same, looks simple by gross anatomy - 2 kidneys, 2 ureters a, bladder and urethra but physiologically and biochemically the most challenging. Advances in podocytopathy is one of the exciting fields in Nephrology that clearly explains the mechanisms of the disease making treatment targeted. The proteonomics of podocytes actually helps us in the detection of the disease. These early markers helps us to decide the diagnosis -short of the gold standard which is kidney biopsy. Standardization of the renal biomarkers are now published and it has really one of the most exciting things in nephrology. Because of a lot of advancements in the

The question remains are we in the different training programs in the country ready to embrace this reality that soon we have to sit down and relook , converse and discuss, define and redefine the competencies and refine the outcomes of our curriculum as we set forth to open our door to the rest of Asia ? As we align the Philippine Qualifications Standards with our ASEAN neighbors and the rest of the world, are we ready to defend our graduates and place them in the proper bracket of qualifications? We need to find the answer to these questions , we need educators in our ranks who will share their valuable time to take a closer look at our curriculum and standardized our tools of assessment and evaluation to be worthy of being a significant training ground of valuable and well-equipped manpower in Asia. The time to evaluate the need for change is now--- for the changes are now a reality.

field of Nephrology – the trust of the PSN in the last year was to uplift the training program. It has revived the Nephroforum ,on line service examination was instituted and research training was enhanced. With the multi-media in our hands dissemination of such advances will not be a problem. We must continue to have an open mind and heart to be able to pick up the good things and reject information that is of no use to us. Empowerment... Discernment... Enlightenment... PSN-- no other way but to move forward.

The Department of Health, thru the Bureau of Health Facilities and Services, conducted a series of meetings last May 27-29, 2014 with hemodialysis stakeholders at the Widus Convention Center in Clark Freeport Zone in Pampanga. The NCR-Metro Manila group convened on May 29, and subsequent meetings with delegates from other regions were planned. This was part of the bureau's continuing effort to improve and strengthen the implementation of the agency's administrative orders, namely “New Rules and Regulations Governing the Licensure and Regulation of Dialysis Facilities in the Philippines” (A.O. 2012-0001) and “Implementing Guidelines in the Analysis, Monitoring and Maintenance of Water Used in Dialysis Facilities Pursuant to Administrative Order 2012-0001” (A.O. 2013-0003). As reported by the Renal Disease and Control Program (REDCOP), a listing of dialysis facilities in 2010 showed a

The event aimed to evaluate the status of the actual implementation of aforementioned standards. The administrative orders were reviewed with the delegates; a summary on the common deficiencies and violations during inspection and monitoring was reported. An open forum followed wherein stakeholders-doctors, nurses, technicians, unit owners and operators- raised issues and conferred with bureau representatives. Atty. Rodel C. Flores, Head of the Licensing Division of the BHFS, said “ We hope that we can be partners in this undertaking and tackle the issues and concerns with regards to the operations of HD facilities, so we can help our patients further.”
