W h e n the i n c r e a s e in food p r o d u c t i o n d u r i n g the last 10 y e a r s is a n a l y s e d in r e l a t i o n to demographic c h a n g e s , it appears that the quantity of food produced in d e v e l o p i n g countries has increased annually by 2.6 per cent., which barely keeps pace with the population growth. Even more important than the average quantity of food available per caput is its unequal d i s t r i b u t i o n among the d i f f e r e n t socio-economic groups of the p o p u l a t i o n . These facts explain the great m a g n i t u d e of the problem of calorie and p r o t e i n d e f i c i e n c y in the w o r l d . goitre

Endemic goitre is a deficiency disease which occurs in people of all a g e s . The largest goitres and the m o s t serious complications are commonly found in adults, but the p a t h o l o g i c a l changes begin in childhood. Iodine deficiency constitutes the m a i n etiological factor of endemic goitre, but goitrogenic substances and genetic factors m a y also play a r o l e . Endemic goitre affects m o s t of the d e v e l o p i n g countries, but its p r e v a l e n c e considerably, both between and within countries.

The main methods used are anthropometry, clinical examination and biochemistry, within the c u l t u r a l a n d s o c i o - e c o n o m i c context of the p o p u l a t i o n . They are usually c o m p l e m e n t ed by food c o n s u m p t i o n s u r v e y s c o n d u c t e d w i t h the c o - o p e r a t i o n of F A O . Some standardization of methods has been obtained

following two WHO

During the last few years further advances have been made which will permit comparative studies among different countries and systematic surveillance of the population. WHO has already standardized for computer processing the forms used in a n t h r o p o m e t r i c studies• It will also standardize the forms and codes used in other surveys. A successful attempt at doing this has already been made in East Africa by WHO staff. In N o v e m b e r 1971 a m e e t i n g o r g a n i z e d by P A H O / W H O t o o k p l a c e in B u e n o s A i r e s discuss this subject.3 It w i l l b e f o l l o w e d b y t r i a l s i n t h e f i e l d .

From the beginning it w a s recognized t h a t , in view of the high prevalence and the severity of PCM and the limited availability, the high cost and the economic restraints on the increased production of t r a d i t i o n a l p r o t e i n foods (milk, m e a t , eggs and f i s h ) , there w a s little l i k e l i h o o d that these needs could be met from such sources.
