EVACUATION PROCEDURES AND FIRE PROTECTION SYSTEMS TRAINING Written procedures for evacuation and fire safety of the Fire Code of NYS facility shall be developed and made readily available to 404.1 employees and a copy submitted to the code official. Comments: All employees shall be trained in the proper use of fire protection equipment and evacuation procedures. Evacuation Fire Code of NYS drills shall be conducted in accordance with sect. 405 and 405, 406 records shall be kept. Comments: EXITING PROVISIONS All exit components shall be maintained clear and Fire Code of NYS unobstructed at all times. 1011.3 Comments: Locks and latches. Egress doors shall be readily openable Fire Code of NYS from the egress side without the use of a key or special 1012.2 knowledge or effort. FIRE – RESISTANCE –RATED CONSTRUCTION V108 Fire –Resistance-rated Construction, Maintenance The required fire-resistance rating of fire-resistance-rated construction (including walls, fire stops, shaft enclosures, partitions, and floors) shall be maintained. Such elements shall be properly repaired, restored or replaced when damaged, altered, breached or penetrated. Openings made therein for passage of pipes, electrical conduit, wires, ducts, air transfer openings, and holes made for any reason shall be protected with approved methods capable of resisting the passage of smoke and fire. Self-closing or automatic closing doors of approved construction meeting the fire protection requirements for the assembly shall protect openings through fire-resistance-rated assemblies. Comments: V109 Opening protectives; Opening protectives shall be maintained in an operative condition with NFPA 80. Fire doors and smoke barrier doors shall not be blocked or obstructed or otherwise made inoperable. Fusible links shall be replaced promptly whenever fused or damaged. During the period that such device is out of service for repairs, the door it operates shall remain in the closed position. STORAGE; MISCELLANEOUS COMBUSTIBLE MATERIAL STORAGE Storage in buildings. Storage of combustible materials in buildings shall be orderly. Storage shall be separated from heaters or heating devices by distance or shielding so that ignition cannot occur. Comments: Ceiling clearance. Storage shall be maintained 2 feet or more below the ceiling in non-sprinklered areas of buildings or a minimum of 18 inches below sprinkler head deflectors in sprinklered areas of buildings. Comments: Means of egress. Combustible materials shall not be stored in exits or exit closures. Comments: Equipment rooms. Combustible material shall not be stored in boiler rooms, mechanical rooms or electrical equipment rooms. Comments: Attic, under-floor and concealed spaces. Attic, under-floor, and concealed spaces used for storage of combustible material shall be protected on the storage side as required for 1-hour fire-resistant construction. Openings shall be protected by assemblies that are self-closing and are of a noncombustible construction or solid wood core not less than 1.75 inches in thickness. Storage shall not be placed on exposed joists. Comments: ELECTRICAL SYSTEMS AND EQUIPMENT Working space and clearance; A working space of not less than 30” in width, and 36” in depth and 78” in height shall be provided in front of electrical service equipment. Where the electrical service equipment is wider than 30”, the working space shall not be less than the width of the equipment. No storage of any materials shall be located within the designated working space. Comments: Extension cords and flexible cords shall not be a substitute for permanent wiring. Extension cords and flexible cords shall not be affixed to structures, extended through walls, ceilings, or floors, or under doors or floor coverings, nor shall such cords be subject to environmental damage or physical impact. Extension cords shall be used only with portable appliances. Comments: Unapproved conditions; open wiring Open junction boxes and open-wiring splices shall be prohibited. V122 Abatement of electrical hazards. Identified electrical hazards Fire Code of NYS shall be abated. Identified hazardous electrical conditions in 605.1 permanent wiring shall be brought to the attention of the code official. Electrical wiring, devices, appliances and other equipment that is modified or damaged and constitutes an electrical shock or fire hazard shall not be used. Comments: PORTABLE FIRE EXTINGUISHERS V123 Fire extinguishers shall be installed, maintained, and serviced Fire Code of NYS annually as per NFPA 10 and Sect. 906 of the NYS Uniform 906.2 Fire Prevention Code. These locations shall be along normal paths of travel, unless the code enforcement official determines that the hazard presented indicates the need for placement away from normal paths of travel. In rooms or areas in which 906.6 visual obstruction cannot be completely avoided, means shall be provided to indicate the location of the fire extinguisher. Shutoff valves shall be located in Fuel Gas Code of places so as to provide access for operation and shall be NYS installed so as to be protected from damage 409.1.3 Comments: Fuel Gas Code of V129 Equipment shutoff valve. Each appliance shall be provided NYS with a shutoff valve separate from the appliance. The shutoff 409.5 valve shall be located in the same room as the appliance, not further than 6 feet from the appliance, and shall be installed upstream from the union, connector or quick disconnect device it serves. L.P. GAS CONTAINERS AND TANKS V130 Protection of containers. Containers shall be stored within a suitable enclosure or otherwise protected against tampering. Vehicular protection shall be provided as required by the code enforcement official. Comments: V131 Storage of portable containers of 1,000 lbs. Or less, weather filled, partially filled or empty, at consumer sites or distributing points, and for resale by dealers or resellers shall comply with sections 3809.2 through 3809.15 Comments: V132 Prohibited Use of LP-Gas: Unapproved equipment. Liquefied petroleum gas shall not be used for the purpose of operating devices or equipment unless such device or equipment is approved for use with L-P-gas. Comments: V133 Prohibited Use of L-P gas. Balconies: L-P gas containers with a water capacity of greater than 2.5 pounds shall not be stored on balconies. L-P gas burners having and LP-gas container with a water capacity greater than 2.5 pounds shall not be located on balconies or with in 10 feet of combustible construction. Comments: FUEL OIL SYSTEMS V134 Fuel oil storage systems shall be installed in accordance with this code and shall be installed as per the NYS Mechanical Code. Comments: V135 Above ground storage tanks shall rest on structurally sound, noncombustible supports. Where connected to fuel oil storage allowed outside above ground without additional protection shall be 660 gallons. The storage of fuel oil above ground exceeding quantities exceeding 660 gallons shall comply with NFPA 31. Comments: V136 Maximum inside fuel oil storage. Where connected to a fuel oil piping system, the maximum amount of fuel oil storage inside any building shall not exceed 660 gallons. Where the amount of fuel oil stored inside a building exceeds 660 gallons, the storage area shall be in compliance with the Building Code of New York State. COMMERCIAL COOKING EQUIPMENT V138 Commercial cooking systems. The automatic fire extinguishing system for commercial cooking systems, as required by section 609 of this code, shall be of the type recognized for the protection of commercial cooking equipment and exhaust systems of the type and arrangement protected. Pre-engineered automatic dry- and – wet chemical extinguishing systems shall be tested in accordance with UL 300and listed and labeled for the intended application. Where a duct system is protected by a pre-engineered system and the installed length of the duct exceeds the length tested for the pre-engineered systems, additional engineered fireextinguishing protection shall be provided in the unprotected portion of the duct. Other types of automatic fireextinguishing systems shall be listed and labeled for specific use as protection for commercial cooking operations. The system shall be installed in accordance with this code, its listing and the manufacture’s installation instructions. Automatic fire-extinguishing systems of the following types shall be installed in accordance with NFPA 96 and the referenced standards indicated.. Comments: V139 Hoods grease – removal devices, fans, ducts, and other appurtenances shall be cleaned at intervals necessary to prevent the accumulation of grease. Cleanings shall be recorded, and records shall state the extent, time and date of cleaning. Such records shall be maintained on the premises. Comments: V140 Extinguishing system service. Automatic fire – extinguishing systems shall be serviced at least every 6 months and after activation of the system. Inspection shall be by qualified individuals, and a certificate of inspection shall be forwarded to the code enforcement official upon completion. Comments: Manual system operation. A manual actuation device shall be V141 located at or near a means of egress from the cooking area, a minimum of 10 feet and a maximum of 20 feet from the kitchen exhaust system. The manual actuation device shall be located a minimum of 4.5 feet and a maximum of 5 feet above the floor. Comments:

SPRINKLER SYSTEMS V142 Sprinkler systems shall be maintained and tested in accordance with section 901 (N.F.P.A. 25) Pipe and fittings. Sprinkler pipe and fittings shall be inspected annually from the floor level. Pipe and fittings shall be in good condition and free of mechanical damage, leakage, corrosion, and misalignment. Sprinkler piping shall not be subject to external loads by material either resting on the pipe or hung from the pipe. (NFPA 25 2-2.2) Comments: V143 The storage of materials shall not interfere with the discharge of water from fire sprinkler heads or the activation of a sprinkler head. Comments: V144 Valves controlling water supply shall remain readily accessible and be electronically supervised. Comments: FIRE DEPARTMENT CONNECTIONS V145 Access. Immediate access to the fire department connection shall be maintained at all times and without obstruction by fences, bushes, trees, walls, or any other object for a minimum of 3 feet. Comments: V146 Inspection, testing and maintenance. All fire department connections shall be periodically inspected, tested and maintained in accordance with NFPA 25 Comments: PRIVATE FIRE MAINS, HYDRANTS AND TANKS V147 Inspection, testing and maintenance. Fire hydrant systems shall be subject to periodic tests as required by the code enforcement official. Fire hydrant systems shall be maintained in an operative condition at all times and shall be repaired where defective. Additions, repairs, alterations, and servicing shall comply with approved standards. Comments: V148 Private fire service mains and water tanks. Private fire service mains and water tanks shall be periodically inspected, tested and maintained in accordance with NFPA 25 at following intervals. 1. Private fire hydrants (all types): inspection annually and after each operation; flow test and maintenance annually. 2. Fire service main piping: inspection of exposed, annually; flow test every 5 years. 3. Fire service main piping strainers: inspection and maintenance after each use. Comments:

V149 Obstruction. Posts, fences, vehicles, growth, trash, storage, and other materials or objects shall not be placed or kept near fire hydrants, fire department inlet connections or fire protection system control valves in a manner that would prevent such equipment or fire hydrant from being immediately discernible. The fire department shall not be deterred or hindered from gaining immediate access to fire protection equipment or fire hydrants. Comments: V150 Clear space around fire hydrants. A 3foot clear space shall be maintained around the circumference of fire hydrants except as otherwise required or approved. Comments: FIRE AND SMOKE DETECTION & ALARM SYSTEMS V151 The fire and life safety systems shall be maintained in an operable condition at all times. Service personnel shall meet the qualification requirements of NFPA 72 for maintaining, inspecting and testing such systems. The maintenance, inspection and testing schedules and procedures for fire alarm and fire detection systems shall be in accordance with this section and chapter 7 of NFPA 72 Comments: V152 Single and multiple station smoke alarms. Single and multiple station smoke alarms shall be installed in existing group R occupancies in accordance with sections 907.3.2.2.1 through 907.3.2.2.2 and 907.3.2.2.3 Comments: v153 Local alarm systems shall have a clearly legible sign posted above each fire alarm box stating "LOCAL ALARM ONLY NOT CONNECTED TO FIRE DEPT. Each access aisle shall be provided with signage reading “ No Parking Anytime” Signs shall be permanently installed at a clear height of between 60 inches and 84 inches above grade and shall not interfere with an accessible route from an accessible aisle.
