Confidentiality of Information Temple University values every employee and the protection of employee privacy is very important. Your health records are protected under Federal law: The Health Insurance Portability and Accountability Act of 1996 (HIPPA) requires that health information be kept confidential and not shared with your employer. Your health information recorded through this program is stored on the secure servers of WebMD; Temple University will not have access to your protected health information. The University will only know if you participate in a program and the number of points earned in order to issue your rewards. Total Wellness Program Eligibility: Employees and Faculty members enrolled in one of the University's medical plans, Personal Choice PPO, Keystone Health Plan East HMO or Personal Choice PPO High Deductible plan. Rewards:

$50 reward cards – maximum $200 Premium discount on employee health plan contributions beginning July 1, 2017 and ending June 30, 2018

Total Reward Value: $200 in reward cards + premium discount on employee health plan contribution Plan Year

July 1, 2016 to June 30, 2017 to earn rewards

Total Wellness Program- Earning Rewards Earn up to (4) $50 reward cards per plan year - $200 maximum Earn one of the four $50 reward cards for completing:  Wellness Profile  Biometric Screening  Set a goal using the online health assistant Earn additional reward cards: To earn additional reward cards complete specific milestone and activities for reward points, accumulate 75 points to earn a $50 reward. Premium discount in health contribution: Earn 300 points during the plan year AND complete your Wellness Profile, a Biometric Screening and set a goal using your on-line Health Assistant, you may be eligible for a discount towards your employee health contribution for the plan year beginning July 2017 and ending June 30, 2018 (Premium discounts are based on job classification and collective bargaining agreements).

Wellness Rewards is easily accessible through your member website: www.ibxpress.com. This will access the ibxpress login page. If you have never registered for ibxpress.com, click the Register link. If you have registered for ibxpress.com in the past but do not remember your username and/or password, click the Forgot your username? and/or Forgot your password? links. Healthy Lifestyle Solutions - Fitness Program How it works: 1. Join an approved fitness center (Temple’s Campus Recreation has an approved fitness center) 2. Work out 120 times during a 365-day period (any period of time) (Ex: September 2015 to September 2016) 3. Record your workouts (logbook from Blue Cross or Fitness Center computer print out) 4. Submit your documentation and request reimbursement (Up to $150 reimbursement)

Submit documentation and request your reimbursement (Up to a $150 reimbursement – can include cost of nicotine replacement products or prescription medications) Complete a tobacco cessation program and earn 30 reward points.
