Pancake Breakfast Please join us for a complimentary program “Fundamentals of Estate Planning: Leaving a Legacy or a Costly Mistake” Presented by Omni Kiecker, Kiecker Law Office
One of the most difficult things to do, whether it’s for yourself or your family, is to think about death. No one likes to admit their mortality. It’s difficult. It’s not fun. Frankly, it’s scary for most people. What can be even scarier is planning for what happens to your family in the event of your death. Fortunately, law firms like Kiecker Law Office exist and can help guide you through the process of planning what happens to your stuff, who will care for children or dependents, who will make decisions on your behalf and what decisions will be made in your medical care. Omni will give a brief overview of your options and answer any questions you might have on these topics. Refreshments served & walk-ins welcome Date:

NON DATED Thrive On @ Home Program Reduces Readmissions by 65% and Reaches Triple Aim Earns ‘Spirit of Collaboration’ Award Lifesprk was honored at Augustana Care’s Annual Leadership Conference as the recipient of the honorable ‘Tim Tucker Spirit of Collaboration Award’ from Augustana Care for their vision and proactive guidance approach to keep people out of the hospital through the Thrive On @ Home program. Today the Medicare national average for readmissions within 30 days has dropped to 15.2% but that still represents far too many people experiencing poor outcomes. Many Lifesprk team (L to R): Kris Teigen, reasons are likely, RN, Joel Theisen, RN, CEO, but at the top of Lyn Lais, RN the list are: follow-through with discharge plans, medication management issues, support system changes once home, and early warning signs that exacerbate into emergencies. Augustana Care’s Chief Operating Officer, Tim Middendorf, said “we know we can’t solve the issue of readmission rates alone. That’s why the strategic partnership with Lifesprk has been successful at keeping our clients thriving at home once they are discharged from our communities. It’s the main reason we nominated Lifesprk for the ‘Spirit of Collaboration’ award – no one else has been able to successfully achieve such dramatic results.” Joel Theisen, RN, CEO / founder of Lifesprk added, “We’re honored to be recognized in this way. We believe in building partners-for-life and it has been a tremendous partnership with Augustana Care right from the start.”
