Life Care Consulting Prairie River Home Care is excited to introduce our new Life Care Consultant, Kate Roden.

We are excited to announce our new Executive Director:

Ryan Chies, LNHA

Kate Roden is a Certified Professional Gerontologist. In May 2015, she earned a Master’s degree in Gerontology, the study of aging, from Bethel University. Kate loves working with people, especially the aging population. Her enthusiasm stems from a deep and dear relationship with her late paternal grandparents. As a way of filling the void left by their loss, Kate dedicated herself to a career focused on aging paired with her passion for serving others. She enjoys working with Prairie River Home Care because its values, leadership, and caring philosophies align with her own vision. Kate loves life, and many would describe her as a positive ray of sunshine motivated by serving others. Life Care Consulting: • •

Ryan comes with many years of experience in skilled nursing facilities. He graduated from Gustavus Adolphus College along with the University of Minnesota - Carlson School of Management and is also currently on the Board of Trustees for Care Providers of Minnesota.

NON DATED Thrive On @ Home Program Reduces Readmissions by 65% and Reaches Triple Aim Earns ‘Spirit of Collaboration’ Award Lifesprk was honored at Augustana Care’s Annual Leadership Conference as the recipient of the honorable ‘Tim Tucker Spirit of Collaboration Award’ from Augustana Care for their vision and proactive guidance approach to keep people out of the hospital through the Thrive On @ Home program. Today the Medicare national average for readmissions within 30 days has dropped to 15.2% but that still represents far too many people experiencing poor outcomes. That’s why the strategic partnership with Lifesprk has been successful at keeping our clients thriving at home once they are discharged from our communities.
