SHORT FORM SPECIFICATIONS General The work in Schedule 1 includes the removal of the existing 2 sets of doors at the North Entrance at the North Simcoe Sports and Recreation Centre (NSSRC). The removed door sets are to be salvaged and stored in a place as directed by the NSSRC Manager. The Contractor shall supply and install all materials and equipment required to complete the installation of two (2) sets of biparting automatic commercial sliding doors. The work in Schedule 2 includes the supply and installation of 2 sets of automatic door mechanisms (1 set includes push button apparatus for entering and exiting and the automatic door opener mechanism) for 2 change rooms at the NSSRC. Payment Payment(s) will be subject to a 15% holdback under the Construction Lien Act. Holdback will be reduced to a maintenance holdback of 2.5%, without advertisement, after 45 days. The 2.5% holdback will be released after the expiration of six (6) months (calculated from the time the holdback is reduced to 2.5%), provided there are no deficiencies outstanding within that time period. General Conditions of Contract The Ontario Provincial Standards (OPS), General Conditions of Contact, latest revision, shall be applied to the administration of this project. The successful Bidder shall be required to submit the following documents prior to commencement of any works: a) Current Clearance Certificate from the Workplace Safety and Insurance Board (WSIB). It should be noted that the Town must be in possession of a current clearance certificate from WSIB at all times throughout the duration of the works and the maintenance period; b) Constructor Contract and Contract Pre-Job Checklist Health & Safety Form HS-14 (as attached hereto) together with all Health and Safety related documentation as noted therein; c) Liability and all Risk Insurance Certificate in the minimum amount of $2 million, noting the Town of Midland as an additional named insured; d) A Work Schedule outlining the proposed timing of the works. All works must be completed no later than Friday, September 30, 2016. Technical Specifications All work shall be E.S.A inspected and approved. Shop Drawings designed to the manufacturers specifications are to be submitted to the Town for review and approval prior to installation of the doors. Doors are to be installed to the manufacturers’ specifications.

SHORT FORM SPECIFICATIONS SPECIAL PROVISIONS Remove Existing Doors at NSSRC (Item 1.1) For the price bid, the Contractor shall remove and salvage (to an area designated by the facility manager) the existing two (2) sets of manual doors (and frame, if necessary) on the entrance to the arena change room area at the North Simcoe Sports and Recreation Centre (NSSRC). Supply and Install Exterior Bi-Parting Automatic Commercial Sliding Doors At NSSRC (Item 1.2) For the price bid, the Contractor shall supply and install one (1) set of bi-parting Automatic Commercial Sliding Doors, approximately 10’-6” wide x 96” high, for the exterior north entrance at the North Simcoe Sports and Recreation Centre (NSSRC). Exterior door is to be a 2003 series automatic bi-parting door. The unit is to be a sealed unit with tempered glazing. Full breakout on all panels. Doors are to have panics and auto-lock with a key switch and 2” mid-rails. Anodized Finish is to be Dark Bronze. It should be noted that the door opening is ten (10) feet wide and existing height opening is 7’-2” (86”). The height opening will need to be modified to fit the new door size of 96” high. All modifications would be the responsibility of the Contractor. Restoration of any modified area must be returned to as good as or better than the existing condition. Supply and Install Interior Bi-Parting Automatic Commercial Sliding Doors At NSSRC (Item 1.3) For the price bid, the Contractor shall supply and install one (1) set of bi-parting Automatic Commercial Sliding Doors, approximately 10’-6” wide x 96” high, for the interior north entrance at the North Simcoe Sports and Recreation Centre (NSSRC). Interior door is to be a 2003 series automatic bi-parting door. The unit is to be 6mm tempered single glass. Full breakout on all panels and 2” mid-rails with standard MS locking. Anodized Finish is to be Dark Bronze. It should be noted that the door opening is ten (10) feet wide and existing height opening is 7’-2” (86”). The height opening will need to be modified to fit the new door size of 96” high. All modifications would be the responsibility of the Contractor. Restoration of any modified area must be returned to as good as or better than the existing condition. Supply and Install Automatic Door Mechanisms on 2 change room doors at the NSSRC (Item 2.1) For the price bid, the Contractor shall supply and install two (2) sets of automatic door mechanisms. One set will include the supply and installation of 2 accessible push buttons (one to open the door from each side) and the automatic door mechanism on the door. These automatic door mechanisms are to be installed on 2 change room doors at the North Simcoe Sports and Recreation Centre (NSSRC).
