Dear Friends: Thanks to your generosity in 2013, the Visiting Nurse Association of Somerset Hills completed our 109th year providing home health and hospice services, wellness programs, adult day center services and family support to the communities we serve. Our top priority, as always, was to ensure that our patient’s experience, care and outcomes remained exceptional and exceeded expectations. This was accomplished through the efforts and dedication of our highlyqualified staff, the implementation of quality and service improvement measures, collaboration with community organizations and other providers, and investment in the latest technology. Agency highlights include: • A 12% increase in home health patients served • T he number 1 patient satisfaction rating in patients who would recommend us among our leading competitors • 9 5% of patients utilizing telehealth in-home monitoring remained home and out of the hospital • Implementation of a new scheduling process, part of our transition to a patient-centered care model that emphasizes wellcoordinated, timely care and improved communication • N  ational recognition for our innovative telehealth partnership with Summit Medical Group Your contributions made the success of our work possible during the ongoing period of unprecedented change in our national healthcare system. Along with the increased care demands of an aging population, we continue adapting to regulatory and financial challenges impacting our industry, including reduced Medicare reimbursements and increased competition among nonprofit and for-profit providers. For the VNA, this is a time of opportunity. Home care plays an important role in health reform by providing essential care outside the costly hospital setting, and by helping individuals remain independent and at home. We are grateful for your generous support of the VNA and our mission during 2013. Your help ensures that our full range of health and wellness services can be provided to all those in need, and that all members of our community can benefit from important and innovative programs uncompensated by Medicare or private insurance. Suffering from the debilitating effects of Alzheimer’s disease and other life-limiting conditions, long-time Adult Day Center member, Emily is now a VNA hospice care patient. Emily and her husband, Ben, are both spiritual people and enjoy the warmth and comfort provided through frequent visits from the VNA Hospice Chaplin. Sitting together with Emily, Ben often shares touching family stories and special mementos from their 61 year marriage. Our HOSPICE CARE program provides compassionate, supportive care for patients at the end of life. A specially-trained hospice team works to manage the patient’s pain, ensure their comfort and fill their remaining days with meaning and dignity. Our focus is not only on medical support, but on the emotional, social and spiritual needs of the entire family. Dealing with multiple chronic illnesses and the devastating loss of her mother, Barbara’s health declined. Over 18 months, she endured several hospitalizations and time at a rehabilitation facility. Her recovery continued at home where the VNA provided NURSING, PHYSICAL AND OCCUPATIONAL THERAPY and HOME HEALTH AIDE services. As part of her home care, Barbara was followed on our TELEHEALTH PROGRAM. This program provided additional guidance to help Barbara lose weight, reverse her Type II diabetes and improve her overall health. Our CHRONIC CARE MANAGEMENT and TELEHEALTH MONITORING services help patients who suffer with a serious, chronic illness to remain home and independent, while gaining the confidence and ability to manage their own health. Through remote daily monitoring of patient’s vital signs, the VNA nurse can detect subtle changes in their patient’s condition. Working in conjunction with their physician, this early intervention of potential problems helps reduce the risk of hospital or emergency room visits. As caregiver for his mother, Pat is very familiar with the stress and responsibilities that can overwhelm people managing their own lives while caring for elderly loved ones. Since Pat and his wife both work full time, our Adult Day Center provides an ideal daily care choice for their family. While his mother enjoys the companionship, activities and meals provided at the Center, Pat feels secure knowing she spends her days socializing and safe. Our ADULT DAY CENTER offers frail and chronically disabled adults, including those with mild to moderate dementia, a supportive and engaging environment that enhances their quality of life. Center members gain mental and social stimulation through specially-designed interactive programs, enjoy the outdoors in our beautiful, enclosed garden, and have the opportunity to receive personal care services. In addition, through our RESPITE CARE PROGRAM, primary caregivers can receive much-needed, short-term relief while allowing their loved one to remain home and out of a long-term care facility. Vincent is a 93-year-old widower who joined our Bereavement Support Group several years ago following the passing of his wife of 60 years, a VNA hospice patient. Being part of this group has helped him fill the emptiness felt after losing Philomena. Now, Vincent offers encouragement and insight to newer members with humor and empathy. He has found meaning by helping others through their difficult time of grief. Providing support for family members and caregivers of patients with a chronic or life-limiting illness is an important part of care provided by the VNA. Our BEREAVEMENT SUPPORT GROUP provides comfort to those who, like Vincent, are grieving the loss of a loved one. Family members of chronically ill, elderly and disabled people benefit from our CAREGIVER SUPPORT GROUP. Both groups welcome individuals from the community in addition to family members of patients receiving VNA services.
