Option 1: Supply physicians to the retail “pool”
Physician(s) staff the retail side of LiveHealth Online where any patient in the states(s) the physician is licensed in would have the option of selecting this physician to have their online consultation. More than likely will be a patient that they have never seen before.

Option 2: Branded Storefront for the public on Retail LHO
Storefront branded with the practice information that shows just the physicians in that practice that are available for online consultations to consumers selecting a physician.

Option 3: Practice Edition: A fully customized online practice for your current patients A fully customized ‘online practice’ privately for current patients and/or to the public. Enables the practice to offer a new channel of care for their existing population. It will become an essential part of the Enhanced Personal Health Care provider’s program tools to enhance access to the practice and to support the care management of patients following discharge and with chronic conditions.

Business Scenarios Triage Allow provider to staff their LiveHealth Online practice (instead of “call service” after hours or “front desk staff “during business hours) with a nurse or other staff personnel who can triage patients to appropriate setting, schedule appointments (online or in the office for during business hours), or ‘connect with a doctor ’ a practice MD on call if needed. This function can be used to support practice scheduling and demand management as it allows channeling patients to appropriate point of care (office or online) After Hours Physician Coverage (Activate Retail LiveHealth Online) Provider to activate/turn on LiveHealth Online “retail” physicians or other designated practices to cover office after hours (instead of call service). A summary of information from the LiveHealth Online visit is available to provider the next morning. Online Care for Chronic Conditions (Conduct On-going Chronic Condition Check Points) Following an in-office visit, the PC2 provider schedules and establishes on-going, required follow-up online check points for patients with various chronic conditions via LiveHealth Online. Discharge Coordination (Conduct Post Discharge Follow-up) The care team is aware of a patient discharge via reports from Anthem and contacts patient for a discharge follow-up using LiveHealth Online Specialty Consultations – (Coming in future version) Incorporate specialist physician availability and 3-way video dialog, and specialists to the practice edition model LiveHealth Online is the trade name of Health Management Corporation
