Describe your app Given a choice we all want to be as healthy as possible but managing our health can get complicated … life gets in the way Personify is a simple way of turning your doctor’s advice into lasting health habits Personify connects you with a nurse to get you doing more of the things that keep your health in check You’ll learn how to create lasting health routines that are simple and convenient Your nurse will give you support and keep you accountable Our health and the cost of our care, depends on our behaviour; our day-to-day actions, after we walk out of the health system with our set of clinical recommendations in hand Personify gives patients a way of managing the impact of health conditions on their lives. Personify allows care providers to measure and improve adherence rates and the means to deliver quality care to more patients efficiently.

The patient interface design is based on 3 primary principles:

UX – USER EXPERIENCE Design elements to attract, engage and influence

Internal Motivation– why does this matter to the patient? The entire design of the app is geared towards identifying the personal impact of a condition on the person’s own life and how the activities reduces that impact. Graphically tracking progress serves to reinforce this. Low friction communication: Each day the patient is focused on the to-do list for today. Additional messages can be sent to the nurse through whatever channel the patient happens to be accessing at the time (text, email or app). Based on a relationship: The user experience design is aimed at extending the reach of a trusted relationship with a care nurse. A design that reinforces that your nurse is there to help when needed (is expecting to see your progress) enhances patient engagement.

2. For the patient, the Personify platform provides the means to deal with the impact of a health condition on their life. To turn clinical advice into health habits that last. This is achieved by:

Traditional models of supporting patients in managing their care are not scalable / commercially viable. This effectively means that
